The relationship between irritability and suicidal ideation: the chain mediating effects of rumination and distress tolerance.

BACKGROUND: Irritability is a core symptom of Major Depressive Disorder (MDD) in children and adolescent. Previous studies have reported a positive association between irritability and suicidal ideation. Based on the cognitive model of suicide and emotion regulation theory, the current study aimed to explore the role of rumination and distress tolerance in the relationship between irritability and suicidal ideation in adolescent patients with MDD. METHODS: A cross-sectional survey was conducted with 319 adolescent patients with MDD. Patients were clinically interviewed using the Development a

Adverse drug reactions of leukotriene receptor antagonists in children with asthma: a systematic review.

BACKGROUND: Asthma is the most common chronic condition of childhood. Leukotriene receptor antagonists (LTRAs) are included in international guidelines for children and young people (CYP), but there have been highly publicised concerns about potential adverse effects. The aim was to identify and understand the reported frequency of adverse drug reactions (ADRs) attributed to LTRAs in CYP with asthma. METHODS: Embase, MEDLINE, PubMed and CINAHL were searched up to October 2020. Reference lists of eligible papers were manually screened. Eligible studies identified adverse events attributed to a

Face Emotion Processing in Pediatric Irritability: Neural Mechanisms in a Sample Enriched for Irritability With Autism Spectrum Disorder.

OBJECTIVE: Characterizing the pathophysiology of irritability symptoms from a dimensional perspective above and beyond diagnostic boundaries is key to developing mechanism-based interventions that can be applied broadly. Face emotion processing deficits are present in youths with elevated levels of irritability. The present study aimed to identify the neural mechanisms of face emotion processing in a sample enriched for irritability by including youths with high-functioning autism spectrum disorder (HF-ASD). METHOD: Youths (N = 120, age = 8.3-19.2 years) completed an implicit face emotion tas

Pharmacological management of behavioral disturbances in children and adolescents with autism spectrum disorders.

Autism spectrum disorder (ASD) is a heterogeneous neuropsychiatric condition that, based on recent CDC estimates affects an estimated 1 in 59 American children. Behavioral treatments remain the mainstay of treatment for the core symptoms of ASD including communication deficits, social interaction deficits and repetitive behavior. However, youth with ASD may also have severe behavioral challenges including irritability, aggression, and hyperactivity. Currently there are only two medications (risperidone and aripiprazole) approved by the FDA for the treatment of irritability associated with ASD

Dopamine antagonists for treatment resistance in autism spectrum disorders: review and focus on BDNF stimulators loxapine and amitriptyline.

Drug development and repurposing are urgently needed for individuals with autism spectrum disorders (ASD) and psychiatric comorbidity, which often presents as aggression and self-injury. Areas covered: We review dopamine antagonists, including classical and atypical, as well as unconventional antipsychotics in ASD. The older antipsychotic loxapine is discussed in terms of preliminary albeit limited evidence in ASD. Emerging promise of amitriptyline in ASD is discussed, together with promising BDNF effects of loxapine and amitriptyline. Expert opinion: In ASD, pharmacotherapy and specifically d

The State of the Art of the DSM-5 "with Mixed Features" Specifier.

The new DSM-5 "with mixed features" specifier (MFS) has renewed the interest of the scientific community in mixed states, leading not only to new clinical studies but also to new criticisms of the current nosology. Consequently, in our paper we have reviewed the latest literature, trying to understand the reactions of psychiatrists to the new nosology and its epidemiological, prognostic, and clinical consequences. It seems that the most widespread major criticism is the exclusion from the DSM-5 MFS of overlapping symptoms (such as psychomotor agitation, irritability, and distractibility), with

Does temperature or sunshine mediate the effect of latitude on affective temperaments? A study of 5 regions in Japan.

BACKGROUND: Previously, we compared the hyperthymic scores of residents in Sapporo, Koshigaya, and Oita (which are located at latitudes of 43°N, 36°N, and 33°N in Japan, respectively) using the Temperament Evaluation of Memphis, Pisa, Paris and San Diego-auto questionnaire version (TEMPS-A). We found that residents who lived at lower latitudes had higher hyperthymic temperament scores; however, the mechanism of the effect of latitude on hyperthymic temperament remained unclear. The current study examined the mediators of the latitude effect in additional regions with different annual temperatu

Risperidone for the treatment of affective symptoms in children with disruptive behavior disorder: a post hoc analysis of data from a 6-week, multicenter, randomized, double-blind, parallel-arm study.

BACKGROUND: Despite the increasing recognition of bipolar disorder in childhood, there have been no controlled, randomized clinical trials of atypical antipsychotics in this population. Preliminary data from open-label trials in children suggest that these agents might be effective in treating pediatric bipolar disorder, however. OBJECTIVE: The purpose of this post hoc analysis of data from Aman et al was to determine the effects of risperidone in the management of affective symptoms in children with disruptive behavioral disorders (DBDs). METHODS: This report presents a secondary analysis o

Clinical Phenotypes of Behavioral Variant Frontotemporal Dementia by Age at Onset.

BACKGROUND: Behavioral variant frontotemporal dementia (bvFTD) is generally considered a young-onset dementia, although age at onset is highly variable. While several studies indicate clinical differences regarding age at onset, no biomarker validated cohort studies with updated clinical criteria have been performed. OBJECTIVE: We aimed to examine behavior, cognition, and mortality over the full age spectrum in a cohort of bvFTD patients with neuroimaging, genetic, or histopathological confirmation and exclusion of positive Alzheimer's disease biomarkers or severe cerebrovascular damage. Severe affective and behavioral dysregulation in youths is associated with a proinflammatory state.

OBJECTIVE: A heritable behavioral phenotype, the so-called Dysregulation Profile (DP), characterized by extreme scores on the syndrome scales Anxious/Depressed (A/D), Attention Problems (AP), and Aggressive Behavior (AGG), has been identified on the Child Behavior Checklist (CBCL). It characterizes children with severe affective and behavioral dysregulation. The present study examined possible alterations of the inflammatory system in CBCL-DP using a clinical sample of n = 133 children and adolescents. METHOD: Participants with the CBCL-DP scoring ≥ 2.5 SDs above average constituted the CBCL-

Parenting and Peer Victimization in the Development of Callous-Unemotional Behaviors: Moderation by Irritability and Basal Cortisol.

The present study tested three aims regarding the socializing roles of peer victimization and harsh parenting in the development of callous-unemotional (CU) behaviors. First, we investigated whether peer victimization's promotive effects on the development of CU behaviors extend downward to early childhood and persist above effects of harsh parenting. We then considered whether, consistent with recent theoretical models, the effects of family and peer stressful experiences may be stronger for those who are emotionally (i.e., higher irritability) or physiologically (i.e., higher basal salivary

Irritability in children with RASopathies, insights into emotional dysregulation and social impairment.

RASopathies, including Noonan Syndrome (NS) and Neurofibromatosis type 1 (NF1), are developmental disorders caused by germline mutations in genes of the RAS-mitogen-activated protein kinase pathway (RAS-MAPK). This study investigates irritability, a highly prevalent transdiagnostic construct, in children with NS and NF1 and the impact of RASopathy status on the associations between irritability, emotional dysregulation-related disorders, and social impairment. The sample comprises 174 children aged 4–17 (age mean = 9.49; 98 females), including 113 children with RASopathies (NS n = 85, NF1 n =
