Strengthening the role of community health workers in supporting the recovery of ill, undernourished children post hospital discharge: qualitative insights from key stakeholders in Bangladesh and Kenya.

BACKGROUND: Undernourished children in low- and middle-income countries remain at elevated risk of death following hospital discharge, even when treated during hospitalisation using World Health Organisation recommended guidelines. The role of community health workers (CHWs) in supporting post-discharge recovery to improve outcomes has not been adequately explored. METHODS: This paper draws on qualitative research conducted as part of the Childhood Acute Illnesses and Nutrition (CHAIN) Network in Bangladesh and Kenya. We interviewed family members of 64 acutely ill children admitted across fo

Negotiating access to comprehensive HIV healthcare: Fear, stigma, and health system constraints in Tamale Metropolis, Ghana.

Access to comprehensive HIV healthcare services remains uneven in many low-resource settings, despite the availability of antiretroviral therapy. In northern Ghana, limited qualitative evidence exists on how people living with HIV/AIDS experience, interpret, and navigate access to care within public health facilities. This qualitative study explored factors influencing access to comprehensive healthcare services among people living with HIV/AIDS in public hospitals in the Tamale Metropolis, Ghana. In-depth interviews guided by a semi-structured interview guide were conducted with 30 participan

From diagnosis to treatment: Streamlining TB care pathways in the hilly terrains of Shimla district.

Background & objectives Tuberculosis (TB) remains a significant public health challenge in resource-limited settings like Shimla District, Himachal Pradesh, where delays in anti-tubercular treatment (ATT) initiation persist due to geographic and systemic barriers. This study aimed to evaluate the effectiveness of quality improvement interventions in reducing treatment delays using plan-do-study-act (PDSA) cycles. Methods The study implemented three interventions targeting different operational barriers: decentralising treatment initiation by routing diagnostic results to local TB units, enabli

The Psychopharmacological Revolution in the USSR: Schizophrenia Treatment and the Thaw in Soviet Psychiatry, 1954-64.

Twentieth-century psychiatry was transformed in the 1950s and 1960s by the introduction of powerful psychopharmaceuticals, particularly Chlorpromazine (Thorazine). This paper examines the reception of Chlorpromazine in the Soviet Union and its effect on the Soviet practice of psychiatry. The drug, known in the USSR by the name Aminazine, was first used in Moscow in 1954 and was officially approved in 1955. I argue that Soviet psychiatrists initially embraced it because Aminazine enabled them to successfully challenge the Stalin-era dogma in their field (Ivan Pavlov's 'theory of higher nervous

Barriers to Quality Perioperative Care and Postoperative Mortality Variations in Elective Abdominal Surgery: A Mixed-Methods Study.

BACKGROUND: Perioperative morbidity and mortality significantly affect the healthcare system. Patients undergoing surgical procedures in Latin America have an increased risk of postoperative mortality. METHODS: We used a mixed-methods design to identify barriers to quality perioperative care for elective abdominal surgery in Brazil. We used secondary data from a cohort of 107,372 patients (ExCareBR) to examine postoperative mortality risk factors revealing significant variations in the mortality rates across different hospitals. Using a rapid qualitative assessment method, data were collected

Audit of a new model of birth care for women with low risk pregnancies in South Africa: the primary care onsite midwife-led birth unit (OMBU).

BACKGROUND: South Africa's health system is based on the primary care model in which low-risk maternity care is provided at community health centres and clinics, and 'high-risk' care is provided at secondary/tertiary hospitals. This model has the disadvantage of delays in the management of unexpected intrapartum complications in otherwise low-risk pregnancies, therefore, there is a need to re-evaluate the models of birth care in South Africa. To date, two primary care onsite midwife-led birth units (OMBUs) have been established in the Eastern Cape. OMBUs are similar to alongside midwifery unit

Conflict-Related Disruption of Maternal Health Services: A Case Report of Maternal Death in Transit Due to Obstructed Labour in Somalia.

Maternal mortality remains a major public health challenge in fragile and conflict-affected settings, where insecurity can disrupt referral pathways, skilled birth attendance, and timely emergency obstetric care. This case report describes a maternal death in transit from a remote nomadic community in Mahas District, Hirshabelle State, Somalia, and uses the Three Delays Model to frame the health-system failures contributing to the outcome. A 22-year-old primigravida woman developed prolonged labour at home under the care of a traditional birth attendant. The nearest district health facility ha

Three-Year Recurrence in People With Diabetic Foot Ulcers and Chronic Limb Threatening Ischemia Is Comparable to Cancer.

This study aimed to compare the 3-year recurrence rates of diabetic foot ulcers (DFU) and the rate of endovascular reintervention for chronic limb-threatening ischaemia (CLTI) to recurrence rates of advanced-stage cancers. We systematically collected original data reporting 3-year DFU recurrence from studies published through 2024 and calculated a pooled mean. These findings were compared to recurrence rates for advanced breast, prostate, colorectal, and lung cancers using contemporary sources from the National Cancer Institute and American Cancer Society. CLTI reintervention data were drawn f

Clinical Reporting of Magnetic Resonance Imaging, the Way Forward for Patients With Lumbar Disc Herniation: A Prospective Correlational Study.

Background Lower back pain (LBP) is a major cause of increasing years lived with disability. Many adults suffer from LBP once in their lifetime. Multiple environmental, genetic, and acquired factors lead to disc degeneration. Spinal stenosis can be caused due to bony, ligamentous, or discogenic origin. The majority of cases have a combined etiology of bony, ligamentous, and disc disease. Lumbar disc disease (LDD) has been mentioned by various terminologies in the literature. A standardized nomenclature is needed for better research and communication. Our study is based on the correlation betwe

Clonorchiasis.

On Aug 21, 1875, James McConnell published in The Lancet his findings from a post-mortem examination of a 20-year-old Chinese man--undertaken at the Medical College Hospital in Calcutta, India--in whom he found Clonorchis sinensis in the bile ducts. Now, exactly 140 years later, we have a sound understanding of the lifecycle of this liver fluke, including key clinical, diagnostic, and epidemiological features. Developments in the so-called -omics sciences have not only advanced our knowledge of the biology and pathology of the parasite, but also led to the discovery of new diagnostic, drug, an

Metrics for monitoring local inequalities in access to maternity care: developing a basket of markers from routinely available data.

BACKGROUND: Equal access for all based on need is part of a conceptualisation of quality underpinning recent UK NHS policies. OBJECTIVE: To develop metrics for access to maternity care from routinely available data in order to inform inequalities monitoring and commissioning. DESIGN: Cross-sectional cohort design using case-note audit and postnatal questionnaire. SETTING: London hospital, UK, in an area of relative socio-economic deprivation. METHODS: Stage 1: Identification of potential markers. Stage 2: Testing of markers via case note audit and postnatal questionnaire. Stage 3: Selectio

Personal computer teleradiology interhospital image transmission to facilitate tertiary pediatric telephone consultation and patient transfer: soft-tissue lateral neck and elbow radiographs.

Although teleradiology systems are available commercially, they are expensive ($30,000), and different makes are incompatible with each other, making them unusable for interhospital image transfers. Standard components were added to a personal computer (PC) to build a functional teleradiology unit capable of interhospital image transmission at a low cost ($600 upgrade). This PC teleradiology system was studied to assess its accuracy in the interpretation of soft-tissue lateral neck x-rays with epiglottitis or retropharyngeal abscesses and elbow x-rays with joint effusions, fractures, or both.

Engaging community health workers in maternal and infant death identification in Khayelitsha, South Africa: a pilot study.

BACKGROUND: Engaging community health workers in a formalised death review process through verbal and social autopsy has been utilised in different settings to estimate the burden and causes of mortality, where civil registration and vital statistics systems are weak. This method has not been widely adopted. We piloted the use of trained community health workers (CHW) to investigate the extent of unreported maternal and infant deaths in Khayelitsha and explored requirements of such a programme and the role of CHWs in bridging gaps. METHODS: This was a mixed methods study, incorporating both q

Implementing a Patient-Centered Education Bundle to Improve Venous Thromboembolism Prevention.

IMPORTANCE: Venous thromboembolism (VTE) is one of the most common causes of preventable harm in hospitals. While numerous efforts have been made to increase prescription of evidence-based VTE prophylaxis for hospitalized patients, many prescribed doses are either refused or missed for other reasons. Successful interventions to decrease these missed doses have been developed and tested in academic hospitals, but it remains unclear if these interventions can be scaled and implemented in community hospitals. OBJECTIVE: To disseminate and implement an evidence-based, comprehensive strategy to re

Under-reporting of deaths to the coroner by doctors: a retrospective review of deaths in two hospitals in Melbourne, Australia.

BACKGROUND: 'Under-reporting' of deaths to the coroner has significant implications for the identification and investigation of preventable deaths. In extreme cases, it may even be a symptom of the system failures that allowed cases such as Harold Shipman, Australia's King Edward Memorial Hospital, the alleged incidents at the Bundaberg Hospital and the Bristol Royal Infirmary to persist. Several initiatives in Australia and the UK are currently reviewing the coroner's system in light of the recommendations made by the Luce report and the Bundaberg Hospital inquiry to consider whether the coro

Racial Disparities in Birth and Lactation Outcomes Among Neonates with Congenital Heart Defects at a Regional Mid-Atlantic Heart Center.

BACKGROUND: Evidence regarding ways in which systemic racism impacts birth and lactation among neonates with congenital heart defects (CHD) is limited. Thus, we sought to examine racial disparities among a cohort of neonates with CHD by exploring associations between birth and lactation outcomes and individual and community-level indicators of systemic racism. METHODS: This was a post hoc analysis of retrospective electronic health record (EHR) data from hospitalized neonates who had cardiac surgery and resided in the same county as the pediatric hospital (n = 20; 2016-2020). A Framework for Supporting Patients and Families After a Severe Maternal Event.

It is now well recognized that severe maternal morbidity and traumatic experiences during childbirth can have medical and psychological sequelae well after hospital discharge. The words used when communicating with patients can have a profound effect, especially when a severe maternal event has occurred. To create a patient care discussion (patient debrief) framework, a clinician-patient committee was formed to develop guiding principles with input from multiple perspectives as a part of a two-state quality-improvement project. The "Supportive Communication After a Severe Maternal Event: What

Perceptions and attitudes toward palliative care among healthcare professionals in Qatar's home care setting.

INTRODUCTION: Palliative care (PC) is an emerging concept in the Middle East, with Qatar lacking formalized home-based services until the 2021 Doha declaration. This qualitative study addresses a gap in the literature by exploring home healthcare service (HHCS) staff perceptions, attitudes, and challenges in delivering PC in Qatar's home setting, aiming to inform service development and enhance end-of-life care accessibility. METHODS: Using Braun and Clarke's thematic analysis approach, semi-structured interviews were conducted with 13 purposively sampled HHCS staff from various disciplines (

Psychiatric emergency service use and homelessness, mental disorder, and violence.

OBJECTIVE: This study examined relationships between homelessness, mental disorder, violence, and the use of psychiatric emergency services. To the authors' knowledge, this study is the first to examine these issues for all episodes of care in a psychiatric emergency service that serves an entire mental health system in a major city. METHODS: Archival databases were examined to gather data on all individuals (N=2,294) who were served between January 1, 1997, and June 30, 1997, in the county hospital's psychiatric emergency service in San Francisco, California. RESULTS: Homeless individuals a
