Central diabetes insipidus and adipsia due to astrocytoma: diagnosis and management.

Adipsia and/or diabetes insipidus is rarely a direct complication of astrocytoma. We report a young man with recurrence of anaplastic astrocytoma who presented as severe hypernatremia. This case highlights key diagnostic and therapeutic challenges: (1) the interpretation of the response to exogenous vasopressin in a patient with steroid-induced hyperglycemia and (2) the potential risk of brain edema and herniation if excess water is prescribed along with vasopressin supplementation. The patient was successfully managed with prescribed fluid replacement, daily weights, and regular electrolyte m…

Central diabetes insipidus due to cytomegalovirus infection of the hypothalamus in a patient with acquired immunodeficiency syndrome: a clinical, pathological, and immunohistochemical case study.

We report a case of central diabetes insipidus (CDI) in a patient with AIDS due to cytomegalovirus (CMV) infection of the vasopressin-producing areas of the hypothalamus. The clinical diagnosis is established by definitive clinical and laboratory evidence of CDI. Detailed histopathological and immunohistochemical studies establish CMV as the causative agent and demonstrate the deficit of vasopressin in the synthesizing neurons. Physicians caring for patients with AIDS should be aware of CDI and adipsic hypernatremia as potential complications of CMV infection. The case also demonstrates that p…

Impact of acute kidney injury and dysnatremia on length of stay in infants after cardiac surgery.

BACKGROUND: Acute kidney injury (AKI) and dysnatremia following pediatric cardiac surgery are common conditions associated with worse outcom … ultivariate analysis, prolonged cardiopulmonary bypass (CPB) duration, higher furosemide doses, and hypernatremia were independently associated with AKI. AKI was only associated with prolonged ICU-LOS in infants, while hyponatremia was associated with prolonged ICU-LOS in all individuals with AKI. CONCLUSIONS: AKI occurs frequently in neonates and infants afte …

Osmoregulatory thirst in mice lacking the transient receptor potential vanilloid type 1 (TRPV1) and/or type 4 (TRPV4) receptor.

Recent studies suggest the ability of the central nervous system to detect changes in osmolality is mediated by products of the genes encodi … s strains after injection of 0.15 M NaCl or during a time-controlled period (no injection). Chronic hypernatremia produced by sole access to 2% NaCl for 48 h also produced similar increases in water intake across strains. In a final set of experiments, subcutaneous injection of 0.5 M NaCl produced similar increases in the number of Fos-positive nuclei within th …

Alterations in serum osmolality, sodium, and potassium levels after repeated mannitol administration.

This study was undertaken to evaluate whether significant alterations in serum osmolality, [Na+], and [K+] occur after the repeated dosing o … bsequently its rate reduced, although it still maintained high (70-77%). Regarding the serum [Na+], hypernatremia was observed in 10% of the subjects on the first day of mannitol administration and in 10-21% throughout the 7-day mannitol administration period. In this study, hyponatremia was observed in 12.2% of our subjects on the first day of mannitol adminis …

[Geriatric patient with neurological symptoms and prolonged aPTT].

A 73-year-old man with dementia was referred to our clinic with hypernatremia and volume depletion. New-onset neurogenic dysphagia was likely the reason for both. The patient had chronic embolic strokes on the computed tomography (CT) images. Documentation from previous hospitalizations in different hospitals revealed a repeatedly prolonged activated partial thromboplastin time (aPTT); 5 years prior, antiphospholipid antibody syndrome had already been suspected, but the necessary workup was never completed. We diagnosed the patient with primary antiphospholipid antibody syndrome and initiated …

Effective electroconvulsive therapy for stupor in the high risk patient: a report of two cases.

We report two cases of stupor in which the patients were safely treated by electroconvulsive therapy (ECT) despite high risk conditions. Case 1 was a 72 year old schizophrenic woman who had developed catatonic stupor and had joint contractures as a complication of rheumatoid arthritis. Case 2 was a 52 year old woman who developed a stuporous state which was complicated by severe dehydration with hypernatremia. In both cases, psychotic symptoms were improved by ECT without event. Careful application of ECT seemed to be effective and safe even for stupor in high risk patients.

Sodium retention and salt appetite following deoxycorticosterone in hamsters.

Previous research suggests that hamsters 1) fail to retain sodium after mineralocorticoid injections, 2) retain sodium after adrenalectomy equal to controls, and 3) do not develop salt appetite after mineralocorticoid or adrenalectomy. The present studies demonstrate sodium retention, body weight gain, hypernatremia, plasma volume expansion, and reduced fecal sodium excretion after daily injections of deoxycorticosterone acetate (DOCA). Salt appetite appeared after the 3rd and 4th days. Adrenalectomy caused reductions of sodium balance, plasma volume, and food intake, which were reversed by DO…

Perianal Abscess in a Patient With Central Diabetes Insipidus: A Case Report.

Central diabetes insipidus (CDI) is a rare endocrine disorder that can pose significant risks in the perioperative setting, especially when routine fasting and fluid restriction are applied. We report the case of a 34-year-old man with a history of CDI who underwent surgical drainage of a perianal abscess and subsequently developed severe hypernatremia and neurological symptoms due to postoperative water restriction. Prompt fluid resuscitation and endocrinology consultation led to rapid recovery. This case underscores the importance of early recognition and individualized perioperative managem…

Recurrent Fevers, Dysautonomia, and Dehydration in a Patient With Lesch-Nyhan Syndrome.

Lesch-Nyhan syndrome (LNS) is a disease characterized by a reduced ability to recycle purines, leading to increased de novo purine synthesis and uric acid production. Patients classically present with an array of hyperuricemic, neurologic, and behavioral symptoms. In this report, we describe a 26-year-old male with a history of LNS and recurrent fevers of unknown origin who presented to the emergency department (ED) with a fever, hypotension, and hypernatremia. We suspect that our patient's presentation was caused by autonomic instability in the setting of LNS leading to excessive free water l…

Delay in desmopressin therapy: Disaster in waiting.

WHAT IS KNOWN AND OBJECTIVE: Central diabetes insipidus (DI) is a complex disease that requires firm adherence to desmopressin therapy. There is little information on the onset of hypernatremia after withdrawal of desmopressin. CASE SUMMARY: We present a case of an elderly woman with central DI whose serum sodium jumped from 141 to 171 mEq/L after 48-72 h of holding oral desmopressin. Her DI crisis resolved with intravenous desmopressin and free water administration. WHAT IS NEW AND CONCLUSION: Based on this precipitous onset of DI crisis, we recommend not withholding desmopressin for more t…

Diabetes Insipidus: A Pragmatic Approach to Management.

Diabetes insipidus (DI) is a disorder of water balance characterized by polyuria and polydipsia. It can occur due to genetic and acquired causes that affect the secretion or action of arginine vasopressin (AVP) or antidiuretic hormone (ADH).Markedly increased thirst and urination are not only quite distressing but also increases the risk of volume depletion and hypernatremia in severe situations. A careful diagnosis of the type of DI and its etiology is based on careful clinical evaluation, measurement of urine and serum osmolality, and water deprivation test. Management includes the correctio…

Extrapontine myelinolysis developed after aneurysmal subarachnoid hemorrhage (a case report and literature review).

TOPICALITY: Central pontine and extrapontine myelinolysis is a rare and dangerous form of the demyelinating process of undefined origin, the development of which is associated with a very low sodium level in the blood (hypernatremia). OBJECTIVE: To describe a rare case of extrapontine myelinolysis as a complication occurring in the hemorrhagic period of anterior communicating artery aneurysm rupture as well as to demonstrate that this condition can be diagnosed intravitally. CONCLUSION: Clinical vigilance of extrapontine myelinolysis may promote the timely diagnosis and treatment of this dis…

Is Bicarbonate Therapy Useful?

Despite concerns about the negative effects of metabolic acidosis, there is minimal evidence that sodium bicarbonate administration is an effective treatment. In addition, sodium bicarbonate therapy is associated with many adverse effects, including paradoxic intracellular acidosis, hypokalemia, hypocalcemia, hypernatremia, and hyperosmolality. Definitive recommendations regarding bicarbonate therapy are challenging as there is little high-quality evidence available. In most clinical scenarios of metabolic acidosis, treatment efforts should focus on resolution of the underlying cause, and sodi…

Severe hypernatremic dehydration associated with cerebral venous and aortic thrombosis in the neonatal period.

Severe neonatal hypernatremia is an important electrolyte disorder that has serious effects. Cerebral venous thrombosis and aortic thrombosis are relatively rare in severe neonatal hypernatremic dehydration. The authors report a case of cerebral venous thrombosis, associated with aortic thrombosis revealed by dehydration in a 9-day-old boy. Diagnostic was performed using Doppler ultrasound and imaging techniques. Thrombosis was resolved after anticoagulation treatment with low-molecular weight heparin for 1 month, and then was substituted by oral anticoagulant. The case report is followed by a…

Liddle's syndrome in an elderly woman.

A 78-year-old woman with hypertension was hospitalized with acute bronchitis. However, she was also found to have hypernatremia, hypokalemia, and metabolic alkalosis. Detailed examination showed a low plasma renin activity and plasma aldosterone concentration. A provisional diagnosis of Liddle's syndrome was established and the patient was successfully treated with triamterene. Although Liddle's syndrome is generally considered an inherited hypertensive disease found in young people, a review of the literature indicated that muscle weakness is an important clinical finding in elderly patients …

The diagnosis and management of diabetes insipidus during medical emergencies.

The abrupt presentation of hypertonic polyuria, polydipsia and hypernatremia, reflects vasopressin deficiency owing to multiple potential etiologies. Diabetes insipidus becomes an emergency and leads to severe hyperosmolality and dehydration when fluid intake does not match obligate losses. Decreased mental alertness may impair the ability to sense thirst or to obtain access to fluids, thus placing patients postoperatively or posttrauma at particular risk of complicated diabetes insipidus. Intravenously administered DDAVP and hydration with hypotonic fluids is the preferred therapy in the acut…

A Rare Case of Lithium-induced Partial Nephrogenic Diabetes Insipidus.

Chronic lithium therapy in patients with bipolar disorder and other psychiatric illnesses can lead to a very common side effect of complete or partial nephrogenic diabetes insipidus. After confirmation of hypotonic polyuria, water deprivation test with desmopressin injection is used to make the diagnosis. Incomplete response to desmopressin suggests partial nephrogenic diabetes insipidus. We report a rare case of a 58-year-old patient who presented with hypernatremia and hypotonic polyuria secondary to chronic lithium therapy. She was diagnosed with partial nephrogenic diabetes insipidus secon…

Ovulation induction with pulsatile GnRH in a patient with anovulation of hypothalamic origin and central diabetes insipidus.

Patients with diabetes insipidus (DI) are at high risk for dehydration, hypernatremia and hemodynamic instability. Ovarian hyperstimulation syndrome (OHSS) likewise has negative effects on electrolyte balance. If ovulation induction is required in a patient with DI, there is thus a strong argument for avoiding techniques that increase the risk of OHSS. This paper reports the results of ovulation induction in a patient with anovulation of hypothalamic origin and central (vasopressin deficit) DI. Ovulation was induced with pulsatile gonadotropin-releasing hormone (GnRH), which induces single-ovu…
