Guidelines for the family therapeutic approach to eating disorders.

After a brief historical overview of the literature on family therapy in eating disorders, the authors' own clinical experience is summarized in some practical guidelines: give the family therapist a central position within the team, engage the family and/or spouse as co-therapist, explore the message of the eating disorder on different levels, neutralize the symptoms outside of the family interaction, promote direct communication, facilitate the separation-individuation process, and be available for a long time. Finally some special issues are discussed: sexual and/or physical abuse in the fa

Characteristics and post-hospital perceptions of patients refusing inpatient treatment in a South African general hospital psychiatric unit.

This paper explores characteristics and post-hospital perceptions of patients who refused general hospital psychiatric inpatient treatment in a relatively young unit. It was found that after an initial significant increase of up to 9.6% per year in these patients, the average for the 6-year period studied was 6.2% of all admissions. Most of the patients were single or divorced, not gainfully employed, refused hospital inpatient treatment within the first day after admission and were significantly younger than the controls. Substance use, adjustment and personality disorders predominated, and m

Patient experiences with nursing care during hospitalization on a closed ward due to a manic episode: A qualitative study.

PURPOSE: To describe the experiences of patients with nursing care they had received when hospitalized for mania. DESIGN AND METHODS: Multicenter qualitative study using open interviews. Data were analyzed using the Stevick-Colaizzi-Keen method. FINDINGS: Sense of security is vital for the recovery of these patients, nurses can support and hinder this. Feelings of security are related to clear and calm communication, respect, recognizability of nurses, and daily structure. PRACTICE IMPLICATIONS: Nurses should make sure that the patient recognizes them as nurses. Nurses should spend as much

Perspectives of physicians on risk factors for patient aggression and violence against physicians in Chinese hospitals: a Q-methodology study.

BACKGROUND: The prevalence of patient (and their relatives/friends) aggression and violence against healthcare professionals in general, and physicians in particular, is a recognized problem worldwide. While numerous risk factors for such aggression and violence from patients (and their relatives/friends) have been identified, little is known about which risk factors are perceived as relatively most important in a specific context and among a particular group, and about the potentially differing views on the relative importance. This lack of insight prohibits preventive measures being tailored

How Uncertainty Is Talked About in Language Discordant and Concordant Health Consultations: Patterns of Epistemic Adverb Use and Communication Accommodation Strategies.

INTRODUCTION: In this study, we explored how uncertainty was expressed through epistemic adverbs in health consultations where patients and health practitioners (HPs) spoke the same language (language-concordant) versus when they did not (language-discordant). Further, we applied Communication Accommodation Theory (CAT) to identify, in these settings, how HPs used communication strategies and their patients' communicative responses. METHOD: Fifty-three health consultations between bilingual and monolingual patients and their HPs in a metropolitan emergency department were recorded and transcr

Cues that predict violence in the hospital setting: findings from an observational study.

BACKGROUND: The prevalence of violent acts in the health care environment has been the cause of increasing concern. Several cues associated with violence towards nurses in the acute care setting have been identified qualitatively. However, larger scale observational studies to determine the potential of these cues to predict physical violence, are lacking in the acute health setting. PURPOSE: To report an observational study determining whether particular behavioural cues are predictive of patient-to-nurse physical violence. METHOD: Non-participant structured observations. DISCUSSION: Clini

[Post-traumatic stress syndrome in surgery of the hand].

During our collaboration with the Hand Surgery Unit (HCUG), we have managed trauma patients presenting lesions of the upper limb of varying degrees of severity. We were particularly interested in patients who had undergone amputations. We observed that late psychiatric interventions were generally poorly accepted by the patients and were of no use to these patients. In the light of these findings, we therefore elaborated a new strategy of intervention in collaboration with our surgeon colleagues. Patients with severe upper limb trauma were referred to us immediately after their admission. This

Hostility in coronary artery disease patients and health care workers in Slovakia.

PURPOSE: Increased levels of hostility are associated with the increased risk of coronary artery disease (CAD), and with poorer outcomes in CAD patients. The purpose of this paper is to estimate the level of hostility in CAD patients and in health care (HC) workers, as potential groups for intervention programmes. DESIGN/METHODOLOGY/APPROACH: A qualitative cross-sectional study was undertaken. Hostility questionnaires were distributed in a non-randomized fashion in a group of 236 CAD patients (187 men, 49 women), aged from 33 to 69 years (average 52 years) and 181 health care workers (52 men,

[Pediatric version of the Adequacy Evaluation Protocol: application of the four Diagnostic Related Groups most frequently used in a pediatric hospital at La Coruña].

BACKGROUND: There is wide-spread consensus that a part of the use of hospital resources is inadequate in the sense that the patients receive services that do not provide them with any significant benefits, or that could be more beneficial, or less costly, with a lower care standard. The main aim of this work is to determine the percentage of inadequate admissions and stays in hospital for the four most frequent Diagnosis-Related Groups (DRGs) in a maternity-children's hospital through the retrospective application of the paediatric version of the Adequacy Evaluation Protocol (AEP). METHOD: It

Consistent Use of Assistive Devices for Patient Transfer Is Associated With Less Patient-Initiated Violence: Cross-Sectional Study Among Health Care Workers at General Hospitals.

This study investigated whether factors related to bodily contact between health care workers and patients were associated with patient-initiated violence. This cross-sectional study surveyed 496 Danish health care workers measuring patient-initiated violence, use of assistive devices, body mass index, physical exertion, frequency of patient transfers, psychosocial work environment, gender, age, and seniority. Associations were modeled using logistic regression analyses using patient-initiated violence as the outcome. Twenty-five percent of the respondents had experienced physical or verbal vi

Van Gogh, Cowper, and Unfinished Suffering: Narrative Hope in Clinical Chaplaincy.

This clinical pastoral reflection explores how historical and artistic narratives may support meaning-making in chaplaincy encounters. During a pastoral visit with a patient experiencing advanced cancer and depression, the lives of Vincent van Gogh and William Cowper became interpretive resources through which suffering was reframed as spiritually continuous rather than spiritually disqualifying. The encounter illustrates how narrative engagement can foster renewed hope while suffering remains unresolved within clinical pastoral care.

Interprofessional collaboration between hospital-based palliative care teams and hospital ward staff: A realist review.

INTRODUCTION: To ensure high quality in-hospital palliative care delivery, we need a deeper understanding of the interprofessional collaboration processes between palliative care teams (PCTs) and hospital ward staff. This realist review aims to unravel the relationships between contextual (C) factors, mechanisms (M), and outcomes (O) related to this interprofessional collaboration process. METHODS: This realist review employed an iterative, theory-driven approach which led to the development of a refined program theory explaining how, for whom, and under which conditions interprofessional col

Evaluation of post-discharge engagement for emergency department patients with opioid use history who received telehealth recovery coaching services.

BACKGROUND: In recent years, emergency departments (EDs) across the nation have implemented peer recovery coach (PRC) services to support patients who use opioids. The majority of such interventions discussed in the literature follow an in-person modality where PRCs engage patients directly at the ED bedside. However, the use of telehealth services in EDs is becoming more popular. These services connect PRCs with ED patients in real-time via secure communications technology, and very little is known about the service- and clinical-based outcomes with which they are associated. The current stud

Managing Opioid Withdrawal for Hospital Patients in Custody.

Dr. Brown, a hospitalist, admits Mark, a patient transferred from a local jail for management of cellulitis. The patient, who was taken into custody two days prior to hospital admission, has a history of intravenous heroin use. Mark explains that he had been prescribed buprenorphine-naloxone maintenance therapy for opioid use disorder for several years prior to being arrested and had not used other opioids during that time. As a policy, the jail where Mark is detained does not prescribe opioid agonists, and his maintenance therapy was stopped upon his arrival there. Dr. Brown discovers that Ma
