Heparin-induced thrombocytopenia (HIT) is a life-threatening immune response to heparin that is associated with a high risk of thromboembolic complications. We prospectively treated seven subjects with acute HIT with fondaparinux and compared the results to a similar historical control population from the same hospital. We report a case of 46-year-old man who underwent endovascular coil embolization for left anterior inferior cerebellar and posterior inferior cerebellar aneurysms. During embolization of both aneurysms, thrombotic complications were observed along with a relative lack of response to heparin. Intra-arterial abciximab was used to recanalize an almost completely occluded posterior inferior cerebellar artery. Several counterintuitive treatment paradoxes complicate the management of immune heparin-induced thrombocytopenia (HIT). Recently published trials strongly support the use of low-molecular weight heparins in the treatment of unstable coronary syndromes. These agents provide an alternative to heparin that is at least as effective and safe, and does not require intravenous infusions or meticulous monitoring. Although comparative trials are not yet available, the available evidence allows for selection of specific agents in acute management of unstable angina. This review summarizes the current clinical data supporting the use of these agents.

This study investigated the local effects of heparin on locally administered corticosteroid-induced experimental Achilles tendinitis. After performing Achilles degeneration, 28 rats were divided into two groups; one was treated with local heparin and the other with saline injections at 3 and 6-day intervals. Low-molecular-weight heparins (LMWHs) are derivatives of unfractionated heparin (UFH) and have a more favourable pharmacokinetic profile and a more predictable anticoagulant effect than UFH.
