Imaging of hepatic infections.

Imaging plays a significant role in the detection, characterization and treatment of hepatic infections. Infectious diseases of the liver include pyogenic and amoebic abscesses and parasitic, fungal, viral and granulomatous infections. With increases in worldwide travel, immunosuppression and changing population demographics, identification of cases of hepatic infection is becoming more common in daily practice. Knowledge of the imaging features seen with hepatic infections can assist in early diagnosis and timely initiation of appropriate therapy. This review presents the imaging appearances …

[Immunopathogenesis of Leishmania infections].

Leishmaniasis represents a complex of diseases with a clinical and epidemiological diversity. Leishmaniasis remains a severe public health p … pathogens have evolved with the vertebrate immune system and typically produce long lasting chronic infections. A critical step in their host interaction is the evasion of innate immune defenses. The ability to avoid attack by humoral effector mechanisms, such as complement lysis, and to resist killing by lysosomal enzymes and toxic metabolytes is of particu …

Neonatal cutaneous fungal infections.

PURPOSE OF REVIEW: Cutaneous fungal infections are not uncommon in newborns and are seen in premature or otherwise immunocompromised neonates as well as in healthy full-term neonates. Healthy newborns can develop clinical manifestations as a result of infection with Candida species or as a result of skin colonization with Malassezia species; cutaneous infection with other fungal pathogens is rare. Immunocompromised and premature neonates, however, are susceptible to infection with opportunistic pathogens and are also at higher risk for invasive infection with common pathogens such as Candida. …

Immunogenetics of viral infections.

The HLA class I and II genes encode molecules that lie at the heart of the acquired immune response against infectious diseases. Associations between these polymorphic loci and genetically complex infectious diseases have been historically elusive, in contrast to the more obvious HLA associations with autoimmune diseases. High resolution molecular typing of large, clinically well-defined cohorts has begun to uncover evidence for the influence of HLA diversity on diseases of viral etiology, such as those caused by HIV-1, hepatitis B virus, hepatitis C virus and human papilloma virus. Combinatio

Musculoskeletal infections: ultrasound appearances.

Musculoskeletal infections are commonly encountered in clinical practice. This review will discuss the ultrasound appearances of a variety of musculoskeletal infections such as cellulitis, infective tenosynovitis, pyomyositis, soft-tissue abscesses, septic arthritis, acute and chronic osteomyelitis, and post-operative infection. The peculiar sonographic features of less common musculoskeletal infections, such as necrotizing fasciitis, and rice body formation in atypical mycobacterial tenosynovitis, and bursitis will also be presented.

Infections and autoimmune diseases.

The high percentage of disease-discordant pairs of monozygotic twins demonstrates the central role of environmental factors in the etiology of autoimmune diseases. Efforts were first focussed on the search for triggering factors. The study of animal models has clearly shown that infections may trigger autoimmune diseases, as in the case of Coxsackie B4 virus in type I diabetes and the encephalomyocarditis virus in autoimmune myositis, two models in which viruses are thought to act by increasing immunogenicity of autoantigens secondary to local inflammation. The induction of a Guillain-Barré sy…

Foodborne infections in France.

To quantify the impact of foodborne diseases on health, and set priorities for data collection, prevention and control of these diseases, we compiled and analyzed information from surveillance systems and other sources on the morbidity and mortality due to foodborne infectious diseases in mainland France in the last decade of the 20th century. Illness due to 13 bacteria, two viruses, and eight parasites were studied. The number of foodborne infections, hospitalizations, and deaths were estimated from multiple data sources. For each agent, several estimates were derived from the different sourc…

Cytokines in Salmonella infections.

Cytokines are intercellular signal molecules involved in the immune pathogenesis of infectious diseases. Cytokine mediated pathological alterations are mainly attributable to an imbalance in cytokine production. Experimental and clinical studies have documented the fact that Salmonella infections induce a Th1 dependent immune response and interferon-gamma and interleukin-12 are central to its genesis. Salmonella endotoxins, flagellins and porins are among the chief mediators of cytokine release. Lectin binding proteins, Toll-like receptors, transcription factors and various cells also contribu…

Combating opportunistic infections: coccidioidomycosis.

Coccidioidomycosis is a growing problem in the southwestern US. Although most cases of symptomatic coccidioidomycosis occur in healthy individuals, individuals with depressed cellular immunity are at particular risk. These include individuals with HIV infection, those who have undergone allogeneic transplantation and others on immunosuppressive medications. Most instances of primary pulmonary coccidioidomycosis do not require antifungal therapy. However, almost all cases of disseminated coccidioidomycosis will require such therapy. The triazole antifungals, fluconazole and itraconazole, are ef

Skin infections and ageing.

Elderly individuals have an increased susceptibility to skin infections due to age-related anatomical, physiological and environmental factors. The types of organisms that cause primary skin and soft tissue infections are diverse, and include bacterial, viral and fungal pathogens as well as parasites. In the elderly, these infections and infestations may present with atypical signs and symptoms or may complicate underlying chronic skin disorders. Clinical features, investigations and management of the following important and common skin infections are described in more detail: cellulitis, erys…

HIV and gynaecological infections.

Human immunodeficiency virus (HIV) infection primarily affects women during their reproductive years, and the co-existence of gynaecological infections is not surprising, given the fact that HIV is mainly acquired via heterosexual contact. Most gynaecological infections are themselves sexually acquired, and have the potential to increase the risk both of acquiring and transmitting the HI virus. As most sexually transmitted infections are asymptomatic, there is a need to improve methods of diagnosis and algorithms for early detection of sexually transmitted infections. HIV infection, however, p…

Cirrhosis and bacterial infections.

Half of cirrhosis patients die within two years after diagnosis, in most cases from cirrhosis related causes; most frequently variceal bleeding closely followed by infections. There seems to exist associations between infection and other complications such as malnutrition, hepatic encephalopathy and variceal bleeding. Cirrhosis patients have an acquired immune deficiency because of dyshomeostasis and malnutrition. All host defence systems are compromised, e.g. the acute phase response, and macrophage, neutrocyte, and lymphocyte functions. Simultaneously, there is increased microbiotic invasion…

[Candida infections in newborns].

Candida infections in newborns are classified, as bacterial ones, in early onset or maternofoetal infections and late onset diseases, primitive and nosocomial. Candida infections are responsible of less than 1% of early onset sepsis. Their diagnosis is facilitated by suggestive cutaneous lesions which can be associated or not with systemic infection. Candida nosocomial infections are the most frequent, 7% in premature infants < 1500 g; they are associated with a central venous catheter or with cutaneous infection in extremely low birth weight infants. C. albicans is the predominant species in …

Necrotizing Soft Tissue Infections.

Necrotizing soft tissue infections are characterized by necrosis of skin and associated structures. Despite advances in the diagnosis and treatment of these infections, the mortality remains high. There have been increasing reports of necrotizing fasciitis caused by group A Streptococcus over the past decade. Recent information supports the role of superantigens in the pathogenesis of this infection. The approach to management requires expeditious evaluation with early surgery and appropriate antimicrobial agents. Limited data suggest that surgical debridement may be delayed in selected patien…

Vertically transmitted herpesvirus infections.

The importance of recognizing perinatally acquired herpes simplex virus infections is now well understood and recently, increased attention has been devoted by clinicians to the problem of perinatally acquired cytomegalovirus infections. Less commonly, other members of the herpesvirus family may also be transmitted from mother to infant in the perinatal period, causing disease. Future developments of the management of herpesvirus infections will include greater roles for antiviral therapies and vaccines. Expanded use of oral nucleoside antiviral therapies in pregnancy has engendered uncertaint…

Hantavirus infections in Europe.

Hantaviruses are enveloped RNA viruses each carried by a specific rodent species. Three hantaviruses, Puumala, Dobrava, and Saaremaa viruses, are known to cause haemorrhagic fever with renal syndrome. In Europe. Puumala causes a generally mild disease, nephropathia epidemica, which presents most commonly with fever, headache, gastrointestinal symptoms, impaired renal function, and blurred vision, whereas Dobrava infections often also have haemorrhagic complications. There are few available data about the clinical picture of confirmed Saaremaa infections, but epidemiological evidence suggests t…
