Muscle deficits in 58 patients with chronic anterior cruciate ligament (ACL) insufficiency were evaluated after completion of a 6 month rehabilitation program. Quadriceps and hamstring torques were measured on a modified Cybex II isokinetic dynamometer. Twenty-nine of our patients were tested just prior to undergoing ACL reconstruction, and patients who were continuing to tolerate their conditions served as controls. For the surgical group, significant quadriceps deficits (P less than 0.01) were found for all speeds and at both the 30 and 60 degrees positions. The nonsurgical group had signifi

BACKGROUND: A "floating fibula" is a rare and severe injury pattern characterized by complete disruption of both proximal and distal tibiofibular attachments. Its association with a multiligamentous knee injury (MLKI) has not been previously documented.
METHODS AND RESULTS: We report the case of a 27-year-old male who sustained high-energy trauma in a motorcycle accident, resulting in a complex MLKI with anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), medial collateral ligament (MCL), and posterolateral corner (PLC) injuries, associated with complete disruption of the pro

BACKGROUND: Knee ligament injuries associated with pelvic and acetabular fractures are frequently overlooked during high-energy trauma, contributing to delayed diagnosis and long-term disability. Evidence describing their incidence and associated fracture patterns remains limited.
OBJECTIVE: To determine the incidence of knee ligament injury and identify the fracture patterns associated with it in patients with pelvic or acetabular fractures.
MATERIALS AND METHODS: We retrospectively reviewed 328 patients with pelvic or acetabular fractures (after excluding 44 of 372 eligible patients lost t

BACKGROUND: Multi-ligament knee injuries (MLKI) are complex injury patterns that affect two or more ligaments of the knee. Although surgical intervention can improve functional and clinical outcomes, a large percentage of patients are unable to return to preoperative activity levels. One reason for this is loss of knee range of motion (ROM), which characterizes the exaggerated pro-inflammatory environment of arthrofibrosis and is associated with future osteoarthritis risk. The purpose of this study is to assess the effectiveness of a 30-day course of losartan, a common angiotensin II antagonis…

Purpose: Popliteal artery (PA) injury during knee surgery poses significant challenges for orthopedic surgeons. This study aims to determine the precise distance between the PA and anatomical landmarks of the knee joint and identify influencing factors using knee magnetic resonance imaging (MRI), in order to establish targeted preventive measures for knee surgery based on the actual situation of the patient. Methods: We conducted a retrospective analysis of knee MRI scans from 172 patients. Patients were categorized into two groups: the osteoarthritis group (Group A) and the non-osteoarthritis

BACKGROUND: Acute kidney injury (AKI) is associated with increased complications after total hip arthroplasty (THA) and total knee arthroplasty (TKA). The purpose of this study was to determine the risk factors for AKI after THA and TKA and evaluate if preoperative use of antihypertensive drugs is a risk factor for AKI.
METHODS: A retrospective review of 7406 primary TKAs and THAs (4532 hips and 2874 knees) from 2013 to 2019 was performed. The following preoperative variables were obtained from medical records: medications, chemistry 7 panel, Elixhauser comorbidities, and demographic factors.

The popliteal arteries are rarely injured during total knee replacement. Although uncommon, these complications warrant attention because the consequences can be disastrous, with high mortality rates, amputation risk, and additional comorbidities such as foot drop, infection, and functional impairment. However, it has detrimental consequences for the patient. Avoiding such injuries starts with preoperative planning, which helps prevent injuries. Furthermore, preoperative planning prompts early recognition and facilitates immediate interventions to reduce complications. We presented the case of

BACKGROUND: The pendulum test is a quantitative method used to assess knee extensor spasticity in humans with spinal cord injury (SCI). Yet, the clinical implementation of this method remains limited. The goal of our study was to develop an objective and portable system to assess knee extensor spasticity during the pendulum test using inertial measurement units (IMU).
METHODS: Spasticity was quantified by measuring the first swing angle (FSA) using a 3-dimensional optical tracking system (with external markers over the iliotibial band, lateral knee epicondyle, and lateral malleolus) and two w

A combined injury of the patellar tendon and both the anterior and posterior cruciate ligaments is disabling. It directly affects knee kinematics and biomechanics, presenting a considerable surgical challenge. In this complex and uncommon injury, decision-making should take into account the surgeon's experience and consider one- or two-stage surgery, tendon graft, graft fixation, and rehabilitation protocol. This manuscript discusses the surgical approach based on a comprehensive understanding of the patellar tendon and bicruciate biomechanics to guide which structures should be reconstructed

Traumatic knee dislocations are serious and complex injuries, defined as clinical and/or radiological loss of tibiofemoral congruence, which may represent real emergencies to the integrity of the affected limb. This lesion is responsible for multi-ligament tears but the most serious possible complications are related to vascular and peripheral nerve injuries. Recent studies show that surgical treatment has better functional results and higher return rates to sports practice compared to conservative treatment. However, there is still no consensus on the ideal surgical technique and the timing o

Post-traumatic osteoarthritis (PTOA) of the knee is often attributed to anterior cruciate ligament (ACL) and meniscus injury. The development of PTOA, however, does not seem to depend on whether or not the damaged ACL is reconstructed. There has been a need to develop animal models to study the mechanisms of PTOA following reconstruction of a traumatized knee. Eighteen rabbits underwent closed-joint trauma to produce ACL rupture and meniscus damage. Then, for the first time, the traumatized knee was surgically repaired in this animal model. Upon euthanasia at 1-, 3- or 6-month post-trauma, joi

Common peroneal nerve (CPN) injury is a recognised complication of traumatic knee dislocation with a direct association between the degree of ligamentous injury and the degree of CPN injury. It is essential explore and repair these injuries in good time to reduce morbidity. Often exploration only involves the portion of this nerve associated with the joint as it courses around the fibular head. However, a recent case highlighted the importance of proximal exploration to its branching point from the sciatic nerve, a known point of fragility, even if other defects have been identified.

Understanding the anatomy and biomechanics of the medial collateral ligament (MCL) is crucial in producing good outcomes after total knee arthroplasty. A solid grasp of the surgical techniques that address the MCL are necessary to ensure good coronal plane ligament balance. Furthermore, intraoperative injury to the MCL in total knee arthroplasty is an uncommon yet serious complication which often goes unrecognized. Loss of the integrity of the MCL can lead to instability, loosening, and accelerated polyethylene wear. There is still controversy regarding the ideal method of treatment of intraop

This article reports a case of the blunt open injury to the lower extremities resulting from the overextension of the knee joint that, despite it severity, turned out to be a mere accompanying event of a more serious condition. This case is of great interest in the context of forensic medical expertise by virtue of the apparent discrepancy between the extent of the injury and the circumstances of the accident. Clearly, such discrepancy may influence the reliability of the expert conclusion as regards the evaluation of such injury.

BACKGROUND: Popliteal arterial injury (PAI) is the second most common infrainguinal arterial injury after femoral artery injury with an incidence < 0.2%. A 2003 analysis of the National Trauma Data Bank (NTDB) reported a below the knee amputation (BKA) rate of 7.1% in patients with PAI as well as higher risk in those with an associated fracture or nerve injury. Given advances in vascular surgical techniques, improved multidisciplinary care, and expeditious diagnosis with computed tomography angiography, we hypothesized that the national rate of BKA in patients with PAI has decreased and sought

PURPOSE OF REVIEW: The purpose of this manuscript is twofold: (1) to review the literature related to rehabilitation after surgery for multiple ligament knee injury (MLKI) and after isolated surgery for the posterior cruciate ligament (PCL), posterolateral corner (PLC), and medial side of the knee and (2) to present a hierarchy of anatomic structures needing the most protection to guide rehabilitation.
RECENT FINDINGS: MLKIs continue to be a rare but devastating injury. Recent evidence indicates that clinicians may be providing too much protection from early weight bearing and range of motion

The objective of the study is to show possibilities of several combinations of suture techniques in a rare bicompartmental bucket handle injury. According to specific injury characteristics, combined suturing techniques were used. The option for different meniscal suture techniques in the two knee compartments allowed the patient, after completing the treatment, to return to his activities with a preserved meniscus. Although meniscectomy continues to be a chosen technique in bucket handle injury, we attempted to show a case of bicompartmental meniscal suture with different techniques. In this,

CONTEXT: Posttraumatic osteoarthritis (PTOA) is a specific phenotype of osteoarthritis (OA) that commonly develops after acute knee injury, such as anterior cruciate ligament (ACL) or meniscal injury (or both). Athletic trainers (ATs) are well positioned to educate patients and begin PTOA management during rehabilitation of the acute injury, yet it remains unknown if ATs currently prioritize long-term outcomes in patients with knee injury.
OBJECTIVE: To investigate ATs' knowledge and perceptions of OA and its treatment after ACL injury, ACL reconstruction, or meniscal injury or surgery.
