Bioavailability and toxicity after oral administration of m-iodobenzylguanidine (MIBG).

meta-iodobenzylguanidine (MIBG) radiolabelled with iodine-131 is used for diagnosis and treatment of neuroadrenergic neoplasms such as phaeochromocytoma and neuroblastoma. In addition, non-radiolabelled MIBG, administered i.v., is used in several clinical studies. These include palliation of the carcinoid syndrome, in which MIBG proved to be effective in 60% of the patients. Oral MIBG administration might be convenient to maintain palliation and possibly improve the percentage of responders. We have, therefore, investigated the feasibility of oral administration of MIBG in an animal model. Ora

Unusual causes of death in Haitians residing in Miami. High prevalence of opportunistic infections.

Twenty-one (100%) Haitians and 42 (21.5%) of 192 native black Americans autopsied in a 33-month period at Jackson Memorial Hospital, Miami, were included in this review. All autopsied materials were examined. Among the Haitians autopsied, infectious diseases accounted for 11 (52%) of 21 deaths. Toxoplasma encephalitis was the leading cause of death (five cases). Other infectious causes of death included disseminated cryptococcosis (one), disseminated cytomegalovirus diseases (one), Pneumocystis carinii pneumonia (one), chronic active hepatitis B (two), and bacterial pneumonia (one). Malignant

Hamartomas and other tumor-like malformations of the lungs and heart.

Tumor-like malformative lesions are seen throughout the body, and they may be confused with true neoplasms by clinicians and pathologists alike. In the lungs, they are principally represented by hamartomas-which may contain chondroid, adipocytic, fibroblastic, and myxoid tissue, with entrapped bronchiolar epithelium-and congenital pulmonary airway malformations (CPAMs). The latter have been subdivided into 5 groups, based on their histological features, but they basically comprise proliferations of malformed bronchopulmonary tissues of different types. Type 1 lesions have a capacity for malign

Successful detection and surgical treatment of cardiac hemangioma with right ventricular invasion.

Cardiac hemangiomas are extremely rare benign tumors. These tumors are usually asymptomatic but they can present symptoms such as palpitations, shortness of breath, and arrythmia. We report the case of a 73-year-old man who presented with an abnormal shadow on chest computed tomography during follow-up for lung metastatic tumor after resection of his rectal cancer. A cardiac tumor was detected, and surgical resection and right ventricular plasty were successfully performed with the patient under cardiopulmonary bypass. Histopathologic examination revealed a benign cardiac hemangioma, which was

Right atrial metastasis from hepatocellular carcinoma.

A 65-year-old patient with a past medical history of hypertension, alcoholism, micronodular cirrhosis, and coronary artery bypass grafting 10 years ago developed a hepatocellular carcinoma, treated by chemoembolization. One month after treatment, thoracoabdominal CT scan showed no residual hepatic tumor, but tumoral aspect in the right atrium with extension into the inferior vena cava. The patient being asymptomatic, cardiac ultrasound confirmed the presence of a free, mobile, pediculated tumor in the right atrium. Surgical exploration found a well-circumscribed mass, attached to the atrial wa

Association Between Charlson Comorbidity Index Items and Outcomes in Patients With Blunt Trauma Using a Nationwide Trauma Registry Database in Japan.

OBJECTIVE: Understanding a patient's medical history and comorbidities before arrival at a hospital can help select the appropriate medical institution for transport and may be related to outcomes by assisting with treatment after arrival at the hospital. This study aimed to clarify the comorbidities associated with blunt trauma outcomes. METHODS: Using the Japan Trauma Data Bank (JTDB), a nationwide large dataset in Japan, we studied 119,929 cases of blunt trauma among patients admitted to JTDB-participating hospitals from January 2019 to December 2022, excluding those with cardiac arrest be

Availability of home palliative care services and dying at home in conditions needing palliative care: A population-based death certificate study.

BACKGROUND: Avoiding inappropriate care transition and enabling people with chronic diseases to die at home have become important health policy issues. Availability of palliative home care services may be related to dying at home. AIM: After controlling for the presence of hospital beds and primary care physicians, we examined the association between availability of home palliative care services and dying at home in conditions requiring such services. DESIGN: Death certificate data in Japan in 2016 were linked with regional healthcare statistics. SETTING/PARTICIPANTS: All adults (18 years o

Pathologic rupture of the spleen in a patient with acute myelogenous leukemia and leukostasis.

Rupture of the spleen can be classified as spontaneous, traumatic, or pathologic. Pathologic rupture has been reported in infectious diseases such as infectious mononucleosis, and hematologic malignancies such as acute and chronic leukemias. Splenomegaly is considered the most relevant factor that predisposes to splenic rupture. A 66-year-old man with acute myeloid leukemia evolved from an unclassified myeloproliferative neoplasm, complaining of fatigue and mild upper left abdominal pain. He was pale and presented fever and tachypnea. Laboratory analyses showed hemoglobin 8.3g/dL, white blood

Use of mouse models to understand the molecular basis of tissue-specific tumorigenesis in the Carney complex.

Carney complex (CNC) is an autosomal dominant, multiple endocrine neoplasia syndrome comprised of spotty skin pigmentation, myxomatosis, endocrine tumours and schwannomas. The majority of cases are due to inactivating mutations in PRKAR1A, the gene encoding the type 1A regulatory subunit of the 3',5'-cyclic adenosine monophosphate (cAMP)-dependent protein kinase, PKA (protein kinase A). In order to understand the molecular basis for tumorigenesis associated with PRKAR1A mutations, we have developed conventional and conditional Prkar1a knockout (KO) mice as well as primary cell culture models c

Spindle Cell Cardiac Lipoma in an Adolescent: A Case Report and Systematic Review of Cases.

BACKGROUND: Spindle cell lipoma is a benign adipocytic neoplasm that typically arises in subcutaneous soft tissues and is exceptionally rare in the heart. CASE SUMMARY: A 13-year-old male patient was referred for evaluation of a cardiac murmur. Transthoracic echocardiography demonstrated multiple intracardiac masses involving the aortic valve, interventricular septum, and posterior left ventricular wall, resulting in severe aortic regurgitation and moderate aortic stenosis. Cardiac magnetic resonance imaging revealed a large lobulated mass arising from the left coronary cusp of the aortic val

Spotting the clues: cluster of differentiation-a perspective of immune response intertwined with dysautonomia in colon cancer - a prospective cohort study.

Dysautonomia, a parasympathetic-sympathetic imbalance, has clinical and public health consequences. Colon neoplasm is linked to dysautonomia through a complex interplay between the two conditions. In this prospective cohort study, we evaluated 18 patients divided into three groups: six patients with both colon cancer and dysautonomia, six patients with colon cancer without dysautonomia, and six patients with dysautonomia only (control group). Dysautonomia was defined by the presence of orthostatic hypotension, a non-increased or dropped heart rate, and various autonomic symptoms. During abdomi

Parallel Transcatheter Valve and Stent Placement with Subsequent Stent Occlusion to Address a Giant Dysfunctional RVOT in a Patient with Repaired Tetralogy of Fallot.

Tetralogy of Fallot (TOF) is a common cyanotic congenital heart defect whose surgical repair has improved long-term survival. However, many patients develop right ventricular outflow tract (RVOT) dysfunction and severe pulmonary insufficiency (PI), often leading to right ventricular dilatation and arrhythmias. Pulmonary valve replacement (PVR) is the standard treatment. Open surgery remains the preferred option in many cases, especially when the RVOT is large or has a complex anatomy. However, in patients with significant comorbidities that increase surgical risk, percutaneous options may be a
