The synthesis of taurocholic and glycocholic acids in human liver homogenates and subcellular fractions in obstructive jaundice.

The synthesis of taurocholic and glycocholic acids was studied in the whole homogenate and in subcellular fractions of livers from 18 patients operated on for cholestasis and compared with a control material with clinically normal liver function. A considerable decrease of synthesis of cholic acid conjugates was found in icteric homogenates, while no significant alteration of the synthesis was found in subcellular fractions containing the bile acid activating and transferring enzyme. The synthesis of cholic acid conjugates in icteric homogenates showed an inverse significant correlation to ser

Interdigitating dendritic cell sarcoma presenting as retroperitoneal mass and obstructive jaundice. An uncommon entity of complex diagnosis.

A 60-year-old male was admitted for obstructive jaundice secondary to a 2 cm retroperitoneal lesion. Ultrasound endoscopy (UES) with fine needle biopsy (FNB) was performed, as well as endoscopic retrograde cholangiopancreatography (ERCP) with placement of a metal stent for bile duct drainage. Initially IgG4-related disease was suspected from FNB. After 8 months of treatment the patient attended with a progression evidenced in computed tomography (CT) with retroperitoneal adenopathies and liver metastasis, with involvement of duodenum and fistulation towards hepatic angle of colon. Upper endosc

Percutaneous Transhepatic Endobiliary Microwave Ablation Before Stenting for Malignant Obstructive Jaundice: Evidence Synthesis and Preliminary Technical Experience.

Malignant biliary obstruction is commonly treated with biliary stenting either endoscopically or percutaneously; however, tumor ingrowth might occlude the stent, often leading to recurrent jaundice and repeat interventions. Endobiliary microwave ablation (MWA) is an emerging adjunct intended to devitalize intraductal tumors and potentially prolong stent patency. This review assesses the state of the art of endobiliary ablation for malignant biliary obstruction, focusing on the technique and safety of percutaneous procedures, as well as patient outcomes. It also discusses the use of flexible en

Shedding of endothelial glycocalyx and increased vascular permeability in patients with obstructive jaundice: A prospective observational study.

During the perioperative period, increased vascular permeability is associated with elevated morbidity and mortality. Obstructive jaundice (OJ) has been demonstrated to increase vascular permeability. The endothelial glycocalyx (EG), comprising hyaluronic acid (HA), chondroitin sulfate (CS), and syndecan-1 (SDC1), is essential for vascular integrity. However, its role in OJ-related vascular dysfunction remains unclear. This study aimed to investigate whether EG degradation contributes to vascular hyperpermeability in OJ patients. Plasma was preoperatively collected from 35 OJ patients and 33 n

Survival impact of pancreatic stenting in unresected pancreatic head cancer with obstructive jaundice: a retrospective cohort study.

BACKGROUND: Malnutrition is common in patients with pancreatic head cancer, which can diminish their tolerance to anti-cancer treatments. Pancreatic duct blockage is one of the contributing factors leading to malnutrition. OBJECTIVES: This study aimed to clarify whether pancreatic stenting offers a survival benefit in patients with unresected pancreatic head cancer. DESIGN: Retrospective observational study. METHODS: Patients with newly diagnosed pancreatic cancers who received endoscopic retrograde cholangiopancreatography (ERCP) because of biliary tract obstruction were collected from a h

Duodenal wall hematoma imitating a pancreatic tumor as a rare cause of obstructive jaundice - a case report.

Obstructive jaundice is mostly caused by malignant lesions such as pancreatic head cancer and cholangiocarcinoma. Atypical benign conditions such as intramural duodenal hematoma (IDH), can imitate malignant processes in both clinical and radiological presentation. The diagnostic workup included computed tomography (CT), magnetic resonance cholangiopancreatography (MRCP), endoscopic retrograde cholangiopancreatography (ERCP), and endoscopic ultrasound (EUS) with biopsy. A 65-year-old patient was referred for assessment of obstructive jaundice and endoscopic retrograde cholangiopancreatography (

Tuberculous Peripancreatic Lymphadenitis Presenting as Obstructive Jaundice Due to Extrinsic Common Bile Duct Compression: A Case Report.

We report the case of a 29-year-old male from the Indian subcontinent (a tuberculosis (TB)-endemic region) residing in Qatar, who presented with a three-day history of epigastric pain, dark urine, and pale stools. He had no known TB exposure, was HIV-negative, and had received the Bacillus Calmette-Guérin or BCG vaccination in childhood. Physical examination revealed scleral icterus without peripheral lymphadenopathy. Initial imaging showed a dilated common bile duct (CBD) (10 mm) and an overdistended gallbladder with sludge. Magnetic resonance imaging demonstrated conglomerated necrotic lymph

Obstructive jaundice caused by traumatic neuroma of the common bile duct: A case report and literature review.

RATIONALE: Obstructive jaundice caused by traumatic neuroma of the common bile duct is extremely rare and easily misdiagnosed. PATIENT CONCERNS: We herein report a 66-year-old female who presented with obstructive jaundice. Magnetic resonance cholangiopancreatography showed common hepatic duct stenosis accompanied by intrahepatic bile duct dilation. Nine months ago, a laparoscopic cholecystectomy was complicated by the transection of the common bile duct, which was treated with end-to-end anastomosis and T-tube drainage. After readmission, the patient underwent an exploratory laparotomy. DIA

Multifocal Biliary Adenoma Involving the Gallbladder and Common Bile Duct Presenting With Obstructive Jaundice: A Case Report.

Biliary adenomas are rare neoplastic processes of the gallbladder and extrahepatic biliary tree that are pre-malignant. These tumors infrequently present with obstructive jaundice, which is more suggestive of underlying invasive malignancy. Here, we present a case of multifocal biliary adenoma without invasive cancer in a 70-year-old man who presented with jaundice. Initial laboratory results and imaging suggested a malignant process, prompting surgical resection. Intraoperative findings and histopathologic examination revealed a multifocal tubulovillous adenoma involving the gallbladder and d

Acute pancreatitis in patient with malignant obstructive jaundice following percutaneous trans-hepatic biliary drainage: A case report.

BACKGROUND: Percutaneous Trans hepatic biliary drainage (PTBD) is a successful palliative treatment for malignant obstructive jaundice. Despite the past that PTBD complications occur more frequently. Currently, with the help of an ultrasound guide, these side effects and possibility of adjacent organ damages have decreased interestingly. One of the rare complications of this procedure is acute pancreatitis. CASE PRESENTATION: We discuss an uncommon case of acute pancreatitis following PTBD in an elderly woman who presented to the hospital with jaundice and anorexia. Conservative therapy witho

Effect and mechanism of omega-3 polyunsaturated fatty acids on intestinal injury in rats with obstructive jaundice.

OBJECTIVE: Obstructive jaundice (OJ) is a common clinical pathological syndrome in hepatobiliary surgery. High incidence of multiple organ injuries during perioperative period and its associated mortality remains challenging in clinical practice. Omega-3 polyunsaturated fatty acids (ω-3 PUFA) is an important enteral immune nutrition. This study investigated the protective role of ω-3 PUFA in the regulation of inflammatory response in OJ. MATERIALS AND METHODS: Seventy-two rats were randomly divided into obstructive jaundice (OJ) group, obstructive jaundice + ω-3 PUFA group (OJPUFA) group, and

Impressive Response to Capecitabine in a Patient with Obstructive Jaundice due to Metastatic Triple-Negative Breast Cancer.

BACKGROUND: Metastatic breast cancer with obstructive jaundice due to para-aortic lymph node enlargement is an unusual case that poses a therapeutic challenge in determining a chemotherapy regimen. CASE REPORT: A 61-year-old woman presented with triple-negative left invasive ductal breast carcinoma with liver and pulmonary metastases. After receiving gemcitabine and carboplatin as the 4th-line treatment, chemotherapy was postponed due to an increased bilirubin level. Abdominal imaging revealed para-aortic lymph node metastases compressing the distal common hepatic duct. The patient then recei

Metastatic mucinous adenocarcinoma of the distal common bile duct, from transverse colon cancer presenting as obstructive jaundice.

The patient was a 70-year-old male whose chief complaints were obstructive jaundice and weight loss. Abdominal imaging studies showed a 2.5 cm sized mass at the distal common bile duct, which was suggestive of bile duct cancer. Eccentric enhancing wall thickening in the transverse colon was also shown, suggesting concomitant colon cancer. A colonoscopy revealed a lumen-encircling ulcerofungating mass in the transverse colon, that was pathologically proven to be adenocarcinoma. The bile duct pathology was also adenocarcinoma. Pylorus-preserving pancreaticoduodenectomy and extended right hemicol

Benign inflammatory pseudotumour mimicking extrahepatic bile duct cholangiocarcinoma in an adult man presenting with painless obstructive jaundice.

Inflammatory pseudotumours (IPTs) of the biliary tract are extremely rare and heterogeneous by aetiology and clinical presentation. They might cause biliary obstruction and mimic cholangiocarcinomas and their final diagnosis is usually achieved only after surgical excision. The most characteristic feature of IPT is the presence of chronic inflammatory cell infiltrates with variable degree of proliferating fibrous tissue. IPTs have the potential for recurrence even after resection and if untreated they can grow causing a variety of symptoms due to compression of the surrounding structures and o

High pallidal T1 signal is rarely observed in obstructive jaundice, but is frequently observed in liver cirrhosis.

Although high signal intensities in the globus pallidus are frequently observed in T1-weighted magnetic resonance images (MRI) of patients with liver cirrhosis, it is unclear whether these increases are due to portal-systemic shunt or obstruction of biliary excretion. We therefore studied pallidal signals in 18 cancer patients with bile duct obstruction and marked jaundice (>10 mg/dl). Patients who had fever, leukocytosis or liver cirrhosis were excluded to ensure that jaundice was due to bile duct obstruction. All patients showed a dilated intrahepatic duct on computed tomography (CT) scan. A

Renal endothelin system in obstructive jaundice: its role in impaired renal function of bile-duct ligated rats.

1. Obstructive jaundice predisposes the kidney to acute renal failure. Endothelin (ET), a potent renal vasoconstrictor and modulator of the tubular action of arginine vasopressin, has been suggested to play a pathogenetic role in acute renal failure. In the present study we therefore investigated renal function and the renal ET system in rats on day 4 after bile-duct ligation (BDL) or sham-operation (SO), without (n = 7 in each group) and with treatment with bosentan, a combined ETA/ETB receptor blocker, (n = 5 in each group). 2. On day 4 after BDL, serum bilirubin had increased to 226 +/- 10

Bone marrow involvement and obstructive jaundice in Farber lipogranulomatosis: clinical and autopsy report of a new case.

We report a case of Farber lipogranulomatosis in a girl with hepatosplenomegaly, macular cherry-red spot, and subcutaneous nodules who developed liver dysfunction with jaundice and ascites, and myelophthisic anaemia because of infiltration of bone marrow with storage cells. Acid ceramidase assay confirmed the diagnosis. We conclude that the bone marrow dysfunction and cherry-red spot are features of type IV Farber lipogranulomatosis that have not been previously recognized, and should be added to the clinical phenotypic description.
