High failure rate 10.8 years after vastus medialis transfer and lateral release (Green's quadricepsplasty) for recurrent dislocation of the patella.

PURPOSE: In adolescent patients with recurrent patellar dislocation, the Green's quadricepsplasty stabilizes the patella in a combination of a lateral release, a transfer of the medial head of the quadriceps onto the lateral part of the patella and an imbrication of the medial patellar retinaculum and joint capsule. This study aimed to evaluate the long-term performance, considering re-dislocations and functional outcomes. We hypothesized a high failure rate in the long term. METHODS: In this single surgeon and single center study 26 knees in 23 patients (mean age 14.2 years; 4-22 years, 18 f

Versatile Bacillus Serine Proteases: Catalysts for Innovation in Industry, Medicine, and Environmental Sustainability.

INTRODUCTION: Patellar dislocation in children and adolescents, particularly habitual and congenital types, is a challenging condition that can cause significant functional impairment and long-term morbidity if untreated. Medial patellofemoral ligament (MPFL) is a critical structure in stabilizing the patella, and its reconstruction has been widely attempted in clinical practice using various tissue autografts. The same has been extensively published for adults; however, not many studies have been published in pediatric patients with habitual patellofemoral instability using the iliotibial ban

Insall proximal realignment for disorders of the patella.

Proximal realignment with standard lateral retinacular release (Insall operation) was performed on 35 knees with a follow-up time of two to 11 years. The results for chondromalacia patellae were satisfactory to excellent results in 20 of 23 knees (87%) at two years and in eight of 15 knees (55%) at five to 11 years. Satisfactory to excellent results in 11 of 12 knees (92%) were noted for recurrent patellar dislocation. This procedure should be reserved for advanced chondromalacia patellae (Grades III and IV) and recurrent patellar dislocation. Anatomic studies suggest that the pain relief was

The results of adductor magnus tenodesis in adolescents with recurrent patellar dislocation.

Recurrent dislocation of the patella is a common orthopaedic problem which occurs in about 44% of cases after first-time dislocation. In most cases of first-time patellar dislocation, the medial patellofemoral ligament (MPFL) becomes damaged. Between 2010 and 2012, 33 children and adolescents (39 knees) with recurrent patellar dislocation were treated with MPFL reconstruction using the adductor magnus tendon. The aim of our study is to assess the effectiveness of this surgical procedure. The outcomes were evaluated functionally (Lysholm knee scale, the Kujala Anterior Knee Pain Scale, and isok

[Experimental hip dysplasia in the rabbit].

Neglected cases of congenital dislocation of the hip (CDH) always present with profoundly interesting X rays and pathoanatomical changes. Because of progressive dysplastic change, it is difficult to determine clinically at what stage a patient can not have reconstructive surgery. We used an animal model to induce hip dysplasia in order to understand the mechanism of dislocation and to demonstrate the dysplastic changes at various ages of dislocation. We used a plastic tube to immobilize the left knee of one week old New Zealand white rabbits in the extension position. Fifty-five rabbits in a n

Patellar Dislocations and Reduction Procedure.

Acute patellar dislocations are a common injury occurring in adolescents involved in sports and dancing activities. This injury usually occurs when the knee is in full extension and sustains a valgus stress on the knee. The medial patellofemoral ligament is the medial restraint that assists in stabilizing the patella from lateral dislocations. The patella usually dislocates laterally and is usually not difficult to reduce after patient evaluation and prereduction radiographs. After postreduction radiographs confirm proper position of the patella postreduction and the absence of fractures, the

Surgical versus conservative management of acute patellar dislocation in children and adolescents: a systematic review.

PURPOSE: The goal of this study was to perform a comparative review to determine whether there is a significant difference in the rate of repeat dislocation and clinical outcome between surgical and conservative management of acute patellar dislocation in children and adolescents. METHODS: A systematic review of the MEDLINE database was performed. English-language clinical outcome studies with a primary outcome/treatment specific to acute patella dislocation in a paediatric population were included. Eleven studies met inclusion criteria; Chi-square analysis, independent t tests and weighted m

Patellar dislocation in a 16-year-old athlete with femoral trochlear dysplasia.

OBJECTIVE: This case report discusses the diagnosis and conservative management of a patient with femoral trochlear dysplasia (FTD) and subacute patellar dislocation. Femoral trochlear dysplasia is characterized by an abnormally shallow trochlear groove. Disengagement of the patella from the shallow femoral trochlea is common in FTD and is a predisposing risk factor to recurrent patellar dislocation and subsequent premature osteoarthrosis. Accurate diagnosis is necessary to prevent recurrent patellar dislocations and damage of the patellofemoral joint cartilage and supporting structures. CLIN

Through-tunnel reconstruction with Ligament Augmentation and Reconstruction System (LARS) for managing post-arthroplasty patellofemoral instability.

BACKGROUND: Despite advancements in prosthetic designs and surgical techniques, patellar dislocation remains a rare but significant complication following total knee arthroplasty, with an incidence ranging between 0.15% and 0.5%. This condition often requires revision surgery to alleviate discomfort and restore joint function. Among the methods to address patellofemoral instability, medial patellofemoral ligament reconstruction has gained attention. In this case, we present the first report of using the synthetic Ligament Augmentation and Reconstruction System (LARS; Surgical Implants and Devi

Late results of Hauser procedure for recurrent dislocation of the patella.

To determine whether the Hauser procedure for recurrent dislocation of the patella prevents repeated dislocation and provides a functional knee without late degenerative arthritis, 52 patients with 67 surgically treated knees were studied clinically and roentgenographically with an average follow-up of 17.1 years. Greater than 70% of patients had an excellent or good result up to as long as 30 years postoperatively. There appeared to be no predisposition to the development of degenerative arthritis. Recurrent dislocation persisted in 7% of cases and female patients with generalized ligamentous

Clinical features and treatment of joint dislocations in Larsen's syndrome. Report of three cases in one family.

Of 3 cases of Larsen's syndrome in one family, one had bilateral dislocation of the hip and the knee joints. The knee joint was not reduced by manipulation and corrective cast, while both hip and knee joints were simultaneously reduced by skeletal traction of the tibia at the age of 4 months, and the course was satisfactory. Reduction of joint dislocations should be attempted by a conservative method such as skeletal traction as early as possible. Various radiographic skeletal abnormalities occurred in all 3 cases. Larsen's syndrome very likely has a genetic origin.

Implantation of an attachment tube preserves knee extension after nonunion of Felix IV fracture: a case report.

BACKGROUND: While commonly utilized to fix tissue and muscles to megaprostheses to restore function and stability after tumor surgery, an attachment tube was used as a synthetic reconstruction of the knee joint's extension mechanism after nonunion of Felix IV C fracture. Fixation of the tibial fragment, and therefore its osteointegration, is complicated after total knee arthroplasty, causing tibial tubercle dislocation. CASE PRESENTATION: A 61-year-old German patient presented to our clinic with Felix IV C fracture, persistent knee pain, and reduced knee extension strength. In this special ca

Outcomes after patellar realignment surgery for recurrent patellar instability dislocations: a minimum 3-year follow-up study of children and adolescents.

BACKGROUND: The purpose of this study was to report early outcomes after patellar realignment surgery (PRS) was performed for patellar instability in children and adolescents. METHODS: The study cohort consisted of 23 patients (27 knees) who had PRS between March 1, 2000 and July 30, 2004, by a single surgeon, with a minimum 3-year follow-up and validated outcome measures. Preoperative diagnoses were dislocations in 22 knees and subluxations in 5 knees. The mean age was 14 years and 1 month. RESULTS: Postoperatively, 2 knees had possible recurrent dislocation/subluxation episodes; neither re

[Ligament lesions of the knee joint in childhood].

As a matter of principle, all lesions of the knee associated with haemarthrosis must be examined for internal injuries. Ligament ruptures are rare; usually, there are bony ligament tears that show up on x-ray film. Examination of ligament stability in children is useful only if the x-ray is negative and after the knee joint has been punctured under anaesthesia, and can be justified only in that case. Diagnosis is possible only by means of x-ray films taken in forced extreme joint position in two planes, with lateral comparison. Treatment of all non-dislocated ligament tears is conservative; sl

Patellofemoral relationships in recurrent patellar dislocation.

Magnetic resonance imaging was used to analyse the patellofemoral relationships during the first 30 degrees of knee flexion in women with recurrent patellar dislocation. The patellofemoral joints were imaged both sagittally and axially with the knee flexed 0 degrees, 10 degrees, 20 degrees, and 30 degrees. At the beginning of knee flexion the sulcus angle was greater than in unaffected women, the lateral patellofemoral angle was smaller, the patella displaced further laterally, tilted more laterally and the congruence angle was directed more laterally. At 30 degrees of knee flexion these diffe
