Kounis Syndrome in Cardiac Surgery: Pathophysiology, Antimicrobial Triggers, and Perioperative Recognition and Management.

BACKGROUND: Kounis syndrome is an allergic acute coronary syndrome precipitated by coronary vasospasm, plaque destabilization, stent thrombosis, or bypass occlusion. Cardiac surgery represents a uniquely high-risk setting due to cardiopulmonary bypass-associated inflammation and exposure to multiple pharmaceutical agents. Importantly, Kounis syndrome remains underrecognized in this context, as classical signs of anaphylaxis may be masked under general anesthesia and cardiopulmonary bypass, while ischemic manifestations may be misattributed to other perioperative conditions. METHODS: A narrati

Preoperative embolization of a pulmonary carcinoid tumor with unusual arterial supply: a case report.

BACKGROUND: Pulmonary carcinoid tumors are rare neuroendocrine neoplasms that carry a risk of bleeding. We report a case of a tumor with unusual arterial supply from multiple sources including collateral from the renal artery, successfully managed with preoperative embolization followed by surgical resection. CASE PRESENTATION: A 39-year-old woman with no significant medical history presented with chronic cough. Chest radiography revealed an opacity in the right hilar region. Computed tomography (CT) showed a mass obstructing the right middle lobe bronchus. 18 F-fluorodeoxyglucose positron emi

Comparison of End-tidal Sevoflurane Concentration Required for Optimal Depth of Anesthesia during Opioid-free General Anesthesia Using Intravenous Lignocaine versus Morphine.

BACKGROUND AND AIMS: In opioid-free anesthesia (OFA) protocol, intravenous lignocaine can offer perioperative analgesic benefits. However, m … ostintubation, while it was lower at 60 min postintubation. Neither group reported any incidence of intraoperative awareness. CONCLUSION: Lignocaine-based OFA required a significantly higher end-tidal sevoflurane to maintain sufficient anesthetic depth compared to morphine-based anesthesia. In addition, sevoflurane use, intraoperative hypertension, and the need for suppl …

Bezold-Jarisch Reflex-Induced Cardiac Arrest During Colonoscopy Under Sedation: A Case Report.

BACKGROUND Bezold-Jarisch reflex (BJR) is a physiologic cardioinhibitory reflex, but its occurrence can lead to severe bradycardia and hypotension, or even cardiac arrest. Due to the rarity of its occurrence and the lack of awareness among most anesthesiologists, this clinical presentation may be mistaken for other adverse cardiovascular events, and effective preventive and curative measures may not be taken. CASE REPORT We report a case of cardiac arrest during a routine sedated colonoscopy. Following the arrest, the patient underwent cardiopulmonary resuscitation and received atropine and ep

Association of acidosis with coagulopathy and transfusion requirements in liver transplantation.

The relationship between acidosis and coagulopathy has long been described in vitro and in trauma patients, but not yet in orthotopic liver transplantation (OLT). The association of metabolic acidosis with coagulopathy and with transfusion requirements was evaluated in patients submitted to OLT. Changes in acid-base and coagulation parameters were analyzed by repeated measures. Regression analyses [adjusted for sex, age, model for end stage liver disease (MELD) score, and baseline values of hemoglobin, fibrinogen, international normalized ratio, platelets] determined the association of acid-ba

Comparing Magnetic Resonance Imaging and High-Resolution Dynamic Ultrasonography for Diagnosis of Plantar Plate Pathology: A Case Series.

Plantar plate pathology has gained noticeable attention in recent years as an etiology of lesser metatarsophalangeal joint pain. The heightened clinical awareness has led to the need for more effective diagnostic imaging accuracy. Numerous reports have established the accuracy of both magnetic resonance imaging and ultrasonography for the diagnosis of plantar plate pathology. However, no conclusions have been made regarding which is the superior imaging modality. The present study reports a case series directly comparing high-resolution dynamic ultrasonography and magnetic resonance imaging. A

[Surgical assessment of complications after thyroid gland operations].

BACKGROUND: The extent, magnitude and technical equipment used for thyroid surgery has changed considerably in Germany during the last decade. The number of thyroidectomies due to benign goiter have decreased while the extent of thyroidectomy, nowadays preferentially total thyroidectomy, has increased. Due to an increased awareness of surgical complications the number of malpractice claims is increasing. OBJECTIVES: In contrast to surgical databases the frequency of complications in malpractice claims reflects the individual impact of complications on the quality of life. In contrast to surgi

Relevance of light conditions for clinical in situ Raman spectroscopy.

BACKGROUND: Clinical Raman spectroscopy is a promising optical technology for intraoperative brain tumor delineation and diagnosis. As it exploits weak signals, illumination of the operating room (OR) and OR devices might impact the acquired spectra. The aim of this study was to investigate which optical artifacts can be tolerated during the intraoperative analysis of brain tumors in situ while still allowing the extraction of meaningful Raman spectra. METHODS: Tissue samples of rat brain and human brain tumors were investigated in an experimental OR, and background signals and Raman spectros

Acute Gallbladder Volvulus in a 93-Year-Old High-Risk Patient: A Case Report.

BACKGROUND A gallbladder volvulus is a rare and life-threatening emergency requiring immediate surgery to prevent gallbladder perforation and sepsis. A correct preoperative diagnosis is often difficult. CASE REPORT We describe the case of a 93-year-old woman with an acute gallbladder volvulus that was initially misdiagnosed as acalculous cholecystitis. She was started on i.v. antibiotics for conservative management due to a symptomatic, high-grade aortic valve stenosis. After her clinical condition worsened, an emergency cholecystectomy was scheduled. After an interdisciplinary discussion with

Sterile Inflammatory Response Mimicking Infection after DuraGen Duraplasty: A Case Report.

DuraGen, a collagen matrix-based artificial dura mater, is widely used in neurosurgery because of its biocompatibility and ease of use. Although generally considered safe, we present a rare case of a foreign body reaction to DuraGen presenting as a sterile inflammatory response. A 40-year-old woman underwent total resection of a right convexity meningioma, with dural repair using DuraGen. On postoperative day 31, she developed headache, nausea, and vomiting. Imaging revealed significant cerebral edema around the surgical cavity with midline shift but no signs of infection on diffusion-weighted

Femorocele: A Rare Form of Femoral Hernia.

INTRODUCTION: Femorocele, also known as a hydrocele of the femoral hernia (FH) sac, is a rare form of FH. Initial misdiagnosis is common not only in FH but is even more frequent in femorocele, due to its rarity and non-distinctive features that make preoperative recognition challenging; in many cases, femorocele is identified only intraoperatively or postoperatively. This report describes a case of femorocele that initially posed a diagnostic challenge and was initially misdiagnosed as a ganglion. CASE PRESENTATION: A 63-year-old woman presented with a right sided groin swelling which was sus

Non-recurrent right laryngeal nerve identified during endoscopic thyroidectomy via areolar approach: a case report.

BACKGROUND: Nonrecurrent laryngeal nerve (NRLN) is a rare but significant anatomical variation in thyroid surgery, and lack of awareness of NRLN may lead to intraoperative injury. Here, we report a clinical case of NRLN discovered during endoscopic thyroid surgery via total areola approach in a 23-year-old female patient. CASE PRESENTATION: A 23-year-old female patient presented with bilateral thyroid nodules for three years. She underwent bilateral thyroid nodule fine-needle aspiration biopsy and BRAF gene testing at our hospital, with results indicating bilateral papillary thyroid carcinoma

Clinicopathologic Features of Thoracolumbar Interdural Disc Herniations: A Retrospective Case Series with a Systematic Literature Review.

BACKGROUND: Transdural disc herniations represent about 0.3% of all herniated discs. Preoperative imaging rarely demonstrates this condition. Therefore, diagnosis of transdural disc herniation is usually made intraoperatively. We describe the clinicopathologic features of extremely rare cases of thoracic and lumbar interdural disc herniations mimicking spinal intradural en plaque tumors. METHODS: This is a retrospective case series with a systematic literature review. Clinical presentation, imaging, differential diagnosis, intraoperative microsurgical findings, and possible pathogenesis were
