Estimating Hidden Cholera Burden and Intervention Effectiveness.

Cholera remains a significant public health threat in many parts of the world, with differing levels of compliance to intervention strategies and undocumented cases contributing to reservoir contamination with Vibrio cholerae at varying rates alongside reported cases. To address this, we incorporate an inapparent cholera-infected compartment into the iSIR model and equip it with parameters depicting vaccination and compliance levels for water and food sanitation, handwashing, and safe fecal disposal. Our model shows that the bacteria shedding from the inapparent infection can significantly aff

Factors associated with hygiene practices among primary school children in southern Ethiopia.

BACKGROUND: Eight years into the Sustainable Development Goal period, Ethiopia is not on track to achieve good hygiene practices among school children. Ensuring good hygiene practices among primary school children to prevent the spread of communicable diseases remains a challenge in most primary schools in Ethiopia. Therefore, the aim of this study was to identify factors associated with hygiene practices among primary school children in southern Ethiopia. METHODS: A school-based cross-sectional study was conducted from June 3 to July 28, 2022, in five primary schools. The simple random sampl

Effects of skin washing frequency on the epidermal barrier function and inflammatory processes of the epidermis: An experimental study.

BACKGROUND: Increased hand hygiene measures during the COVID-19 pandemic have led to an increased quantum of hand eczema (HE). OBJECTIVES: To examine the effects of varying washing frequencies using current mild cleansing agents-alongside with the effect of a rehydrating cream-on the epidermal barrier function and inflammatory processes of the stratum corneum(SC). METHODS: Standardized skin washings on the volar aspects of the lower arms of skin-healthy volunteers were performed using the automated cleansing device either 5 or 11 times within 4 h for 60 s each with a standard cleanser, a lip

Rapid drinking water safety estimation in cities: Piloting a globally scalable method in Cochabamba, Bolivia.

BACKGROUND: Systematically collected and comparable data on drinking water safety at city-scale is currently unavailable, despite the stated importance of water safety monitoring at scale under the United Nations Sustainable Development Goals (SDGs). We developed a rapid drinking water quality assessment methodology intended to be replicable across all cities and useful for monitoring towards achieving SDG 6 (Clean Water and Sanitation). METHODS: We collected drinking water samples at the point-of-consumption for basic microbial, physical and chemical water quality analysis and conducted hous

Promotion of Cholera Awareness Among Households of Cholera Patients: A Randomized Controlled Trial of the Cholera-Hospital-Based-Intervention-for-7 Days (CHoBI7) Intervention.

Previous studies have demonstrated that household contacts of cholera patients are highly susceptible to cholera infections for a 7-day period after the presentation of the index patient in the hospital. However, there is no standard of care to prevent cholera transmission in this high-risk population. Furthermore, there is limited information available on awareness of cholera transmission and prevention among cholera patients and their household contacts. To initiate a standard of care for this high-risk population, we developed the Cholera-Hospital-Based-Intervention-for-7-Days (CHoBI7), whi

Efficacy of Handwashing with Soap and Nail Clipping on Intestinal Parasitic Infections in School-Aged Children: A Factorial Cluster Randomized Controlled Trial.

BACKGROUND: Intestinal parasitic infections are highly endemic among school-aged children in resource-limited settings. To lower their impact, preventive measures should be implemented that are sustainable with available resources. The aim of this study was to assess the impact of handwashing with soap and nail clipping on the prevention of intestinal parasite reinfections. METHODS AND FINDINGS: In this trial, 367 parasite-negative school-aged children (aged 6-15 y) were randomly assigned to receive both, one or the other, or neither of the interventions in a 2 × 2 factorial design. Mixed-effect models for evaluating multiple measures of atrazine exposure among custom applicators.

The exposure of custom (or commercial) applicators to the herbicide atrazine was measured in environmental (hand wash and dermal patch) and biological (urine and saliva) samples. Surrogate exposure data, such as amount of atrazine sprayed, were also collected. A systematic sampling design was used that included both spray and nonspray days. Fifteen applicators were sampled 5 to 7 days each during a 6-week spring spray season for a total of 89 sampled days. Mixed-effect regression modeling was used to examine the relationship among the surrogate, environmental, and biological atrazine exposure

Hand-touch contact assessment of high-touch and mutual-touch surfaces among healthcare workers, patients, and visitors.

BACKGROUND: Unlike direct contact with patients' body, hand hygiene practice is often neglected by healthcare workers (HCWs) and visitors after contact with patients' environment. Contact with hospital environmental items may increase risk of pathogen transmission. AIM: To enumerate the number of hand-touch contacts by patients, HCWs and visitors with any hospital environmental items. METHODS: All contact-episodes between person and item were recorded by direct observation in a six-bed cubicle of acute wards for 33 working days. High-touch and mutual-touch items with high contact frequencies

[Tetanus following an abrasion injury].

HISTORY AND CLINICAL FINDINGS: A 80 year-old woman contracted an abrasion of her right forearm while gardening. 5 days later, dysphagia, trismus and a cramp of the right hand developed. On admission (8 days after the injury) cramps of the platysma and pharyngeal muscles were observed. A tetanus vaccination had not been carried out for years. INVESTIGATIONS: Electrophysiologically a pathological masseter inhibitory reflex with a missing "silent period" was observed. The level of serum anti-tetanus-toxoid IgG antibodies was 0.03 IU/ml (definitively protective above 0.1 IU/ml). Free tetanus toxi

Short- and long-term effects of handwashing with antimicrobial or plain soap in the community.

Little is known about effects of public use of antimicrobial handwashing soap. A double-blinded, randomized clinical trial of hands of primary caretakers in 238 inner city households was conducted in which effects of plain or antimicrobial (containing 0.2% triclosan) handwashing soap on bacterial counts of the hands were compared before and after a single wash and before and after handwashing following a year of product use. The randomly assigned product was provided without cost to each household during monthly home visits, and compliance with product use was monitored. Households were contac

Behavioral interventions and respiratory virus incidence in a two-cohort study.

Behavioral interventions are a critical tool for managing novel and emerging pathogens. Examples include hand-washing, masking, or staying home when feeling ill. The dynamics of behavioral interventions have been difficult to measure and are poorly understood. This makes it difficult to assess their impact during outbreaks. In this study, we investigate the uptake, persistence, and waning, of behavioral interventions against respiratory viruses among two cohorts in Centre County, PA. We focused on the three years following the emergence of SARS-CoV-2. We detected clusters of behaviors that fol

Child's play: Harnessing play and curiosity motives to improve child handwashing in a humanitarian setting.

In humanitarian emergency settings there is need for low cost and rapidly deployable interventions to protect vulnerable children, in- and out-of-school, from diarrhoeal diseases. Handwashing with soap can greatly reduce diarrhoea but interventions specifically targeting children's handwashing behaviour in humanitarian settings have not been tested. Traditional children's handwashing promotion interventions have been school-focused, resource-intensive and reliant on health-based messaging. However, recent research from non-humanitarian settings and targeting adults suggests that theory-based b
