Sheehan's syndrome and lymphocytic hypophysitis often occur in relation to pregnancy, making their differentiation difficult. We describe a 52-yr-old woman with hypopituitarism, Hashimoto's thyroiditis and candidiasis. She was admitted to our hospital because of nausea, vomiting and constipation. The role of hypothalamo-pituitary-adrenocortical (HPA) axis in stress-induced ulceration is reviewed. The special attention is devoted to corticosteroids. It is discussed our hypothesis according to which a gastric ulcer formation after administration of high corticosteroid doses is due to the insufficiency of corticosteroid production as a result of suppression of HPA axis. A 59-year-old male patient was transnasally operated on because of a pituitary adenoma with hypopituitarism. A second operation and X-ray therapy followed a half year later due to recurrent tumor. Both neoplasmas were classified as sparsely granulated prolactin cell adenomas. Immunohistochemical studies revealed strong immunoreactivity for prolactin and FSH in the tumor cells of both the pituitary adenoma and the recurrent tumor. The serum levels of svT3, svT4, TTH, T3 and T4 are evaluated at two, six and twelve months after partial resection of the thyroid gland in patients with euthyroid struma nodosa. The patients are divided up in two groups: group one--given hormonal substitution therapy, and group two--untreated. The results are estimated with a special reference to the need of hormonal administration in view of precluding recurrences.

Parathyroidectomy and subsequent hypocalcemia were shown to inhibit the response of the hypothalamo-hypophyseal-adrenocortical system (HHACS) to unilateral adrenalectomy, as expressed by a lesser compensatory hypertrophy of intact adrenal gland and a lower concentration of 11-oxycorticosteroids in adrenal gland and in plasma. The greatest activity of the HHACS was revealed in 3-month old rats. The data obtained suggest an interaction between parathyroid and adrenal glands.

A patient is described with a testicular Non-Hodgkin's Lymphoma (NHL) presenting with panhypopituitarism caused by a hypophyseal localization. A 67Gallium scintigraphy showed avid uptake in the hypophyseal region. Obviously 67Gallium could reach the tumor, by the intravenous route, which was the reason to treat the patient with intravenous chemotherapy. A complete remission was induced, which seems to be lasting (+25 months). As far as we know this is the first report of panhypopituitarism caused by a hypophyseal NHL in the hypophysis and successfully treated by intravenous chemotherapy.

The authors report the case of a young woman with advanced breast cancer who developed diabetes insipidus due to pituitary involvement and also gastric metastases. This patient had a normal brain CT scan. Gastric metastases were diagnosed when she was operated for a perforated gastric ulcer. Although very rare, and even if the brain CT scan is normal, pituitary metastases should be diagnosed in the presence of suggestive clinical symptoms. Abdominal pain also warrants investigation in these patients in an early attempt to document any possible gastric metastases.

The time course of LH, FSH, prolactin, estradiol, hydrocortisone, insulin and catecholamine levels in the blood was followed up in 62 patients of a reproductive age after surgical switch-off of the ovaries. A case of lymphoid hypophysitis in a woman who died during labor is presented. From a review of the 14 previously reported cases it is apparent that this is a specific disease entity, that it involves only woman, especially in association with pregnancy, and that it may have a fatal outcome. The clinical manifestations are either related to hypopituitarism, or those of a space occupying lesion. Attention drawn to the possible occurrence of the disease may result in earlier recognition of the disease and a better outcome of this potentially fatal disease.
