Infliximab for the treatment of posterior uveitis with retinal neovascularization in Behçet disease.

PURPOSE: To report a case of posterior uveitis with retinal neovascularization in a patient with Behçet disease treated with infliximab. METHODS: A 50-year-old man with a history of recurrent relapses of ocular inflammation despite immunosuppressive therapy developed retinal neovascularization near the optic disk. The patient was treated with infliximab and followed up for 12 months. RESULTS: Retinal neovascularization regressed 8 months after the first anti-tumor necrosis factor (TNF) treatment and with six infusions of infliximab. The ocular inflammation resolved almost completely. CONCLU…

Infliximab for treatment of pyoderma gangrenosum associated with inflammatory bowel disease.

OBJECTIVES: Pyoderma gangrenosum is an immune-mediated inflammatory condition characterized by ulcerative skin lesions affecting 1-2% of patients with inflammatory bowel disease (IBD). Treatment includes wound care, antibiotics, corticosteroids, and immunomodulators. However, response to therapy varies, and many patients with pyoderma gangrenosum have disease that is refractory to these agents. The aim of this study was to assess the response of medically refractory pyoderma gangrenosum to infliximab. METHODS: This was a multicenter retrospective study of patients with IBD and medically refra…

[Retrospective analysis of adverse effects of infliximab in a hospital rheumatology service].

Infliximab is a chimeric monoclonal antibody against human tumour necrosis factor-alpha (TNF alpha), and has received marketing authorization for the treatment of both rheumatoid arthritis (RA) and Crohn's disease. The aim of the present survey was to assess retrospectively adverse drug reactions (ADRs) in patients treated with infliximab for RA in a rheumatology department of the Toulouse University Hospital (Rangueil Hospital). Among 32 patients included in 2000 and 2001, 43 "expected" ADRs occurred in 21 patients (65.6%) [mean age 51.4 +/- 14.0 years]. In four patients (12.5%), ADRs were cl…

Persistence and safety of subcutaneous infliximab up to 1 year after switching from intravenous infliximab in pediatric inflammatory bowel disease: a multicenter real-world cohort study.

INTRODUCTION: Real-world data regarding subcutaneous infliximab (SC-IFX) in patients with pediatric inflammatory bowel disease IBD (PIBD) is scarce. We evaluated SC-IFX as maintenance therapy in PIBD patients who switched to SC-IFX from intravenous infliximab (IV-IFX) treatment. METHODS: In this retrospective multicenter study we identified PIBD patients who switched to SC-IFX. The primary outcome was treatment persistence at up to 12 months post-switch. Secondary outcomes included relapse rate (defined as Pediatric Ulcerative Colitis Activity Index [PUCAI]  ≥10/ weighted PCDAI  ≥ 12.5 with b…

Dual Therapy with Tacrolimus and Infliximab for Patients with Refractory Ulcerative Colitis.

BACKGROUND: In refractory cases of ulcerative colitis (UC) where tacrolimus treatment is ineffective, colectomy is often required. This study aimed to evaluate the efficacy and safety of dual therapy with tacrolimus and infliximab. We also assessed the effectiveness of infliximab monotherapy in maintaining remission after discontinuation of dual therapy. METHODS: We included 21 patients with moderate-to-severe UC treated at our institution between March 2011 and August 2024, in which infliximab was added because clinical remission was not achieved with tacrolimus. In cases where clinical remi…

When to Switch to Subcutaneous Infliximab? The RE-WATCH Multicenter Study.

BACKGROUND: The infliximab (IFX) biosimilar, CT-P13, is available as an intravenous (IV) and subcutaneous (SC) formulation. Although current indications allow the transition from IV CT-P13 to SC CT-P13 after two IV administrations, some clinicians prefer to postpone switching until stable clinical remission has been achieved. METHODS: We evaluate the endoscopic response, treatment persistence, clinical remission, endoscopic remission, and safety profile after one year of treatment with IFX in patients switched from IV to SC after 6 weeks (early switch group) or after 6 months (late switch gro…

Comparative Effectiveness of Tacrolimus and Infliximab in Hospitalized Patients With Ulcerative Colitis.

INTRODUCTION: Cyclosporine or infliximab (IFX) have been used to avoid surgery in patients with severe refractory ulcerative colitis (UC). Tacrolimus (Tac) is occasionally used as an alternative to cyclosporine; however, the comparative efficacy of Tac and IFX has not been reported. We aimed to compare the effectiveness of Tac and IFX in hospitalized patients with UC. METHODS: In a propensity score-matched cohort derived from a large nationwide database, 4-year effectiveness was compared between patients initiated on Tac and those initiated on IFX. The primary outcome was the colectomy rate d…

Composite Lymphoma after Treatment with Infliximab for Sarcoidosis and Ankylosing Spondylitis.

Lymphoid proliferations and lymphomas associated with immune deficiency and dysregulation (LP/L-IDD) are rare entities associated with the use of immunosuppressive drugs (ISD) for autoimmune conditions. Composite lymphomas, featuring both B-cell and T-cell lymphomas, are infrequent, and their occurrence as LP/L-IDD is rare. We herein report the case of a 70-year-old man with right pleural effusion and lymphadenopathy, who was treated with infliximab for sarcoidosis and ankylosing spondylitis. A biopsy revealed a composite lymphoma of DLBCL and PTCL-NOS. CHOP chemotherapy led to significant rem…

Ramsay Hunt Syndrome in a Patient with Ulcerative Colitis Treated with Infliximab.

UNLABELLED: Ramsay Hunt syndrome is a rare complication of herpes zoster that results from reactivation of varicella-zoster virus in the geniculate ganglion of the VII cranial nerve. Immunosuppression can lead to reactivation of latent varicella-zoster virus, resulting in herpes zoster. Here, we present a case of Ramsay Hunt syndrome in a patient with ulcerative colitis under treatment with infliximab. LEARNING POINTS: Ramsay Hunt syndrome is a rare form of herpes zoster and characterized by the presence of otalgia, facial palsy, vertigo and vesicular rash in the external ear or on the oropha…
