Sampling rate of heart rate variability impacts the ability to detect acidemia in ovine fetuses near-term.

BACKGROUND: To evaluate the impact of sampling rate on the predictive capability of continuous fetal heart rate (FHR) variability (fHRV) monitoring for detecting fetal acidemia during labor, we tested the performance of the root mean square of successive differences (RMSSD) in R-R intervals from the ECG when acquired with the sampling rate of 4 Hz currently available in FHR monitors, in comparison to the gold standard of 1000 Hz. METHODS: Near-term ovine fetuses (N = 9) were chronically prepared with precordial electrodes for recording ECG, vascular catheters for blood sampling, and an umbili…

Heart rate and heart rate variability in panic, social anxiety, obsessive-compulsive, and generalized anxiety disorders at baseline and in response to relaxation and hyperventilation.

It remains unclear if diminished high frequency heart rate variability (HF-HRV) can be found across anxiety disorders. HF-HRV and heart rate (HR) were examined in panic (PD), generalized anxiety (GAD), social anxiety (SAD), and obsessive-compulsive disorder (OCD) relative to healthy controls at baseline and during anxiety stressors. All disorders evidenced diminished baseline HF-HRV relative to controls. Baseline HRV differences were maintained throughout relaxation. For hyperventilation, PD and GAD demonstrated greater HR than controls. Psychotropic medication did not account for HF-HRV diffe…

Relationship of heart rate turbulence, heart rate variability and the number of ventricular premature beats in patients with mitral valve prolapse and non-significant regurgitation.

Cardiac arrhythmia is frequently detected in patients with mitral valve prolapse (MVP) and improper autonomic tone is considered the etiology. Heart rate turbulence (HRT) and heart rate variability (HRV) are methods assessed for autonomic dysfunction. To evaluate whether the number of ventricular premature contractions (VPCs) is influenced by HRT or HRV in patients with MVP. Holter monitoring for the number of VPCs and correlation analysis with HRT and HRV was performed. There were 31% and 3% with abnormal turbulence onset (TO) and turbulence slope (TS) values in totally 68 patients. No signif…

Simultaneous measurement of instantaneous heart rate and chest wall plethysmography in short-term, metronome guided heart rate variability studies: suitability for assessment of autonomic dysfunction.

Instantaneous heart rate and chest wall motion were measured using a 3-lead ECG and an air pressure chest wall plethysmography system. Chest wall plethysmography traces were found to accurately represent the breathing pattern as measured by spirometry (average correlation coefficient 0.944); though no attempt was made to calibrate plethysmography voltage output to tidal volume. Simultaneous measurements of heart rate and chest wall motion were made for short periods under metronome guided breathing at 6 breaths per minute. The average peak to trough heart rate change per breath cycle (AVEMAX) …

Heritability of cardiac vagal control in 24-h heart rate variability recordings: influence of ceiling effects at low heart rates.

This study estimated the heritability of 24-h heart rate variability (HRV) measures, while considering ceiling effects on HRV at low heart rates during the night. HRV was indexed by the standard deviation of all valid interbeat intervals (SDNN), the root mean square of differences between valid, successive interbeat intervals (RMSSD), and peak-valley respiratory sinus arrhythmia (pvRSA). Sleep and waking levels of cardiac vagal control were assessed in 1,003 twins and 285 of their non-twin siblings. Comparable heritability estimates were found for SDNN (46%-53%), RMSSD (49%-54%), and pvRSA (48…

Heart rate increase results in case of positional venous entrapment.

INTRODUCTION: Tachycardia has previously been reported as a possible sign of neurovascular entrapment during upper-limb abduction and assumed to result from compression of the adrenergic nerve. However, this increase in heart rate could also be caused by a vascular factor, such as venous entrapment. The aim of this study was to determine whether heart rate increases specifically in the case of venous entrapment during upper-limb dynamic mobilization tasks. METHODS: One hundred and sixteen patients were asked to perform a provocative manoeuvre consisting of consecutive upper limb mobilizations…

Radar-Based Heart Rate Estimation Method Under Respiratory Harmonic Interference.

Non-contact vital sign monitoring using millimeter-wave radar has emerged as a promising alternative to contact-based devices for continuous healthcare and elderly care applications. However, accurate heart rate estimation remains challenging because the weak cardiac-induced chest displacement is approximately an order of magnitude smaller than respiratory motion, and its fundamental frequency is frequently masked by higher-order respiratory harmonics. Here we propose a signal processing framework that addresses this challenge through three integrated stages: a slow-time phase correlation meth…

Prediction of Post-Cardiac Arrest Seizures Using Heart Rate Variability.

BACKGROUND: Seizures are common after out-of-hospital cardiac arrest and may exacerbate secondary brain injury, yet early prediction remains challenging. Although heart rate variability has been used to predict seizures in epilepsy, its role and optimal prediction window after cardiac arrest remain unclear. We aimed to develop a heart rate variability-based model for predicting post-cardiac arrest seizures and identify the optimal prediction window. METHODS: This study prospectively collected and retrospectively analyzed data from adults resuscitated from nontraumatic out-of-hospital cardiac …

U-shaped association of resting heart rate with cognitive decline.

Reports of associations between bradycardia and elevated resting heart rate with cognitive decline are inconsistent, and the underlying mechanisms unclear. Resting heart rate was recorded in a prospective cohort of memory clinic subjects, and the associations of resting heart rate with cross-sectional cognition, brain magnetic resonance imaging and circulating biomarkers (pTau-181 and neurofilament light chain), and longitudinal cognitive decline were investigated. Subjects with atrial fibrillation were excluded. Among 643 subjects (mean age 72.8 ± 8.0 years, 57% female, 39% dementia), 35% had…

Heart rate variability (HRV) in juvenile fibromyalgia: a preliminary study.

BACKGROUND: Juvenile fibromyalgia (JFM) is a chronic pain condition affecting children and adolescents, often accompanied by emotional distress. Evidence suggests that autonomic dysregulation, as measured by reduced heart rate variability (HRV), may play a key role in symptom expression. This study aimed to explore clinical, emotional, and physiological differences between children with and without JFM. METHODS: This observational cross-sectional study enrolled 30 participants aged 8 to 17, divided into two equal groups based on JFM diagnosis according to ACR 2010 criteria. Clinical evaluatio…

High Heart Rate, Sympathetic Overdrive, and Cardiovascular Risk in Hypertension.

Elevated resting heart rate values accompanying essential hypertension are frequently detected in current clinical practice. Evidence shows that a heart rate greater than 80 beats/minute is triggered by pronounced sympathetic overactivity with adverse cardiovascular outcomes. These include the development and progression of hypertension-mediated organ damage, such as cardiac hypertrophy, endothelial dysfunction, impaired arterial distensibility, pro-atherogenic vascular alterations, and renal damage. They also include a greater risk of cardiovascular morbid events, such as myocardial infarctio…

Heart Rate Variability Applications in Medical Specialties: A Narrative Review.

HRV is clinically considered to be a surrogate measure of the asymmetrical interplay of the sympathetic and parasympathetic nervous system. While HRV has become an increasingly measured variable through commercially-available wearable devices, HRV is not routinely monitored or utilized in healthcare settings at this time. The purpose of this narrative review is to discuss and evaluate the current research and potential future applications of HRV in several medical specialties, including critical care, cardiology, pulmonology, nephrology, gastroenterology, endocrinology, infectious disease, hem

Low foetal heart rate, a potentially ominous finding: case report.

BACKGROUND: Congenital long QT syndrome (LQTS) type 1 is characterized by abnormally prolonged ventricular repolarization caused by inherite … arted. No arrhythmic events were noted. DISCUSSION: Detection and careful stratification of foetal heart rate (FHR) is crucial in every pregnancy. Foetal bradycardia can be caused by both maternal and foetal factors. Persistent low FHR should raise a high suspicion for LQTS. The condition may also present with atrioventricular blocks, torsades de pointes, o …

Changes in maternal heart rate in delayed post-partum preeclampsia.

AIM: Delayed-onset postpartum preeclampsia (PET) is defined as a new diagnosis of preeclampsia presenting 48 h to 6 weeks postpartum. This disorder is infrequent and associated with a higher incidence of complications as compared to antepartum PET. There seems to be a need to further characterize this disorder. The aim of the study was to investigate the difference of maternal heart rate in women with delayed onset postpartum preeclampsia as compared to healthy controls. METHODS: The medical files of all women who were readmitted with delayed onset postpartum preeclampsia during 2014-2020 wer…

Maternal sleep behaviours preceding fetal heart rate events on cardiotocography.

In Australia, a significant proportion of stillbirths remain unexplained. Recent research has highlighted nocturnal maternal behaviours as potentially modifiable contributors. This study determined whether sleep-related behaviours including sleep position and sleep-disordered breathing adversely affect fetuses overnight, in both uncomplicated pregnancies and those at increased risk due to hypertensive disorders or fetal growth restriction (FGR). All participants underwent polysomnography with time-synchronized fetal heart rate (FHR) monitoring (cardiotocography - CTG) in late pregnancy. CTGs w…
