Treatment of hypopharyngeal cancer depends on the extent and location of the tumour, patient-specific or by factors (age, performance status, co-morbidity) and the presence of psychosocial support. Options available for the treatment of hypopharyngeal cancer consist of surgery and non-surgery - radiotherapy, chemoradiation, bioradiation, or a combination of these modalities. To maintain normal functioning as much as possible, functional organ preservation is widely recommended and generally utilizes radiotherapy and/or chemoradiation. Although functional organ-sparing approaches can permit lar

This case provides support for electroconvulsive therapy as a safe treatment in adolescents with a feeding tube. The patient presented to our hospital with symptoms of catatonia with minimal oral intake. She had stopped eating, had minimal interaction with her environment, and spent weeks with a nasogastric tube for nutritional support. She had been referred for electroconvulsive therapy but was unable to find a local provider who would perform it on an adolescent with a nasogastric tube. She came to our hospital and received 9 rounds of electroconvulsive therapy with improvement of her catato

Enteral nutrition has a relatively low incidence of major complications. The most common complications are mechanical problems, bronchoaspiration and diarrhoea. A rare complication associated with the use of enteral nutrition is oesophageal bezoar. A bezoar is a body of undigested and partially digested matter in the gastrointestinal tract. The main risks factors are gastric motility dysfunction and the use of opiates or sucralfate. The aim of this paper was to present two cases of oesophageal obstruction resulting from the formation of bezoars due to enteral nutrition. Both patients experienc

Whether nasogastric or nasojejunal decompression (ND) prevents anastomotic leakage, hastens the return of bowel function, and shortens hospital stay after gastrectomy for gastric cancer has long been controversial. We evaluated the necessity of routine ND after radical gastrectomy for gastric cancer with a systematic review and meta-analysis. We searched literature published prior to January 2014 in PubMed, Embase, Cochrane Library, Web of Science, and BIOSIS Previews for relevant randomized controlled trials (RCTs). Only prospective RCTs comparing individuals with and without ND after gastrec

INTRODUCTION: Spontaneous gastric perforation is a well known surgical emergency which carries significant mortality and morbidity. Well documented causes in adults include peptic ulcer disease, drugs such as non-steroidal and gastric malignancy. Iatrogenic causes still remain relatively rare. We report an interesting case of an acutely unwell young man who developed gastric perforation secondary to nasogastric intubation.
CASE REPORT: A 32 year old man initially treated for gastroenteritis underwent laparotomy for acute intra-abdominal bleeding. This was found to be secondary to a ruptured l

Aerophagia, characterized by symptoms related to repetitive swallowing of air, is a functional gastrointestinal disorder. In some cases, severe aerophagia causes massive bowel distention and leads to volvulus, ileus, and even intestinal necrosis and perforation. A 10-year-old intellectually disabled boy was referred to our unit due to severe abdominal distention, bilious vomiting, no passage of feces and flatus during the previous 3 days. He had experienced episodes of severe abdominal distention and flatulence over the past 2-3 years. In the exploratory laparotomy, two old colonic perforation

BACKGROUND: Percutaneous endoscopic gastrostomy (PEG) tubes are often placed in head and neck cancer patients to provide nutritional support, but studies have found the complication rates to be higher than other subsets of patients who undergo PEG placement. Complication rates as high as 50% have been reported, with the bulk of these complications being PEG site issues (i.e., cellulitis, abscess, fascitis, and tumor implantation). Because the pull technique has been the primary technique used, the theory is that the transoral tube passage is the source of the complications in these patients. A

Between January 1997 and December 1998, 30 consecutive children with suspected biliary atresia (BA) were selected to assess whether duodenal intubation (DI) and testing of aspirate for bile would help to rule out BA. Duodenal fluid was aspirated every 2 h for 24 h and tested for bile. A HIDA scan was also done in all cases. Every patient underwent a peroperative cholangiogram (POC) and liver biopsy; a Kasai portoenterostomy was done in indicated cases. In 22 cases all three investigations (DI, HIDA scan, POC) suggested BA. In 3 the HIDA scan ruled out BA, but DI and POC suggested BA. In 2 othe

OBJECTIVE: We conducted a retrospective study to evaluate two different types of percutaneous fluoroscopic gastrostomy procedures and catheters.
MATERIALS AND METHODS: Between July 8, 1999, and August 4, 2000, 80 percutaneous gastrostomy catheters were placed in 80 patients in 80 attempts. Twenty-five 16-, 18-, or 20-French balloon-retained catheters and 55 20-French mushroom-retained catheters were inserted. Typically, the type of catheters placed was based on operator preference. However, balloon-retained tubes were preferred for use in patients with obstructive head and neck or esophageal

The condition of patients with carcinomatous ileus is marked by pain in the final stage of cancer, which ties them to hospital beds. Due to this state they lose valuable time with family. On the other hand, there is not yet any effective carcinomatous ileus therapy and surgeons face much difficulty in the treatment of carcinomatous ileus. We performed pergastrostomal intestinal decompression, and found that this tube is managed easily compared with general ileus tubes and can readily decompress the intestinal tract. A device to improve the QOL of terminal stage cancer patients, in particular Q

Although clinical and experimental evidence favours early enteral feeding in the critically injured, provision of such enteral feeds can be difficult. Gastric feeds are often not tolerated and may lead to aspiration. An intolerance of gastric feeds leads to a cumulative energy and protein deficit and may result in the initiation of expensive parenteral nutrition (TPN). An alternative and under-utilized technique to ensure enteral access in trauma victims is the insertion of a transpyloric (nasojejunal) feeding tube during emergent laparotomy. We have employed this method of enteral access with

Post pyloric placement of small bore feeding tubes can be an elusive goal. Failure to achieve the post pyloric position can result in fatal complications. The following study presents a cost-effective, rapid technique for achieving post pyloric placement of feeding tubes utilizing gastric insufflation and right lateral decubitus positioning. Four study groups were identified. Group I utilized the new technique as first attempt (n = 21). Group 2 utilized the traditional, standard technique of insertion as first attempt (n = 20). Group 3 consisted of Group I failures after which the new techniqu

The case is described of a 29-year-old man with a history of recurrent attacks of paroxysmal supraventricular tachycardia. The patient presented to the emergency ward with complaints of palpitations which had not been relieved by vagal stimulation maneuvers and propranolol. When intravenous metoprolol failed to convert the rhythm, a nasogastric tube was passed which immediately converted the tachycardia to normal sinus rhythm. The literature is reviewed and problems and complications of various techniques are discussed.

A technique that allows for endoscopic placement of a long intestinal tube with an inflatable balloon well beyond the pylorus is described. This procedure has been successful in 24 patients; is well tolerated; removes air and fluid from the stomach, duodenum and upper part of the jejunum, and can be performed in less than 45 minutes. It uses equipment that is standard in all hospitals and can be performed by anyone experienced in performing endoscopy of the upper part of the gastrointestinal tract. It is safe, easy to perform and improves the efficacy of long tube decompression and, therefore,

Pulmonary aspiration of regurgitated feeding material is a recognized complication of enteral nutrition by feeding tube. Nasopulmonary intubation with feeding tubes has been reported infrequently. We present two patients with such complications. In one, nasopulmonary intubation resulted in a pneumothorax. In the other unrecognized misplacement resulted in pneumonia and hydrothorax. Similar cases in the literature are discussed. Common features of this complication are: (1) patients who are obtunded, debilitated, or without gag reflex; (2) previous successful insertion of a tube; (3) successful
