Early Pregnancy Loss Management in the Emergency Department vs Outpatient Setting.

IMPORTANCE: Early pregnancy loss (EPL), or miscarriage, is the most common complication of early pregnancy, and many patients experiencing EPL present to the emergency department (ED). Little is known about how patients who present to the ED with EPL differ from those who present to outpatient clinics and how their management and outcomes differ. OBJECTIVE: To compare the management and outcomes of patients with EPL who present to the ED vs outpatient clinics. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used the IBM MarketScan Research Database, a national insurance cl

The Effect of Payer Type on Patient-Reported Outcomes in Total Joint Arthroplasty Is Modulated by Baseline Patient Characteristics.

BACKGROUND: Patient-reported outcomes (PROs) are gaining an important role in the assessment of quality of care. There are currently limited data on the effect of payer type on PROs in total joint arthroplasty (TJA). This study compared both disease-specific and general health PROs among patients stratified according to their payer type. METHODS: Our institutional joint registry was queried for patients who underwent primary, elective, and unilateral hip and knee arthroplasty. Patients were divided according to their insurance type at the time of surgery into 3 groups: Medicaid, Medicare, or

Timely orchiopexy by 18 months of age: Are we meeting the standards defined by the 2014 AUA guidelines?

BACKGROUND: Cryptorchidism is one of the most common reasons for pediatric urology referral and one of the few pediatric urologic conditions in which there are established AUA guidelines that recommend orchiopexy be performed before 18 months of age. While access to timely orchiopexy has been studied previously, there is no current study with data from a national clinical database evaluating timely orchiopexy after the AUA guidelines were published. Additionally, prior studies on delayed orchiopexy may have included patients with an ascended testis, which is a distinct population from those wi

The interaction between parental concern and socioeconomic status in pediatric hydrocephalus management.

OBJECTIVE: Parent or guardian involvement is implicit in the care of pediatric patients with hydrocephalus. Some parents and guardians are more engaged than others. The relationship between socioeconomic status (SES), the level of parental concern about their child's hydrocephalus management and future, and overall health status has not been clearly delineated. In this study, the authors sought to clarify this connection using hydrocephalus patient-reported health outcomes. METHODS: This cross-sectional study included children with surgically managed hydrocephalus whose parent or guardian com

Virtual surgical planning: Balancing esthetics, practicality, and anticipated stability in a complex Class III patient.

The treatment of skeletal Class III malocclusion with anterior open bite is a complex and challenging aspect of orthodontics. Facial esthetic factors, practicality and the anticipated stability of a provisional surgical plan must all be factored into the final decision of the actual orthodontic-orthognathic treatment. This case report presents the multidisciplinary treatment of a 39-year-old female patient with skeletal Class III, severe open bite with first dental contact being on the second molars, lateral crossbite, and crowding in both arches. The nonextraction treatment started with align

Factors that affect surgical rates in Iowa.

STUDY DESIGN: This study analyzed insurance claims to estimate the probability of medical and surgical treatments in different Iowa communities. The likelihood of surgical treatment was associated with patient characteristics of age and gender as well as hospital characteristics of size (number of beds), occupancy rate, and number of staff. OBJECTIVES: Our findings are being used by a study group of 25 physicians to understand the causes of variation in surgical rates for low back pain. Medical education and other interventions are being implemented. SUMMARY OF BACKGROUND DATA: Hospitalizati

Improving equitable collection and analysis of PROMIS Global Health data over time following spine surgery: characterizing survey nonresponse and missing data.

BACKGROUND CONTEXT: Patient-reported outcome measures (PROMs), the gold standard for outcome assessment in spine surgery, exhibit variability over time. Incomplete PROM collection, however, introduces nonresponse bias and limits the generalizability of time-based analyses of outcomes. PURPOSE: This study compared PROM-respondents and nonrespondents in spine surgery to characterize survey nonresponse and improve equitable patient representation in time series PROM analyses. STUDY DESIGN: Retrospective study. PATIENT SAMPLE: Patients undergoing surgery at a large, tertiary care center in the

Cost of pelvic organ prolapse surgery in the United States.

OBJECTIVE: To estimate the annual direct cost to society of pelvic organ prolapse operations in the United States. METHODS: We multiplied the number of pelvic organ prolapse operations identified in the 1997 National Hospital Discharge Survey by national average Medicare reimbursement for physician services and hospitalizations. Although this reimbursement does not estimate the actual cost, it is a proxy for cost, which estimates what society pays for the procedures. RESULTS: In 1997, direct costs of pelvic organ prolapse surgery were 1012 million dollars (95% confidence interval [CI] 775 do

Nationwide Ethnic/Racial Differences in Surgical Treatment of Discoid Meniscus in Children: A PHIS Database Study.

BACKGROUND: Discoid meniscus, a congenital meniscus variant, may have greater incidence in Asian populations. No US population-based studies have examined the discoid meniscus ethnic/racial distribution. In pediatric patients undergoing meniscus surgery, it is hypothesized that ethnic/racial variability exists in patients with discoid meniscus and this variability is different than in patients with medial meniscus tears. METHODS: The Pediatric Health Information System was queried from 48 hospitals to examine patients 18 years of age and younger between 2015 and 2019, using International Clas

The impact of procedure type, jurisdiction and other factors in workers' compensation on work-disability outcomes following carpal tunnel surgery.

Carpal tunnel syndrome (CTS) is often work-related and associated with prolonged disability. While surgical treatment is common, an alternative endoscopic procedure has been promoted as a way of achieving better outcomes. However, little is known about typical use of the procedure and whether it actually achieves outcomes in community settings. From workers' compensation claims reported to an insurer in six jurisdictions during 1995-1999, we identified 1697 individuals with a single CTS surgery, 17% were treated using the endoscopic procedure. Bivariate analyses of post-surgical outcomes demon

Ambulatory Surgery Centers in Cardiac Electrophysiology: Current Status and Future Directions.

Ambulatory surgical centers (ASCs) are rapidly expanding their role in cardiac electrophysiology (EP). Several studies have demonstrated that they provide comparable safety, effectiveness, and efficiency to hospital settings for elective, nonacute EP procedures. ASC's provide superior patient-centered care with increased convenience, improved access, operational efficiency, and cost savings, which might potentially mitigate delays in EP procedures and improve patient outcomes. Use of appropriate patient selection criteria, standardized workflows, protocols for same-day discharge, and transfer

Incidence and preoperative risk factors for failed back surgery syndrome: a nationwide population-based cohort study.

Failed back surgery syndrome (FBSS), recently redesignated as persistent spinal pain syndrome type 2, is a common complication after lumbar spine surgery, yet population-level data on its incidence and preoperative determinants remain limited. This study aimed to quantify the incidence of FBSS under multiple definitions and to identify preoperative risk factors for its development. This retrospective cohort study analyzed claims data from the Korean National Health Insurance Service (2009-2023), including 1,029,500 adults who underwent lumbar spine surgery. FBSS was operationalized using two m

Validation of I71.3 code for ruptured abdominal aortic aneurysm in Korea: misplaced diagnosis in claims data.

PURPOSE: Studies in western countries have shown a decline in the incidence of ruptured abdominal aortic aneurysm (rAAA) with advancements in endovascular repair and screening. However, according to health insurance data in Korea based on rAAA code (I71.3), overall rAAA has been increasing. This study aimed to validate the I71.3 code for rAAA and attempt to define the true incidence of rAAA in Korea. METHODS: A 20-year multicenter retrospective review of rAAA was undertaken from the period of January 1, 2000 to December 31, 2020. All patients were diagnosed with the rAAA code I71.3 in each of
