The most frequent and severe complication of pancreaticoduodenectomy is pancreatic fistula due to dehiscence of the pancreas anastomosis. Diverticular disease is a common clinical condition among Western populations, which increases with age. It can present in a variety of manners and has myriad of potential disease complications. We present a rare case of an adult patient with an extraperitoneal complications of a diverticular perforation presenting with facial swelling due to a colo-urachal fistula associated with a patent urachal remnant. Perforation should be considered in patients presenting with surgical emphysema with background of diverticular disease.

We report on a case of a 69-year-old man with a benign stricture of the sigmoid colon due to diverticulosis, complicated by a colovesicular fistula. Due to severe co-morbidity, surgical excision of the fistula and resection of the sigmoid colon were not indicated. Instead, the stricture and the fistula were managed via the insertion of a self-expanding, uncoated metallic stent (SEMS). The clinical symptoms disappeared after insertion of the SEMS. We conclude that SEMS may be a possible therapeutic tool in selected cases of benign stricture of the colon, with or without fistula.

A case of gallstone ileus in an elderly woman is described and attention is drawn to the insidious clinical presentation and the significance of the often subtle radiological signs. The data from the literature specifies these aspects and allows a more complete review of this relatively rare pathology in order to remind its importance in differential diagnosis to emergency clinicians and radiologists.

Two patients with duodenal obstruction resulting from erosion of a gallstone through the wall of the gallbladder and into the duodenum are described. Both cases were diagnosed endoscopically and successfully treated by a one-stage procedure to remove the obstruction and gallbladder and to close the fistula without complication.

A case of vesicosigmoidal fistula secondary to the diverticulitis of colon is reported. A 63-year-old man was admitted to our clinic with the chief complaint of pneumaturia. Cystoscopy revealed an edematous, papillomatous lesion at the left posterior bladder wall. Although multiple diverticulosis of descending and sigmoid colon were demonstrated by barium enema, the presence of a fistula from the intestine to bladder was not confirmed. The number of recurrences ranged from 2.1% to 14.3% depending on the degree of fistula complexity.
