Profound suppression of chronic hepatitis C following superinfection with hepatitis B virus.

BACKGROUND: Hepatitis B virus (HBV) and hepatitis C virus (HCV) share similar transmission routes; thus, coinfection is frequent. The consequences of acute HBV infection in patients with chronic hepatitis C are unknown. METHODS: We describe a 47-year-old male with chronic hepatitis C who acquired HBV and then spontaneously and apparently completely cleared HCV but developed chronic hepatitis B. Five serum samples collected over 14 months and lymphoid cells obtained after acquiring HBV were tested for HCV and HBV by both standard assays and ultra-sensitive polymerase chain reaction/nucleic aci…

Outbreak of acute viral hepatitis due to hepatitis E virus in Hyderabad.

PURPOSE: A waterborne outbreak of viral hepatitis occurred in the old city of Hyderabad from March through August 2005. An attempt was made to study the outbreak clinically, serologically, and etiologically. METHODS: Five hundred and forty-six clinically and biochemically documented cases were screened for the hepatotropic viral markers, hepatitis A, B, C, and E by the ELISA method. Their demographic characteristics and outcomes were analyzed. Point source contamination of the water supply with sewerage was identified. RESULT: Our data confirms hepatitis E as the major cause of the outbreak …

Post-infantile giant cell hepatitis associated with autoimmune hepatitis and polyarteritis nodosa.

We report the case of a patient with corticosteroid-responsive giant cell hepatitis associated with typical manifestations and changes of polyarteritis nodosa from the kidney and central nervous system. Initially, the patient presented with transient right hemiparesis, followed by spontaneous remission without any abnormalities on computed tomography scan, magnetic resonance imaging and cerebrospinal fluid examination. A few months later he was admitted to our clinic because of icterus, peripheral oedema and abdominal distension. He was found to have clinical signs of active cirrhosis. Serolog…

Risk of hepatitis B transmission after amniocentesis in chronic hepatitis B carriers.

OBJECTIVE: To measure the risk of perinatal transmission of HBV in chronic carriers who undergo amniocentesis. METHODS: This was a prospective, longitudinal study from 1990 to 1995 of women who were HBV carriers and underwent amniocentesis. The infants of these women were followed from birth to one year of age. Maternal data examined included HBV antigen and antibody status, liver function tests (LFTs) and the amniocentesis report. RESULTS: Twenty-eight women were identified. Two of 28 neonates were stillborn unrelated to hepatitis. Five infants were lost to follow-up leaving 21 mother-child…

Hepatic histology in hepatitis C virus carriers coinfected with hepatitis G virus.

BACKGROUND: A novel flavivirus has been described recently and designated hepatitis G virus (HGV). The virus is transmitted by the parenteral route but it is uncertain whether it is associated with chronic liver disease because liver biopsy is difficult to justify in this group. AIMS: To examine histological features of liver biopsy in patients infected with hepatitis C virus (HCV) according to the presence or absence of HCV and HGV RNA. METHODS: One hundred and thirty one consecutive HCV carriers undergoing staging liver biopsy were studied retrospectively. In each, HCV RNA and HGV RNA were…

Etiology of chronic hepatitis in France: predominant role of hepatitis C virus.

The aim of this study was to assess the causes of histologically proven chronic hepatitis in a series of 357 consecutively admitted patients. Patients with chronic alcohol intake above 50 g per day, Wilson's disease, idiopathic hemochromatosis or homozygous alpha-1 antitrypsin deficiency were excluded. Sera of all patients were tested for antibodies to hepatitis C virus with second-generation enzyme-linked immunoassay and recombinant immunoblot assay, for markers of hepatitis B and hepatitis D viruses, and for autoantibodies. Detection of hepatitis C viral RNA by polymerase chain reaction was …

Immunogenetic background of hepatitis B virus infection and autoimmune hepatitis in Japan.

Human leucocyte antigen (HLA)-typing was studied in 82 hepatitis B carriers and 15 cases of autoimmune hepatitis (AIH) to elucidate the genetic factors which may associated with the etio-pathogenesis of chronic hepatitis in Japan. There were two types of HLA phenotypes: HLA A2 was associated with the progression of the disease induced by HBV infection, and HLA A26 and DR5 were related to the retardation of the disease. A family study confirmed the strong association of these HLA phenotypes with the disease induced by HBV infection. In the analysis of AIH, HLA DR4 is a risk factor related to su…

Posttransfusion hepatitis revisited by hepatitis C antibody assays and polymerase chain reaction.

Sera of 40 patients with posttransfusion non-A, non-B hepatitis were tested for hepatitis C and B viral genomes by polymerase chain reaction and for hepatitis C antibodies by synthetic peptide immunoassays. Five were then considered to have chronic hepatitis before transfusion. Six patients without hepatitis C markers and hepatitis B virus DNA recovered. In 29 recipients who became positive for hepatitis C virus RNA, posttransfusion hepatitis C was diagnosed. Of them, 5 were hepatitis B surface antigen carriers. Synthetic peptide immunoassays detected 28 whereas anti-C100 assay detected 23 of …

Pathogenesis of an acute viral hepatitis: inclusion body hepatitis in the chicken.

The pathogenesis of inclusion body hepatitis was studied following the oral administration of a serotype 8 strain of avian adenovirus into 2-day-old specific pathogen free chickens. Viral antigens were detected in tissues at various times post inoculation (pi) by enzyme-linked immunosorbent assay and by immunocytochemistry. Viral antigens were detected in intestinal epithelium from 12h to 13 days pi and in the plasma fraction of blood by 24 h pi. A biphasic, cell-free viremia with peaks at 2 and 7 days pi was recorded. Antigens were first detected in the liver from 2 days and reached peak leve…

A hepatitis B virus mutant associated with an epidemic of fulminant hepatitis.

BACKGROUND: A nosocomial outbreak of fulminant hepatitis B occurred in five patients in Haifa, Israel. Previous investigations identified the suspected source as a carrier of hepatitis B surface antigen who was positive for antibodies to hepatitis B e antigen and had chronic liver disease. We examined the strain of hepatitis B virus (HBV) that caused this epidemic, in order to identify specific mutations in the precore or core region. METHODS: The presence of HBV was identified by polymerase-chain-reaction amplification of viral DNA in serum from the source patient, the five patients with ful…

[Evaluation of hepatitis B virus markers for surrogate testing of hepatitis C].

To evaluate the surrogate markers of non-A, non-B hepatitis virus (es) carriage state, the relationship between antibody to hepatitis C virus (HCV) and antibody to HBc/HBs and liver enzyme (ALT) was sought in Saitama prefecture. Out of 36,642 blood donor samples, 410 (1.1%) samples were positive for anti-HCV. Anti-HCV was found in 1.79% of donors with anti-HBs and/or anti-HBc, which is 1.7 fold of the frequency found in donors (1.04%) negative for anti-HBc/HBs. In contrast to the reports from Europe or USA, a poor correlation of anti-HCV with the serological history of hepatitis B infection wa…

[Hematological abnormalities in acute viral hepatitis and acute hepatitis in HBsAg carrier].

Various kinds of hematological abnormalities have been known to occur in liver diseases. To understand the hematological changes in acute viral hepatitis, 324 adults with acute viral hepatitis were studied. Of them, 3 were acute hepatitis A, 91 acute hepatitis B, 99 acute non-A, non-B hepatitis (NANB) and 181 acute hepatitis on chronic hepatitis B (AH on CH-B). There were 233 males and 91 females; age ranged from 16 to 74 years (mean age 39 years.) The results showed the incidences of thrombocytopenia (platelet less than 120,000/cmm), anemia (Hb less than 12 g% in male and less than 10% in fem…

IgM antibody to hepatitis C virus in acute and chronic hepatitis C.

To assess possible role of testing for IgM-specific antibody in the diagnosis and monitoring of patients with hepatitis C, we tested sera from 14 patients with acute and 97 patients with chronic non-A, non-B hepatitis for IgG and IgM antibody to hepatitis C virus. IgG antibody to hepatitis C virus was detected in 93% of acute cases and 91% of chronic cases. Of the 101 patients with IgG antibody to hepatitis C virus, 57% had IgM antibody to hepatitis C virus. None of the 20 healthy subjects or 40 patients with acute or chronic hepatitis A or hepatitis B had IgM antibody to hepatitis C virus. At…

The serology of hepatitis C virus in relation to post-transfusion hepatitis.

Natural history studies conducted over the past 15 years have shown that parenterally transmitted non-A non-B hepatitis infection frequently results in an indolent chronic disease with serious long-term consequences. The recent identification of nucleic acid sequences comprising the genome of hepatitis C virus (HCV) has allowed the development of a serological assay based upon recombinant viral proteins specifically associated with the major agent of non-A, non-B hepatitis infection. The HCV antibody assays have now been applied to sera from blood donors worldwide, as well as various populatio…

Hepatitis in clinical practice. 2. Non-A non-B and delta hepatitis.

Non-A non-B (NANB) hepatitis has at least two distinct causative agents. NANB hepatitis can lead to chronic liver disease, cirrhosis, and hepatocellular carcinoma. No definitive policy exists to exclude this disease as a cause of posttransfusion hepatitis. Delta agent is epidemiologically linked to hepatitis B virus (HBV) infection, either as a coinfection or as a superinfection in chronic hepatitis B surface antigen carriers. Recognition of hepatitis due to delta agent will probably increase as testing for this agent becomes more available and its presence becomes more generally well known th…

Serum hepatitis B virus-DNA in chronic hepatitis B and delta infection.

Hepatitis B-associated delta agent, a defective RNA virus requiring helper functions of hepatitis B virus (HBV), has been shown to interfere with HBV replication. Low titers of serum hepatitis B surface antigen, absence of hepatitis B e antigen, and low levels of stainable hepatitis B core antigen in liver cells usually seen in chronic delta infection are indirect evidences of such an interference. Measurement of serum HBV-DNA by hybridization with phosphorus 32-labeled HBV-clone DNA is the most sensitive method currently available to detect HBV replication. Using this method, we found that on…
