The external fixation in the treatment of humeral diaphyseal fractures: outcomes of 84 cases.

We retrospectively review 84 cases of diaphyseal humeral fractures (24 type A, 38 type B, 22 type C of the AO/OTA classification) treated with external fixation (Hoffmann II frame) between 1995 and 2007. Six of these fractures were complicated with radial nerve palsy. Four cases were open fractures. All reductions were achieved closely or through minimal open approaches. All fractures achieved consolidation with an average of 95 days (range 58-140). The six radial nerve palsies had complete spontaneous recovery. According to the Constant score excellent shoulder function was recorded in 54.6% …

Fatal pulmonary fat embolism during humeral fracture repair in a cat.

An 11-month-old cat was referred following a road traffic accident with non-weightbearing forelimb lameness and tachypnoea. A diaphragmatic rupture was repaired two days after trauma. Six days post trauma, a further operation was performed to repair the fractured humerus. As an intramedullary pin was advanced into the bone to achieve stabilisation, acute respiratory arrest occurred. Manual extrathoracic cardiopulmonary resuscitation was immediately instigated, but the cat did not recover. Postmortem examination identified a high degree of embolic fat occluding pulmonary capillaries. A massive

[Repair of humeral fracture and non-union with transfer of vascularized periosteal flap].

OBJECTIVE: To study the results of humeral fracture and non-union repaired by vascularized periosteal flap transfer. METHODS: The clinical data of humeral fracture and non-union in 23 cases were analysized retrospectively since 1995. Among them, minuted or several segmental fracture in 12 cases, non-union in 11 cases, and following injury of radial nerve in 7 cases. The operative method was open reduction, inner or external fixation with vascularized periosteal flap transfer. RESULTS: The period of follow-up was 6 months to 2 years. The repair result of all patients was excellent and good, b

Long-term results of treatment of fractures of the medial humeral epicondyle in children.

BACKGROUND: The treatment of isolated, displaced fractures of the medial humeral epicondyle in children is controversial. Both plaster cast immobilization without reduction and open reduction and internal fixation have been advocated. The purpose of this long-term retrospective study was to analyze the functional and radiographic results of both nonsurgical and surgical management of these injuries. METHODS: Forty-two patients who had had an isolated fracture of the medial humeral epicondyle with displacement of >5 mm at an average age of twelve years (range, eight to fifteen years) were eval

Definitive diagnosis of fracture-separation of the distal humeral epiphysis in neonates by ultrasonography.

Fracture-separation of the distal humeral epiphysis in neonates is difficult to diagnose radiologically because the cartilaginous epiphysis is radiolucent. We report a case in which fracture-separation of the distal humeral epiphysis in a neonate was diagnosed with the help of ultrasonography, which provided a clear delineation of the injury.

[Therapy of humeral head fracture. Open reposition and fixation with PDS pins].

Seven patients who sustained a displaced fracture of the capitulum humeri had been treated by surgery, using a volar approach and internal fixation with PDS pins. The advantage of this method is that by using the volar approach, the displaced capitulum could be reduced properly in its exact anatomical position. The internal fixation with PDS pins was solid enough to allow early postoperative movements. A second operation to remove any material was not necessary. In all cases the clinical and x-ray results have been excellent.

[Is a radial nerve paralysis in a humeral shaft fracture an indication for surgery? Indications].

Surgery for humeral shaft fractures. A series of 78 operative treated humeral shaft fractures was studied retrospectively. Sixteen times we found a radial nerve palsy, preoperatively. In this group we evaluated the type of fracture, the operation indication, the time of treatment and the postoperative recovery of the radial nerve palsy. Fourteen times we did an early exploration. The indication to operate was 9 times the conducting polytrauma, 5 times the radial nerve palsy itself and 2 times a pseudarthrosis. In 13 of the 16 patients nerve function recovered either clinically and/or electromy

Nonunion of slightly displaced fractures of the lateral humeral condyle in children: an update.

Inadequate treatment continues to be the leading cause of nonunion of slightly displaced fractures of pediatric lateral humeral condyles, despite references to prevention in the orthopaedic literature. In most cases, the elbow was established nonunion can be salvaged by early stabilization and bone grafting, provided that the fragment is in acceptable position and the growth plate of the condylar fragment is open. Procrastination may allow the physis of the condylar fragment to close prematurely, preemptying the golden opportunity to salvage the child's elbow. The author has collected 23 simil

Posterior shoulder dislocation with ipsilateral humeral shaft fracture. A case report.

A 41-year-old man incurred the unusual combination of subacromial posterior dislocation of the shoulder and ipsilateral humeral shaft fracture. The importance of inspection of joints proximal to long bone fractures cannot be overemphasized. The transscapular view is easily adapted to the diagnosis of this injury. When recognized early, closed reduction is the preferred treatment; open reduction, as in the present case, is reserved for chronic dislocation.

Humeral shaft periprosthetic fractures: Fracture patterns differ between short and standard-length arthroplasty stems.

INTRODUCTION: There have been no published studies evaluating the impact of humeral stem length on humeral shaft periprosthetic fractures. We sought evaluate the differences in fracture patterns between periprosthetic fractures around a short stem and standard stem humeral implants. MATERIALS AND METHODS: This is a retrospective cohort study. Patients sustaining a humeral shaft periprosthetic fracture around shoulder arthroplasty implants from December 2011 to January 2021 were identified using ICD-9/10 codes. Three upper extremity trained surgeons evaluated all radiographs assessing fracture

Combined Pediatric T-condylar Humeral Fracture and Monteggia Fracture-Dislocation in the Upper Extremity: A Case Report.

Monteggia fractures are characterized by a fracture of the ulna associated with radial head dislocation. T-condylar fractures of the humerus are intra-articular injuries that disrupt the distal humeral epiphysis. This report describes a rare case of a pediatric patient with an ipsilateral T-condylar humeral fracture and a Monteggia fracture-dislocation. A 10-year-old girl presented with elbow pain and deformity after falling while playing basketball. No open wounds or neurological deficits were observed, and the radial artery was palpable. The radiograph revealed a pediatric T-condylar humeral

Understanding the susceptibility to lateral condyle fracture by analyzing unaffected Baumann's angle in children with distal humeral fracture.

BACKGROUND: Previous studies have shown that posttraumatic cubitus varus deformity in children is somehow related to subsequent humeral lateral condyle fracture. Moreover, we had previously encountered an exactly similar case. In this study, we aim to understand whether there is a morphological difference between pediatric supracondylar and lateral condyle fracture of the humerus by comparing Baumann's angle of the unaffected elbow. METHODS: We conducted a retrospective evaluation of 40 cases of supracondylar fractures (36 boys, 4 girls) and 20 cases of lateral condyle fractures (16 boys, 4 g

Treatment of a 4-Part Proximal Humeral Fracture with a Concomitant Glenoid Fracture in an Elderly Patient: A Case Report.

CASE: Reported here is a single case of a highly comminuted fracture of the proximal aspect of the humerus with substantial humeral head displacement and a concomitant glenoid fracture encompassing >20% of the glenoid width. The fractures were treated simultaneously with reverse total shoulder arthroplasty and open reduction and internal fixation, respectively. CONCLUSION: The 1-year follow-up demonstrated excellent pain control and a good functional outcome, with no signs of instability or implant complications.

Daytime Versus After-hours Surgical Fixation of Pediatric Supracondylar Humeral Fractures: A Meta-analysis.

BACKGROUND: Pediatric supracondylar humeral fractures (SCHFs) are among the most common elbow injuries in children and frequently require operative fixation. Although after-hours surgery is often unavoidable due to emergency presentation patterns, many centers now reserve nighttime intervention for urgent indications such as vascular compromise. Concerns nevertheless persist regarding the potential impact of after-hours surgery on surgical efficiency, technical decision-making, and postoperative outcomes. METHODS: A systematic search of PubMed, Scopus, the Cochrane Library, and Google Scholar…
