An experimental animal model was set up to examine the interrelationship between urine flow and progressive ureteric stenosis in producing sonographically detectable hydronephrosis. In four rabbits with mild ureteric stenosis or no stenosis the renal pelvis showed little distension even with rapid diuresis. Progressively tighter ureteric stenoses resulted in progressive renal pelvic distension accentuated by diuresis. Nine patients with a prenatal ultrasonic diagnosis of unilateral ureteropelvic junction obstruction underwent pyeloplasty in early neonatal life. Based on radiological and renographical assessment of the results, immediate postnatal intervention had no demonstrable advantage over those subjects in a control group who underwent an operation after presenting with symptoms. Half of the children in both groups had normal parenchymal function of the hydronephrotic kidney. An experimental model is described in rabbits where chronic partial ureteric occlusion and permanent renal pelvic cannulation were performed. For periods of up to three months, parameters of distal nephron function were measured repeatedly and pressure-flow studies performed. Fractional water clearance, free water clearance/GFR ratio and the relative intrapelvic pressure (IPP) on perfusion were increased (P less than 0.01) as a result of partial ureteric occlusion. The results of surgical treatment for obstruction at the pyeloureteral junction in 282 patients, both children and adults, are presented. Most of the patients were treated by the Anderson-Hynes method, without nephrostomy. Good results were achieved in 71.2%, satisfactory ones in 20.4% of the cases. The rest of the patients underwent further surgery. The causes of late and early postoperative complications are analysed. A good result depends upon the adequate choice of operation and the correct technique of its performance.

Rats with hereditary hydronephrosis and littermates with normal kidneys were given 1% saline solution instead of drinking water, and renal excretory function, blood pressure, fluid volumes, and plasma renin activity were monitored. A marked increase in blood pressure and positive correlation between blood pressure and plasma volume were found in hydronephrotic rats after five weeks of salt loading, while animals with normal kidneys remained normotensive. Histological investigation of ureteropelvic junction (UPJ) specimens of 21 patients with congenital hydronephrosis revealed in 7 instances pathological changes, which might be responsible for the obstruction. In 15 cases a significant increase of collagenous tissue within the UPJ, along with a relative deficiency of monocytes was noted. The unusual clinical course of three cases of dilatation of ureters and renal pelvises in pregnancy was reported. The presenting symptom in each case was acute abdominal pain, which was at first attributed to other surgical or obstetric emergencies like premature separation of placenta, acute appendicitis and acute hydramnion.
