We report a case of sudden death in a 28-year-old male diagnosed with neurofibromatosis at the age of 6 years. The patient had multiple café-au-lait spots which, although being benign, were an ominous sign in this context. The immediate cause of death was intrathoracic neurofibroma causing compression that led to fracture of the thoracic vertebrae and laceration of the aorta with massive hemothorax.

Blunt or penetrating trauma to the chest can cause several life-threatening conditions: open or closed pneumothorax, each with or without hemothorax; flail chest; pericardial tamponade, and injury to other structures in the chest--the esophagus, trachea or great vessels. Any trauma sufficient to compromise function of thoracic organs must also be suspect for extrathoracic injuries, especially to the spleen and other abdominal viscera.

METHODS: This was a retrospective descriptive study of women with HHT (18-55 years of age) from the Toronto HHT Database using a telephone questionnaire regarding pregnancy, delivery, and neonatal outcomes.

Starting in January 1995, we performed heart transplantation, randomly using standard and bicaval techniques. In the latter technique, the anatomy of the right atrium is maintained, since the venae cavae are anastomosed. OBJECTIVE: To report a case of valproic acid-induced eosinophilic pleural effusion and to review the existing literature.
CASE SUMMARY: A 25-year-old African-American man receiving valproic acid, 250 mg/d, had a moderate-sized right pleural effusion. Pleural fluid analysis revealed 75% eosinophils. The patient had no evidence of parasitic infection, hemothorax, or pneumothorax. One month after valproic acid was discontinued, there was no evidence of a pleural effusion by both chest radiography and thoracic ultrasonography.
