Bifascicular block complicating blunt cardiac injury. A case report and review of the literature.

A thirty-five-year-old horse trainer presented to the emergency room of the authors' hospital with minimal nonpenetrating chest injury after having been kicked by a horse. No rib or sternum fractures were demonstrated. The admission ECG demonstrated a right bundle branch block and a left anterior hemiblock that were previously absent. The authors are aware of only two similar reports, but analogous conduction disturbances might have been classified as intraventricular conduction defects. The rarity of these defects may be explained by the anatomic pathways of the bundle of His and its bifurcat

Alternating Wenckebach periods and allied arrhythmias.

Alternating Wenckebach periods (AWPs) are episodes of 2:1 block during which the PR, AH, or AV intervals of the conducted beats gradually increase until a greater degree of block ensues. Most episodes occur at the AV node, but some have also been reported in other structures. AWPs are usually attributed to multilevel block due to transverse (horizontal) dissociation. This assumption was initially based on a method in which the solutions to difficult electrocardiographic rhythms were arrived at by analysis and deduction based on the knowledge existing at that particular time. Subsequently, it w

[Embryological duplications of the node-bundle of His : its pathological rôle].

A histological study by the method of seriated sections in one case of "double-outlet left ventricle" made it possible to confirm and to describe the embryology of the nodal-His junction: not only the atrio-ventricular node (AV N) but also the His bundle (H) have a double origin, atrial and ventricular. Fusion of these two buds, inexistant in the "lone ventricle" is certainly imperfect. Two macroscopically normal hearts provided examples of nodal buds in one case, of a persisting cleft between the two His buds in the other.

Congenital abnormalities of the conduction system in sudden death in young adults.

The sudden death of four apparently healthy young people, three men aged 25, 24 and 21 years, respectively, and one woman aged 20 years, was investigated. The heart was of normal size in two and was slightly enlarged in two. The conduction system in all hearts was serially sectioned. In all hearts the His bundle showed loop formation. In two, the branching bundle was left-sided with the right bundle branch intramyocardial. In the other two, the His bundle was markedly fragmented. Two had myocarditis, and three had arteriolosclerosis of the summit of the ventricular septum. These findings sugge

Atrioventricular junction ablation in patients with conduction system pacing leads: A comparison of His-bundle vs left bundle branch area pacing leads.

BACKGROUND: Single-center studies have shown feasibility of conduction system pacing (CSP) via His-bundle pacing (HBP) or left bundle branch area pacing (LBBAP) in atrial fibrillation (AF) patients undergoing atrioventricular junction ablation (AVJA). OBJECTIVE: The purpose of this study was to compare outcomes in patients with HBP and LBBAP leads undergoing AVJA. METHODS: Consecutive patients with CSP leads referred for AVJA between October 2014 and May 2021 were included. Pacing lead characteristics, procedural characteristics, complications, and long-term outcomes were assessed. RESULTS:

Congenital Heart Block.

Congenital heart block (CHB) is a rare fetal arrhythmia resulting from disruptions in the heart's electrical conduction system, often linked to maternal autoantibodies or congenital heart defects. This article examines the clinical presentation, pathophysiology, and diagnostic strategies of CHB, emphasizing advances in fetal echocardiography, maternal hydroxychloroquine prophylaxis, and multimodal therapies for refractory cases. Management options, including corticosteroids, intravenous immunoglobulin, and pacemaker implantation, are discussed alongside emerging technologies and pacing systems

A case of sick sinus syndrome in primary systemic amyloidosis.

A case of sick sinus syndrome due to primary systemic amyloidosis which involved mainly the heart and lungs is presented. The electrocardiogram showed various changes; low voltage, bradyarrhythmia with junctional escape beats, paroxysmal atrial tachyarrhythmia that terminates abruptly with subsequent long asystole, junctional rhythm from different origins and complete right bundle branch block which appeared at shorter and longer diastolic intervals. Histological examination showed an extensive amyloid infiltration in the upper parts of the conduction system, with major damage in the sinus nod

Desmin variants: Trigger for cardiac arrhythmias?

Desmin (DES) is a classical type III intermediate filament protein encoded by the DES gene. Desmin is abundantly expressed in cardiac, skeletal, and smooth muscle cells. In these cells, desmin interconnects several protein-protein complexes that cover cell-cell contact, intracellular organelles such as mitochondria and the nucleus, and the cytoskeletal network. The extra- and intracellular localization of the desmin network reveals its crucial role in maintaining the structural and mechanical integrity of cells. In the heart, desmin is present in specific structures of the cardiac conduction s

Electrophysiological evaluation of incidental superior vena cava isolation following pulsed field ablation: a case report.

BACKGROUND: Pulsed field ablation (PFA) is increasingly adopted for atrial fibrillation (AF) due to its efficacy and lower risk of complications. However, uniform transmural energy delivery may induce excessive tissue modification and unintended effects on adjacent structures. Pulsed field ablation of the right superior pulmonary vein (RSPV) can result in conduction delay within the superior vena cava (SVC), but its electrophysiological properties remain poorly characterized. CASE SUMMARY: An 81-year-old man with ischaemic cardiomyopathy, chronic heart failure, and paroxysmal AF underwent PFA

Anomalies of cardiac venous drainage associated with abnormalities of cardiac conduction system.

The embryological development of the superior vena cava (SVC) is complex. If the left common cardinal vein fails to occlude it can, along with the left duct of Cuvier form a left SVC, which frequently drains into the coronary sinus. This may result in abnormalities in the anatomy of this structure. A persistent left SVC occurs in 0.5% of the normal population, and 3% to 4.3% of patients with congenital heart anomalies. The pacemaking tissue of the heart is derived from two sites near the progenitors of the superior vena cava. The right-sided site forms the sinoatrial node, the left-sided site

Left Bundle Branch Area Pacing in Congenital Heart Disease: Current Evidence, Challenges, and Future Directions.

Left bundle branch area pacing (LBBAP) has emerged as a promising alternative to right ventricular apical pacing in patients with congenital heart disease (CHD), yet its application in this population remains underexplored despite unique anatomical and physiological challenges. This review synthesizes current evidence on LBBAP in CHD, including anatomical considerations of the conduction system, lead selection between lumenless and stylet-driven leads, technical modifications for different substrates, and clinical outcomes. In congenitally corrected transposition of the great arteries (CCTGA),

The Impact of COVID-19 on the Cardiovascular System.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) induces coronavirus-19 disease (COVID-19), has affected many people in Brazil and worldwide. This disease predominantly affects the organs of the respiratory system, but it also damages the brain, liver, kidneys and especially the heart. In the heart, scientific evidence shows that this virus can damage the coronary arteries, generating microvascular dysfunction, favoring acute myocardial infarction. Furthermore, with the increased expression of pro-inflammatory cytokines, it can lead to myocarditis and cardiac fibrosis, inducing

Effect of scar and His-Purkinje and myocardium conduction on response to conduction system pacing.

INTRODUCTION: Conduction system pacing (CSP), in the form of His bundle pacing (HBP) or left bundle branch pacing (LBBP), is emerging as a valuable cardiac resynchronization therapy (CRT) delivery method. However, patient selection and therapy personalization for CSP delivery remain poorly characterized. We aim to compare pacing-induced electrical synchrony during CRT, HBP, LBBP, HBP with left ventricular (LV) epicardial lead (His-optimized CRT [HOT-CRT]), and LBBP with LV epicardial lead (LBBP-optimized CRT [LOT-CRT]) in patients with different conduction disease presentations using computati

Cardiac involvement in postnatal rubella.

Two cases of rubella myocarditis are reported: a 6-year-old boy who developed a complete, distal to His, atrioventricular block, during the third day of illness; and a 12-year-old boy who developed myocarditis with congestive heart failure 20 days following rubella infection. Although permanent pacing was required in the first patient, six years later he showed a normal growth and maintained normal activity. The second patient has deteriorated markedly and 6 months after the initial illness he had severe heart failure. It should be noted that myocarditis with abnormalities of the conduction sy
