BACKGROUND: To evaluate the outcomes of distal humerus hemiarthroplasty (DHH) in a series of eight consecutive patients with unreconstructible distal humeral fractures or failed open reduction and internal fixation (ORIF).
METHODS: Retrospective data including demographics, postoperative outcomes, and complications were extracted from electronic records at a level 1 trauma center. Surgeries were performed by a single fellowship-trained upper extremity surgeon. The indications for DHH were unreconstructible distal humerus fracture or failed ORIF. Eventually, as accepted by current literature,

OBJECTIVE: Monocortical screws are commonly employed in locking plate fixation, but specific recommendations for their placement are lacking and use of short monocortical screws in metaphyseal bone may be contraindicated. Objectives of this study were to evaluate axial pullout strength of two different lengths of monocortical screws placed in various regions of the canine humerus compared to bicortical screws, and to derive cortical thickness and bone density values for those regions using quantitative computed tomography analysis (QCT).
METHODS: The QCT analysis was performed on 36 cadaveric

Locking Compression Plate (LCP) has the advantageous feature that screws can be locked in the plate leaving an angular stable construct. There is no need to have contact between the plate and the bone to achieve stability resulting from friction of the plate-bone-construct. Therefore the plate does not need to be contoured exactly to the bone and the healing bone's periosteal blood supply is not affected. The LCP is used as a bridging plate to gain relative stability in multi-fragmentary, diaphyseal or metaphyseal fractures. Depending on the fracture, the combination hole can also allow the LC

Iatrogenic vascular injuries are rare but potentially serious complications that can occur during orthopedic procedures involving the proximal humerus. We present a case report of a patient who sustained a brachial artery injury during a long Proximal Humeral Interlocking System (PHILOS) plating procedure for a proximal humeral fracture. A 62-year-old female patient with a left humerus proximal 1/3 shaft fracture underwent open reduction and internal fixation with a long PHILOS plate. During the procedure, difficulty was encountered in achieving adequate plate positioning due to osteoporotic b

INTRODUCTION: Fractures of the distal humerus are a rare entity accounting for approximately 2% of the humeral fractures. Non union of the distal humerus is further rare and poses a major challenge. These fractures usually respond to open reduction and bone grafting but some may fail multiple surgical attempts at union and hence are labelled as "resistant" non union. We report a case of resistant non union of the distal humerus which was managed by total elbow arthroplasty.
CASE PRESENTATION: A 49-year-old male presented to the out patient department with history of a compound comminuted frac

BACKGROUND: Open articular fractures often include contaminated, devascularized osteoarticular fragments that are critical for joint reconstruction. Definitive treatment is often delayed such that decontamination and preservation of critical fragments for joint reconstruction is highly desirable. To validate decontamination and preservation protocols for safe and effective preservation of osteoarticular fragments for re-implantation, a preclinical animal model for inducing type 3 open articular fractures with contaminated, devascularized osteoarticular fragments was developed and validated.

Neurapraxia frequently occurs following traction injury to the nerve intraoperatively, leading to radial nerve palsy which usually recovers in 5-30 weeks. In our case, we had operated a distal one-third of humeral shaft fracture and fixed it with 4.5 mm limited contact dynamic compression plate. The distal neurovascular status of the limb was assessed postoperatively in the recovery room and was found to be intact and all the sensory-motor functions of the radial nerve were normal. On the second postoperative day, following the suction drain removal and dressing, patient developed immediate ra

A 35-year-old patient sustained comminuted intercondylar fracture of the distal humerus. At six months post-open reduction and internal fixation, the malunion was corrected with an intra-articular osteotomy. The patient obtained a painless, functional elbow joint with increased grip strength after corrective osteotomy, although she had complained of severe elbow pain, limited range of motion, and loss of grip strength before osteotomy. A 15 degrees cubitus varus deformity was also corrected . Radiographs of the elbow joint did not show accelerated degenerative changes at over three years follo

A 6-month-old female Yorkshire Terrier was examined because of acute left forelimb lameness secondary to a Salter-Harris type IV fracture of the lateral condyle of the humerus. Radiography revealed an eccentric, osteolytic lesion in the distal humeral metaphysis associated with a pathologic fracture. The limb was amputated, and the dog recovered. Microscopic examination revealed an extensive zone of hemorrhage and dilated coalescent spaces, which were filled with blood. Hemosiderin-laden macrophages and multinucleated giant cells were observed throughout the stroma. On the basis of clinical, r

OBJECTIVE: Precise adaptable fixation of a supracondylar humerus osteotomy with a radial/lateral external fixator to correct posttraumatic cubitus varus.
INDICATIONS: Acquired, posttraumatic cubitus varus as a result of a malhealed and unsatisfactorily treated supracondylar humerus fracture. Idiopathic, congenital cubitus varus (very seldom) if the child (independent of age and after complete healing) is cosmetically impaired; stability of the elbow is reduced due to malalignment (hyperextension); secondary problems and pain (e. g., irritation of the ulnar nerve) are expected or already exist

Treating intra-articular fractures about the osteoporotic distal humerus poses a significant challenge. The purpose of this retrospective study was to evaluate functional outcomes for distal humeral fractures treated with total elbow arthroplasty (TEA) or open reduction and internal fixation (ORIF) in a nonarthritic elderly population with osteoporosis. We reviewed the records of all women older than age 60 who had undergone surgical treatment for intraarticular distal humerus fractures (Orthopaedic Trauma Association types 13B and 13C) by 1 of 2 surgeons. Demographic and operative data were o

This report describes two cases of long bone non-union associated with the use of recombinant human bone morphogenetic protein-2 (rhBMP-2) and is the first of its kind. The first case describes a 25-year-old male who sustained a left diaphyseal femoral shaft fracture initially treated with operative fixation using an intramedullary nail, which subsequently loosened distally and was treated with exchange nailing and rhBMP-2 application. This patient developed acute local soft tissue inflammation post-operatively. The second case describes a 61-year-old female who sustained a right diaphyseal hu

INTRODUCTION: Open reduction and internal fixation (ORIF) with articular reconstruction is the standard treatment for distal humerus fractures; however, ORIF is challenging in elderly patients with severe comminution and poor bone quality. Although favorable outcomes of total elbow arthroplasty (TEA) have been reported for complex distal humerus fractures deemed unsuitable for ORIF, clinical data in Japanese patients remain limited. This study reports the clinical outcomes of TEA in Japanese patients aged 65 years or older when ORIF was deemed unfeasible.
METHODS: We retrospectively reviewed

OBJECTIVE: To determine whether the type of posterior surgical approach for distal humeral fracture open reduction and internal fixation influenced radial nerve strain during simulated operative retraction in a cadaveric model.
METHODS: Three different posterior surgical exposures: triceps splitting, lateral paratricipital, and paratricipital with release of the lateral intermuscular septum were used. Radial nerve strain was measured using a microDVRT, while traction was applied with a digital force gauge at forces 0.1-0.3 kg.
RESULTS: The lateral paratricipital with nerve decompression was

UNLABELLED: The carpal tunnel syndrome is a frequent illness with several etiological factors. Its appearance after trauma is rare. In a retrospective study its incidence and the trauma pattern were analyzed. From 1.1.95 to 31.12.99 144 median nerve decompression procedures for carpal tunnel syndrome were performed in 114 patients. Twelve patients (10.5%) had suffered a trauma in the recent or more distant past. There were six distal radius fractures, three metacarpal fractures, one finger fracture, one humeral shaft fracture and one distal avulsion of the biceps tendon. In eight patients the
