BACKGROUND: The predictive value of short-term arm pain relief after 'indirect' cervical epidural steroid injection (ESI) for the 1-month treatment response has been previously demonstrated. It remained to be answered whether the long-term response could be estimated by the early post-interventional pain course as well.
METHODS: Prospective observational study, following a cohort of n = 45 patients for a period of 24 months after 'indirect' ESI for radiculopathy secondary to a single-level cervical disk herniation (CDH). Arm and neck pain on the visual analog scale (VAS), health-related quali

BACKGROUND: Steroids often are administered into the epidural space through the transforaminal epidural (TFE) route to treat lumbosacral radicular pain secondary to herniated intervertebral discs. However, their efficacy and safety compared with transforaminal epidural local anesthetics (LAs) or saline injections is unclear.
METHODS: We reviewed randomized controlled trials that compared TFE injections of steroids (with or without LA) with LA or saline in adult outpatients with lumbosacral radicular pain secondary to herniated intervertebral disks. Databases searched included MEDLINE, EMBASE,

OBJECTIVE: The aim of this study is to evaluate the efficacy of the different types of epidural injections (EI) to prevent surgical intervention in patients suffering from chronic sciatica due to lumbar disc herniation (LDH).
MATERIAL AND METHODS: Studies were identified by searching PubMed, MEDLINE, and Google Scholar to retrieve all available relevant articles. Lists of references of several systematic reviews were also used for scanning further references. Publications from the past ten years (2006-2016) were considered, and all studies selected were in the English language only. The studi

BACKGROUND: As our population ages, neurogenic claudication (NC) from central canal stenosis of the lumbar spine is becoming an increasingly common condition. Studies have been undertaken to assess the efficacy of caudal, interlaminar, or unilateral transforaminal epidural injections, but bilateral transforaminal epidural injections (BTESIs) have not been evaluated to date.
OBJECTIVE: To assess the therapeutic value and long-term effects of fluoroscope-guided BTESIs in patients with NC from degenerative lumbar spinal stenosis (DLSS) of the central spinal canal.
DESIGN: Case series.
SETTING:

BACKGROUND: While low back pain is the number one cause of disability in the United States, lumbar spinal stenosis along with intervertebral disc herniation and degenerative spondylolisthesis is one of the 3 most common diagnosis of low back and leg pain for which surgery is performed. Numerous modalities of treatments including drug therapy and complex surgical fusions have been recommended for treatment of central spinal stenosis. Epidural injections are one of the commonly performed nonsurgical interventions in managing central spinal stenosis; however, there has been paucity of literature

BACKGROUND: Transforaminal and interlaminar epidural steroid injections are commonly used interventional pain management procedures in the treatment of radicular low back pain. Even though several studies have shown that transforaminal injections provide enhanced short-term outcomes in patients with radicular and low back pain, they have also been associated with a higher incidence of unintentional intravascular injection and often dire consequences than have interlaminar injections.
OBJECTIVES: We compared 2 different approaches, midline and lateral parasagittal, of lumbar interlaminar epidu

OBJECTIVES: Evaluate the prevalence of an anomalous posterior vertebral artery (VA) in the neural foramen and to see if any factors might correlate with proximity of the VA to needle location in a cervical transforaminal epidural steroid injection (CTFESI).
METHODS: A radiologist with subspecialty training in neuroradiology documented VA location in relation to the neural foramen on axial views of 198 consecutive computed tomography angiograms done for various reasons, 11 were excluded because of poor imaging or occluded VA. The levels of C2-3 through C6-7 were evaluated, where the VA courses

Radicular back pain is defined as low back pain radiating into the lower extremities in a dermatomal pattern caused by compression or irritation of the nerve root. Fluoroscopy is an excellent tool for the management of lower limb radiculopathies that are secondary to discogenic pathologies not responding to the conservative trial. Transforaminal epidural steroid injection technique is a minimally invasive intervention for radiculopathies, which can easily be done in a day care setting, and it should be considered before surgery; however, precision and expertise is required.

BACKGROUND: For successful outcomes, interventional pain management procedures commonly require the patient to be prone under fluoroscopy. Nerve injuries can occur from stretching, compression, or direct nerve insult.
CASE REPORT: In this case report, we describe a patient receiving a cervical epidural steroid injection while prone over a 17-minute period. He immediately postoperatively reported one-sided severe hand pain and spasm in his lateral fingers. He initially reported cervical radicular pain on the contralateral side, which the patient said felt better immediately after being injecte

OBJECTIVE: To determine the distribution of injectate (iohexol) at the feline sacrococcygeal space using CT and thereby identify the volume necessary to reach the 10th thoracic intervertebral foramina (T10-T11), where nociceptive neurons from the ovaries enter the spinal cord.
METHODS: 7 healthy, purpose-bred, 1- to 2-year-old intact female domestic shorthair cats were anesthetized and positioned in sternal recumbency. After baseline vertebral column CT, a stimulating needle connected to a nerve stimulator (current, 0.2 mA; frequency, 2 Hz; pulse width, 0.1 milliseconds) was inserted at the s

This is the case of a 33-year-old male with traumatic paraplegic lumbar spinal cord injury after knife assault, who was unable to participate in an intensive inpatient rehabilitation course due to bilateral lower limb spasticity. For therapeutic management of spasticity at the bedside in the inpatient rehabilitation setting, we performed an epidural steroid injection to the right L4-L5 interspace. After the intervention, a significant decrease in spasticity was noted. The patient could subsequently tolerate sit-to-stand transfers with a standing frame and ambulate with the an exoskeleton devic

OBJECTIVE: Cervical epidural steroid injections (ESIs) can provide effective pain management for patients suffering from chronic neck pain due to various pathological changes of the cervical spine. There are several rare adverse effects reported from interventional pain procedures, including persistent hiccups ("singultus"). Based on a limited number of cases, we propose a modified treatment algorithm for this adverse outcome (Fig. 3).
CASE REPORT: Singultus has been documented as an adverse effect of interventional pain procedures, including epidural steroid, facet joint, and sacroiliac join

Cerebral venous sinus thrombosis (CVST) is an exceedingly rare complication of epidural anesthesia, with only a handful of known cases after epidural steroid injection (ESI). We report a case of CVST in a 33-year-old male patient that presented with headache after lumbar ESI. His clinical status initially improved on anticoagulation in the intensive care unit. However, he had a sudden worsening of cerebral edema that required an emergent hemicraniectomy. Ultimately, the patient was pronounced dead by neurologic criteria. This case highlights the importance of keeping this rare but potentially

Cervical radicular pain following anterior cervical discectomy and fusion (ACDF) is a challenging condition, particularly in the presence of hardware complications. This case report discusses the successful use of a cervical interlaminar steroid injection to alleviate radicular pain in a patient who presented with a fractured screw following C6/7 ACDF. The patient's symptoms, treatment plan, and outcome are reviewed, highlighting the success of an interlaminar steroid injection in managing radicular pain until the patient could receive corrective surgery.

BACKGROUND: Cervical radiculopathy is a common cause of neck pain, with radiation into the upper extremity in a dermatomal pattern. Corticosteroid injection is a conservative management option with a low risk of major adverse events. No reviewed literature or case reports have implicated phrenic nerve injury secondary to cervical transforaminal epidural steroid injection (CTFESI).
OBSERVATIONS: A 45-year-old man with severe right C6 radiculopathy secondary to a large right-sided C5-6 herniated intervertebral disc presented to the pain management clinic, where he received a right-sided C6 CTFE

Epidural steroid injections are the cornerstone of symptomatic relief in select spinal pain conditions and remain the most common interventional procedure; however, many patients with low back pain cannot tolerate corticosteroids due to multiple inherent side effects and contraindications. Plant-based or homeopathic injectable therapies, such as Traumeel®, may be viable alternatives to corticosteroids in providing pain relief for those patients that cannot use corticosteroids in epidural injections. A chart review case study exploring the effectiveness of Traumeel epidural injections as an alt

BACKGROUND: Lumbar foraminal spinal stenosis (LFSS) is a common cause of radicular pain in the lower extremities. Transforaminal epidural steroid injection (TFESI) is being used widely for controlling radicular pain induced by LFSS. The efficacy of TFESI has been demonstrated in previous studies. However, no study has evaluated the outcome of TFESI according to the severity of LFSS.
OBJECTIVE: In this study, we evaluated the outcome of TFESI in patients with chronic lumbar radicular pain due to LFSS according to the severity of LFSS by using magnetic resonance imaging (MRI).
