OBJECTIVE: To investigate the value of color doppler flow imaging (CDFI) and pulsed doppler (PD) determinations in early diagnosis, evaluation of therapeutic effects and prediction of prognosis of gestational trophoblastic diseases (GTD).
METHOD: CDFI and PD were performed in 55 cases of GTD and 30 normal pregnant women as controls.
RESULTS: The uterine muscular layer in GTD showed multicolored abundant blood flow and blood sinuses. Blood flow resistance was low with a resistance index (RI) less than 0.5. After chemotherapy the RI changed dramatically. Pathological evidences were found in su

In the field of gynecologic oncology, surgical intervention has been the traditional management of hemoperitoneum. Recently selective arterial embolization has been reported to effectively control gastrointestinal bleeding caused by peptic ulcer and postoperative abdominal or retroperitoneal bleeding associated with gynecologic malignancy. This study reports the use of selective arterial embolization in the management of hemoperitoneum caused by trophoblastic disease. Analysis of 4 cases provides valuable information which enhances the safety and efficiency of this procedure. Results suggest t

Human chorionic gonadotropin (hCG) is considered to be one of the factors which regulate relaxin secretion in humans. Serum immunoreactive relaxin levels are increased and are detectable by radioimmunoassay both in normal and molar pregnancy. Circulating hCG levels are increased in trophoblastic disease. In the present study, relaxin and hCG levels were sequentially measured in patients with invasive mole, choriocarcinoma and persistent trophoblastic disease. Serum relaxin levels were detectable by radioimmunoassay in these patients before treatment, though they were significantly lower than i

Highly recurrent hydatidiform moles (HMs) studied to date are not androgenetic but have biparental genomic contribution (BiHM). Affected women have an autosomal recessive mutation that causes their pregnancies to develop into HM. Although there is genetic heterogeneity, a major locus maps to chromosome 19q13.42, but a mutated gene has not yet been identified. Molecular studies have shown that maternal imprinting marks are deregulated in the BiHM trophoblast. The mutations that cause this condition are, therefore, hypothesized to occur in genes that encode transacting factors required for the e

OBJECTIVE: As complete hydatidiform moles (CMs) have been studied less with respect to aneuploidy and its clinical implications, the significance of cytometric aneuploidy in CMs was evaluated.
METHODS: Two hundred thirty-nine CMs were studied clinicopathologically and analyzed by flow cytometry using formalin-fixed paraffin-embedded tissues.
RESULTS: Of 239 CMs, 182 were diploid, 30 were tetraploid, and 27 were aneuploid (nontriploid/tetraploid aneuploid). There were no significant histologic differences among the diploid, tetraploid, and aneuploid CMs. Persistent disease developed in 20 of

Radioimmunoassays and bioassays based on the reactions of the native molecule of human chorionic gonadotropin (HCG) fail to differentiate HCG from pituitary luteinizing hormone (LH). An assay based on the beta-subunit of HCG which detects HCG exclusively has been used in our laboratory to monitor patients undergoing chemotherapy for gestational trophoblastic disease (GTD). We have been able to differentiate minimal, persisting tumor activity from normal levels of pituitary gonadotropins and have based therapy on these findings. Alternatively, treatment has been terminated when HCG is no longer

BACKGROUND: This is an update of the original review that was published in The Cochrane Database of Systematic Reviews, 2009, Issue 2. Gestational trophoblastic neoplasia (GTN) are malignant disorders of the placenta that include invasive hydatidiform mole, choriocarcinoma, placental-site trophoblastic tumour (PSTT) and epithelioid trophoblastic tumour (ETT). Choriocarcinoma and invasive hydatidiform mole respond well to chemotherapy: low-risk tumours are treated with single-agent chemotherapy (e.g. methotrexate or actinomycin D), whereas high-risk tumours are treated with combination chemothe
