Biomechanical comparison of concussions with and without a loss of consciousness in elite American football: implications for prevention.

Loss of consciousness (LOC) associated with concussion is no longer considered an indicator of severity of injury in concussion management protocols. Studies investigating the association between LOC and recovery time or neurophysiological performance have reported ambiguous findings and resulted in a limited understanding of the severity of LOC-inducing head impacts. Concussive injuries with and without LOC from helmet-to-helmet and shoulder collisions and falls in elite American football were reconstructed in laboratory using a hybrid III headform and finite element model to obtain peak line

Impact attenuation of male and female lacrosse helmets using a modal impulse hammer.

It has been established that substantial negative changes in neurocognitive function can be observed in a large percentage of athletes who participate in contact sports such as soccer or football, motivating a need for improved safety systems. Head accelerations in men's lacrosse are similar to those in football and female lacrosse players experience high rates of concussions, necessitating better head protection in both sports. Previous studies have sought to evaluate the ability of modern football helmets to mitigate impacts both normal and oblique to the surface of the helmet using a system

Crash sequence based risk matrix for motorcycle crashes.

Considerable research has been conducted related to motorcycle and other powered-two-wheeler (PTW) crashes; however, it always has been controversial among practitioners concerning with types of crashes should be first targeted and how to prioritize resources for the implementation of mitigating actions. Therefore, there is a need to identify types of motorcycle crashes that constitute the greatest safety risk to riders - most frequent and most severe crashes. This pilot study seeks exhibit the efficacy of a new approach for prioritizing PTW crash causation sequences as they relate to injury s

Choosing the Proper Interface for Positive Airway Pressure Therapy in Subjects With Acute Respiratory Failure.

Noninvasive ventilation is an effective treatment for a significant proportion of patients with acute respiratory failure. The success of noninvasive ventilation, however, depends on several factors, a major one being the selection of the proper interface. The choice and application of the interface in patients with acute respiratory failure is a considerable challenge for any treatment team. This review discusses the different types of interfaces that can be used in patients with acute respiratory failure, the differences between nasal, oro-nasal, and total face masks and the helmet, as well

Sport-Related Traumatic Brain Injury with and without Helmets in Children.

BACKGROUND: Soccer, bicycling, and horseback riding are sports most commonly associated with traumatic brain injury (TBI) in Germany. The latter 2 sports activities are commonly practiced with helmets, and data on helmet use and usefulness vary widely. METHODS: On Ethics Committee approval, a retrospective analysis was performed for patients age 5-17 between January 2009 and August 2014 based on a diagnosis of TBI, using the electronic patient file for 2 university hospital locations. Descriptive data analysis and multivariate and univariate logistic regression were used to calculate odds rat

Effect of molding helmets on intracranial pressure and head shape in nonsurgically treated sagittal craniosynostosis patients.

OBJECTIVE Cranial vault expansion is performed in pediatric patients with craniosynostosis to improve head shape. Another argument for performing total cranial vault reconstruction is the potential reduction in the harmful effects of elevated intracranial pressure (ICP) that are associated with craniosynostosis. Alternatively, molding helmets have been shown to improve the cranial index (CI) in patients with sagittal synostosis without surgery. However, it is unknown if the use of molding helmets without surgery contributes to adverse changes in ICP. The effect of molding helmets on ICP and CI

Impact of age, location, and bicycle style on helmet usage by adults.

OBJECTIVE: Our objective was to describe current patterns of bicycle helmet usage in a university community using a direct observation approach. PARTICIPANTS: Cyclists in a university community observed in 2010 and 2011. METHODS: We identified 6 locations in Athens, Georgia, and recorded the type of bicycle, age group, sex, and helmet usage of passing cyclists. We performed univariate and multivariate analysis to identify independent predictors of helmet usage. RESULTS: We observed 433 cyclists: 74 percent male, 74 percent college age, and 45 percent of whom wore a helmet. Helmet usage vari

Mini-invasive surgical technique for sagittal craniosynostosis.

INTRODUCTION: Several techniques are currently available for the surgical correction of sagittal craniosynostosis. The most recently introduced ones have been specifically designed to perform a mini-invasive approach in order to reduce the postoperative morbidity. Herein, the surgical steps of a personal, mini-invasive technique used to decrease the impact of the surgical scar are described. SURGICAL TECHNIQUE: The traditional biparietal skull expansion is realized through two to six short skin linear incisions (2-3 cm long) strategically scattered over the scalp, which allow the surgeon to p

Does helmet therapy influence the ear shift in positional plagiocephaly?

BACKGROUND: Helmet therapy is widely accepted in the treatment of severe positional plagiocephaly. The improvement of the cranial asymmetry under therapy is evident, but parents are also concerned about the ear shift. Our study investigated the influence of helmet therapy on the position of the ears and analyzed the reliability of clinical observations regarding cranial asymmetry and ear shift. METHODS: Three-dimensional stereophotogrammetry of 80 infants with severe positional plagiocephaly was performed before and after helmet therapy. The cranial vault asymmetry index (CVAI) and ear shift

Death by complete decapitation of motorcyclist wearing full face helmet: case report.

We describe a case of complete decapitation following a motorcycle accident in which the victim was wearing a full face helmet. A young man lost control of his motorcycle and was thrown about 20 m, hitting his head against the barrier separating a tramline from the road. The resulting trauma caused his decapitation, the only fatal wound ascertained by the various forensic investigations. The authors present this rare case and compare it against the other two cases reported in the literature, providing some observations on the ways in which this injury can come about. The absence of abrasions o

Head and neck injuries in fatal motorcycle collisions as determined by detailed autopsy.

Detailed layer-by-layer autopsy of the head and neck was performed on a series of seventy-three fatally injured motorcyclists in order to identify occult soft tissue injuries such as subluxation of the cervical spine or hemorrhage of vertebral or internal carotid arteries. The fatal cases were gathered as part of a larger study of 1082 on-scene in-depth motorcycle crash investigations in Thailand. Injuries were coded using the 1990 Abbreviated Injury Scale (AIS 90) and an Injury Severity Score (ISS) was determined for each case. Twenty-eight motorcyclists had been wearing a helmet at the start

Effectiveness of a state law mandating use of bicycle helmets among children: an observational evaluation.

In spring 1999, the authors evaluated the effectiveness of a 1997 Florida law requiring helmet use by all bicyclists younger than age 16 years. Sixty-four counties in Florida had enacted the bicycle helmet-use law, while the other three counties had opted out. Using a cross-sectional study design, the authors conducted unobtrusive observations at bicycle racks at public elementary schools statewide. Florida children riding bicycles in counties where the state helmet-use law was in place were twice as likely to wear helmets as children in counties without the law. In counties where the state la

Analysis of posterior plagiocephaly: deformational versus synostotic.

Appropriate management of posterior plagiocephaly requires differentiation of occipitoparietal flattening caused by lambdoid synostosis from that caused by deformational forces. In a 2 1/2-year prospective study of 115 infants presenting with unilateral posterior cranial flattening, only one child had synostotic posterior plagiocephaly (lambdoid synostosis), whereas 114 infants had deformational posterior plagiocephaly. Deformational occipitoparietal flattening was more common on the right (61 percent) than on the left (30 percent), and minor contralateral frontal flattening was not unusual (5

The effect of the 1992 California motorcycle helmet use law on motorcycle crash fatalities and injuries.

OBJECTIVE: To determine the effects of the California motorcycle helmet use law on statewide fatalities and a large sample of nonfatal injuries before and after law implementation. DESIGN: Police reports and death certificates were collected for motorcycle crash fatalities in California for 1991 (prelaw) and 1992 (postlaw). Official counts of registered motorcycles provided a statewide basis for exposure to a motorcycle crash. Autopsy results were collected for fatalities in 11 counties. Hospital records were reviewed for nonfatal injuries in 28 hospitals in 10 of the 11 counties. Police repo

The Effectiveness of Protective Headgear in Attenuating Ball-to-Forehead Impacts in Water Polo.

Recent reports have demonstrated that there is a serious risk of head impact and injury in water polo. The use of protective headgear in contact sports is a commonly accepted strategy for reducing the risk of head injury, but there are few available protective headgears for use in water polo. Many of those that are available are banned by the sport's governing bodies due to a lack of published data supporting the effectiveness of those headgears in reducing head impact kinematics. To address this gap in knowledge, we launched a water polo ball at the forehead of an anthropomorphic testing devi

Wearing Regulation Soft-Padded Headgear Does Not Reduce the Risk of Head Injuries in Professional Men's Rugby Players: An Observational Cohort Study.

There is no empirical evidence that soft-padded headgear is protective against head injury risk in rugby. However, studies that have assessed purported protective effects have not accounted for rates of contact. The aim of this study was to compare head injury rates while considering tackle-event exposure in players with and without headgear. In the 2018 and 2019 professional men's SuperRugby season, video analysts recorded headgear use, playing position, match time and head injury assessments (proxy for head injury risk) for each player. Tackle-event involvements for each player were obtained
