A rare cause of Horner syndrome.

OBJECTIVES: To demonstrate the importance of thorough investigation of patients with Horner syndrome, and to explain the relevant anatomy. CASE REPORT: A 46-year-old man presented with right-sided Horner syndrome. No other abnormality was found. Magnetic resonance imaging showed calcification of the stylohyoid ligament, with a pseudoarthrosis in the mid-portion of the ligament. This pseudoarthrosis was displacing and compressing the internal carotid artery and the adjacent sympathetic chain, causing Horner syndrome. CONCLUSION: In this case, magnetic resonance imaging was invaluable in eluci

Hyoid fracture and strangulation.

Observation of hyoid fracture in skeletonized remains offers potentially valuable information on the history of the skeleton or evidence of foul play, or both. Perimortem hyoid fracture frequently indicates manual strangulation, although ligature strangulation, hanging, and other forms of trauma to the neck cannot be ruled out without additional evidence. Such fractures are rare in children and infants, since the hyoid components are not fully ossified and are more flexible than in adults. Both antemortem and postmortem origins of the fractures must also be considered.

Elongated styloid process syndrome: intraoral versus external approach for styloid surgery.

Elongated styloid process is a source of craniofacial and cervical pain. This condition is characterized by a dull, nagging, pharyngeal pain and a palpatory finding in the tonsillar fossa. Radiologic demonstration of styloid elongation is readily made in most instances. The incidence and pathogenesis of the styloid syndrome are discussed and the differential diagnosis detailed. The only effective treatment is surgical shortening of the styloid process. Eight patients undergoing surgery for elongated styloids are reported and the intraoral and external surgical approaches are presented. The ext

[A neonate with a swelling in the floor of the mouth].

A significant swelling was seen in the floor of the mouth of a newborn girl. The girl could only drink with difficulty. On examination, a soft-elastic swelling was seen beneath the tongue. Ultrasonography and MRI showed a mass located above the hyoid bone. Upon the initial differential diagnosis of a dermoid cyst, an enucleation of the lesion was performed. Histopathological examination suggested a branchiogenic cyst or a digestive duplication cyst. Given the inconclusiveness of additional diagnostic examination, the lesion was diagnosed as a developmental cyst. Six months after enucleation, t…

The impact of mandibular advancement on the upper airway patterns--cephalometric study.

The aim of the presented study was to assess the impact of mandibular advancement on the upper airways patterns, on the position of soft palate, hyoid bone and tongue. The set of 12 patients who underwent bilateral sagittal split osteotomy advancement surgery only, without genioplasty, for correction of retromandibularism (skeletal class II malocclusions) was evaluated. Rigid internal fixation was used in all the patients without rigid intermaxillary fixation. Measurements were taken in cephalograms obtained after the orthodontic decompensation and immediately after the surgery. In the set of …

[Comparative cephalometric study in patients with obstructive sleep apnea syndrome (OSAS), simple snorers and controls].

Lateral cranial teleradiography was carried out in 106 subjects, including obese and non-obese patients with obstructive sleep apnea syndrome (OSAS) and obese and non-obese healthy snorers and controls. In the OSAS patients alone, the hyoid bone occupied an abnormally low position, which pushed the tongue into a more vertical position. OSAS patients had the most voluminous soft tissues, followed by simple snorers and controls. Soft tissue volume generally was related with obesity. The pharyngeal airway space was widest in controls, intermediate in snores and narrowest in OSAS patients, except …

Surgical mandibular advancement and changes in uvuloglossopharyngeal morphology and head posture: a short- and long-term cephalometric study in males.

The aim of the present study was to investigate, by means of an extensive cephalometric examination, the alterations which took place in hyoid bone position, head posture, position and morphology of the soft palate, and tongue and sagittal dimensions of the pharyngeal airway after mandibular advancement osteotomy for the correction of mandibular retrognathism. The sample consisted only of adult males who underwent mandibular advancement by bilateral sagittal ramus split osteotomy (BSRO) with rigid fixation. Anterior retropharyngeal approach (ARPA) for high cervical spine.

BACKGROUND: One of the major challenges in operating on the spine lies in taking an anterior approach for the high cervical spine. In patients with a short neck, Klippel-Fiel syndrome or when the C3 vertebra is high in relation to the hyoid bone, it will be difficult to access the C3 body. The transoral route is a highly contaminated zone, and therefore, no instrumentation or grafts can be placed through it. METHOD: The anterior retropharyngeal approach (ARPA) for the high cervical spine. CONCLUSION: The anterior retropharyngeal approach is an excellent approach for the high cervical spine w…

Suicidal strangulation with a lashing belt.

Ligature strangulation causes death due to neck compression without suspension. However, suicidal ligature strangulation is rare. The suicide of a 32-year-old man is presented. After a dispute, the man was found outdoors with a lashing belt around his neck which had been pulled tight with a ratchet. Conjunctival petechiae were found, but hemorrhages in the soft tissues were not observed. The hyoid bone and larynx did not show fractures or hemorrhage. Other injuries as well as signs of defence were not observed. The differentiation between homicidal and suicidal ligature strangulation can be di…

Extracranial internal carotid artery dissection caused by compression from a giant osteophyte due to atlantoaxial osteoarthritis: case report.

Deformed osseous structures have been reported as rare causes of extracranial internal carotid artery (ICA) dissection, including the styloid process and the hyoid bone. Here, the authors describe the first known case of symptomatic ICA dissection caused by a giant osteophyte due to atlantoaxial osteoarthritis. The left ICA was fixed at the skull base and at the ICA portion compressed by the osteophyte, and it was highly stretched and injured between the two portions during neck rotation. The patient was successfully treated with ligation of the affected ICA following balloon test occlusion. A…
