The payer's role in addressing the opioid epidemic: It's more than money.

BACKGROUND AND OBJECTIVE: County, State, and Federal agencies are addressing the public health opioid crisis. Ohio's 51 county-based Alcohol, Drug Addiction and Mental Health Services (ADAMHS) Boards finance and regulate opioid treatment services within their jurisdictions. This three-year comparative trial collaborated with ADAMHS Boards (n = 14) to test the Advancing Recovery Framework, a suite of organizational and system change strategies designed to promote use of buprenorphine for opioid agonist therapy. METHODS: A multi-level intervention directed payers and treatment agencies to lever

Navigating patient access pathways for non-viral RNA-based therapeutics: a comparative review of health technology assessment and reimbursement pathways across six countries.

RNA therapies are transforming the treatment landscape for several rare, severe, and previously untreatable diseases. Regulatory approval only permits the medicine to be marketed; it does not determine whether a public health system will fund it. In Europe, this typically relies on national reimbursement bodies, following a Health Technology Assessment (HTA) or comparable processes. Despite the clinical promise of RNA therapeutics, they pose a significant challenge for funding decision makers due to their high costs, limited evidence on long-term effectiveness, and often narrow indications. Psychological Adaptation During Prolonged War-Related Stress in Ukraine: A Narrative Review.

BACKGROUND AND AIMS: Civilian populations exposed to prolonged stress, loss, illness, displacement, and other uncontrollable life events demonstrate substantial variability in psychological adaptation. Contemporary research suggests that psychological outcomes are influenced not only by the severity of adversity but also by patterns of cognitive-emotional processing, including locus of control, rumination, self-compassion, and meaning-making. The ongoing war in Ukraine represents a distinct context in which civilians must adapt not after the cessation of trauma, but while recurrent threat, dis

Establishing standard performance measures for adult stroke patients: a nationwide inpatient sample database study.

BACKGROUND: The Agency for Healthcare Research and Quality patient safety indicators (PSIs) and the Centers for Medicare and Medicaid Services hospital-acquired conditions (HACs) are used to evaluate the safety and quality of health care provided by health care systems and individual facilities. To understand better the incidence of PSIs and HACs in hospitalized patients with stroke, we determined the rates of these events among patients with a diagnosis of stroke in the Nationwide Inpatient Sample (NIS) database. METHODS: We queried the NIS for all hospitalizations involving ischemic stroke.

Occupational radiation dose among student radiographers, nuclear medicine technologists, and radiotherapists in an Australian university.

To become registered radiation worker as radiographer, nuclear medicine (NM) technologist, or radiotherapist in Australian health system, a 4-y bachelor's degree, or a 2-y master's degree in medical imaging (MI), NM, or radiation therapy (RT) approved by the Australian Health Practitioner Regulation Agency is required. During their supervised clinical practice period, it is crucial to monitor students' occupational radiation dose as their annual dose limit is 1 mSv y-1 unlike regular occupational dose of 20 mSv y-1. In this study, the distribution and trend of occupational dose among over 300

A strategy to reduce infant mortality.

Using maternal mortality reviews as an historic model, fetal and infant mortality reviews are proposed to reduce infant mortality in the United States. The national program has three elements: 1) guidelines and direction from a national multidisciplinary steering panel and staff, 2) a technical advisory capacity to translate guidelines and to work with local and regional review committees, and 3) local review committees. A special emphasis, lacking in the limited efforts of previous infant mortality reviews, would be given to fetal mortality. The plan proposes a broad classification of potenti

Learning from a multi-agency trauma-informed care training initiative supporting people experiencing homelessness in rural and coastal areas: a qualitative study.

BACKGROUND: People experiencing homelessness have often faced both historical and ongoing trauma, which can be compounded by their interactions with different support services. Trauma-informed care aims to meet the complex needs of people who have experienced trauma and prevent re-traumatisation during service interactions. In rural and coastal areas, where services are often geographically dispersed, multi-agency approaches are particularly crucial for ensuring continuous, coordinated support. This study examines the learning from implementing a multi-agency trauma-informed training pilot for

Diagnostic tests for urinary tract infections and antimicrobial resistance-a reality check between diagnostic innovation and implementation: a critical narrative review.

Urinary tract infections (UTIs) are a significant health concern worldwide, leading to substantial health and economic burden and contributing to the rise of antimicrobial resistance (AMR). Although there has been significant advancement in diagnostic technologies starting from conventional dipstick urinalysis to molecular assays and artificial intelligence (AI)-supported platforms, their clinical utility and impact still remain unclear. This is fundamentally due to the systemic barriers that hinder the implementation including cost, policy, and limitations to integrate into frontline healthca

Responding to A Hazardous Materials Incident in Nepal.

Paper talks about hazardous materials incidents in Nepal that threaten health, safety and the environment. Rapid urbanisation and industrial growth have made chemical, biological, radiological and nuclear emergencies more likely, but there is still not a clear national plan or complete chemical inventory, leaving the health sector unprepared. Emergency responders rely on the Emergency Response Guidebook and the Wireless Information System for Emergency Responders, but many are not aware of these tools or trained to use them. The Nepalese Army has a dedicated chemical, biological, radiological

In Their Own Words: Exploring Family Pathways to Housing Instability.

Housing instability threatens the health and well-being of millions of families across the United States, yet little is known about the characteristics or housing trajectories of at-risk families. To address this gap in our understanding of family risk for housing instability and homelessness, we undertook a qualitative descriptive study utilizing a convenience sample of 16 mothers recruited from a housing service agency living in Detroit and receiving emergency services to avoid homelessness. Participants completed the Brief Patient Health Questionnaire (PHQ), then narrated their life events

Identifying and intervening with substance-using women exposed to intimate partner violence: phenomenology, comorbidities, and integrated approaches within primary care and other agency settings.

Substance use and/or disorders (SUDs) have been identified as a significant correlate of intimate partner violence (IPV) exposure and present complex issues that intersect with the topography of IPV, attendant mental health, and physical co-morbidities and may pose barriers to primary care- and other agency-based screening and intervention efforts. Despite substantial research indicating significantly higher rates of all types and severity of IPV victimization among women with SUDs and bidirectional associations between partner or self-use of drugs or alcohol and IPV victimization, effective s

Telehealth for Contraceptive Care During the Initial Months of the COVID-19 Pandemic at Local Health Departments in 2 US States: A Mixed-Methods Approach.

OBJECTIVES: This study examined implementation of telehealth for contraceptive care among health departments (HDs) in 2 Southern US states with centralized/largely centralized governance structures during the early phase of the COVID-19 pandemic. Sustaining access to contraceptive care for underserved communities during public health emergencies is critical. Identifying facilitators and barriers to adaptive service provision helps inform state-level decision making and has implications for public health policy and practice, particularly in states with centralized HD governance. DESIGN: Mixed-

Transportation-Associated Carbon Dioxide Emissions Avoided by Use of Telehealth Through COVID-19 Pandemic.

Background: Increasing carbon dioxide (CO2) in the atmosphere contributes to Earth's warming, which has negative impacts on human health. The health care system is a major contributor to CO2 emissions. Telehealth has the capacity to reduce health-care-related emissions by eliminating patient travel to in-person appointments. Methods: Data were obtained from Nebraska Medicine's (NM) electronic medical record. Parameters included patient zip code, provider location, calendar year of visit, and provider specialty. Euclidean distance from centroid zip code to clinic location was calculated. Enviro

The second-order effects that the COVID-19 pandemic has had on pediatric populations.

INTRODUCTION: SARS-CoV-2 can have long-term health consequences that persist beyond acute infection. While this is evident in adults and the elderly, the impact on children and adolescents remains under recognized. Here we navigate the second-order post-acute effects that the COVID-19 has had on the pediatric populations, with the exception of the mental health implication of social restrictions. AREAS COVERED: We outline common scenarios related with SARS-CoV-2 infection encountered in pediatric clinical practice, such as in the Multisystem inflammatory syndrome (MIS-C), Long Covid, neurolog

Health system readiness for innovation scale-up: the experience of community-based distribution of injectable contraceptives in Nigeria.

BACKGROUND: Following the successful pilot of the community-based distribution of injectable contraceptives (CBDIC) by community health extension workers (CHEWs) in Gombe, northern Nigeria in 2010, there was a policy decision to scale-up the innovation to other parts of the country. However, there is limited understanding of health system factors that may facilitate or impede the successful scale-up of this innovation beyond the pilot site. Thus, this study assessed the health system readiness to deliver CBDIC in Nigeria and how this may influence the scale-up process. METHODS: This study was

Stigma, Abuse, and Resistance: Unveiling Community Fear of COVID-19 Through Voices of India's Frontline Community Health Workers.

During the COVID-19 pandemic, community health workers (CHWs) worldwide faced violence and abuse. In India, CHWs, known as Accredited Social Health Activists (ASHAs), encountered community resistance and violence in their frontline roles. However, positioned precariously within a neoliberal public health system, their voices remained absent in policymaking, reinforcing structural invisibility. This study, guided by the culture-centered approach (CCA), draws on semi-structured interviews (n = 30) with ASHAs in Odisha, India, to examine their negotiations with community resistance. Findings reve
