Distress Tolerance and Pathological Worry: Tests of Incremental and Prospective Relationships.

Pathological worry and generalized anxiety disorder (GAD) have been linked with low distress tolerance (DT), although questions remain including whether this association exists independent of depression and comorbidity, the directionality of the relationship between worry and DT, and DT's nonredundancy with other worry-relevant variables (i.e., emotional reactivity, stressful life events). Further, it is unclear whether DT is merely a correlate of excessive worry or acts as a risk factor for its development. Two independent studies were completed to evaluate these questions. In Study 1, DT was

Does phasic trauma treatment make patients with dissociative identity disorder treatment more dissociative?

Proponents of the iatrogenic model of the etiology of dissociative identity disorder (DID) have expressed concern that treatment focused on direct engagement and interaction with dissociated self-states harms DID patients. However, empirical data have shown that this type of DID treatment is beneficial. Analyzing data from the prospective Treatment of Patients With Dissociative Disorders (TOP DD) Study, we test empirically whether DID treatment is associated with clinically adverse manifestations of dissociated self-states: acting so differently that one feels like different people, hearing vo

The effects of experimentally induced rumination, positive reappraisal, acceptance, and distancing when thinking about a stressful event on affect states in adolescents.

The current study compares the effects of experimentally induced rumination, positive reappraisal, distancing, and acceptance on affect states in adolescents aged 13-18. Participants (N = 160) were instructed to think about a recent stressful event. Next, they received specific instructions on how to think about that event in each condition. Manipulation checks revealed that the manipulations were successful, except for acceptance. The two most reported events were "a fight" and "death of loved one". Results showed that positive reappraisal (i.e., thinking about the benefits and personal growt

The role of life events and HPA axis in anxiety disorders: a review.

Stressful life events and dysfunctional Hypothalamic Pituitary Adrenal (HPA) axis have been implicated in the pathogenesis of psychiatric disorders, including anxiety disorders. This paper attempts to review the existing literature on childhood traumata, recent life events, HPA axis functioning and their relationship in Post-Traumatic Stress Disorder, Panic Disorder, Generalized Anxiety Disorder, Obsessive Compulsive Disorder and Social Phobia. Preclinical and clinical models will be analyzed. Stressful life events seem to have a role in the onset and in the course of these disorders and HPA a

Annual Research Review: Positive adjustment to adversity--trajectories of minimal-impact resilience and emergent resilience.

BACKGROUND: Research on resilience in the aftermath of potentially traumatic life events (PTE) is still evolving. For decades, researchers have documented resilience in children exposed to corrosive early environments, such as poverty or chronic maltreatment. Relatively more recently, the study of resilience has migrated to the investigation of isolated PTE in adults. METHODS: In this article, we first consider some of the key differences in the conceptualization of resilience following chronic adversity versus resilience following single-incident traumas, and then describe some of the misund

Trauma exposure and mental health characteristics of incarcerated females self-referred to specialty PTSD treatment.

OBJECTIVES: This study examined the traumatic and stressful event exposure history and psychiatric health characteristics of soon-to-be released female inmates who self-referred to specialty posttraumatic stress disorder (PTSD) treatment. METHODS: The final sample (N=209) included female prison inmates aged 18 or older who were assessed for trauma exposure and axis I disorders with standardized clinical instruments. RESULTS: The rate of serious mental disorders was high. Presence of psychiatric disorders was significantly associated with a history of traumatic event exposure and full or subt

Psychopathology and psychogenic movement disorders.

Psychogenic movement disorder is defined as abnormal movements unrelated to a medical cause and presumed related to underlying psychological factors. Although psychological factors are of both clinical and pathophysiological relevance, very few studies to date have systematically assessed their role in psychogenic movement disorder. We sought to assess the role of previous life stress using validated quantitative measures in patients with psychogenic movement disorder compared with age- and sex-matched healthy volunteers as well as a convenience sample of patients with focal hand dystonia. Using a trauma-informed framework to care for incarcerated women.

Available health and social services in women's correctional facilities often do not consider the interrelationship of gender, trauma, and mental illness. As a result, preexisting health issues are often exacerbated, leaving women to reenter their communities with more complex health needs. We propose that a trauma-informed framework can be used to guide clinical interactions with female inmates. A case study is presented as an example of proposed best clinical practice. Future trauma-related practice implications are provided.

Life stress due to losses and deficit in childhood and adolescence as breast cancer risk factor: a prospective case-control study in Kuopio, Finland.

BACKGROUND: To our knowledge, the associations between the life stress due to losses and deficit at childhood and adolescence and the risk of breast cancer are rarely considered together in a prospective study. MATERIALS AND METHODS: This study is an extension of the Kuopio Breast Cancer Study. Women with breast symptoms were referred by physicians to the Kuopio University Hospital (Finland) and were asked to participate in this study. These women (n=115) were interviewed, and all study variables were obtained before any diagnostic procedures were carried out, so neither the investigator nor

Childhood adversities are associated with shorter telomere length at adult age both in individuals with an anxiety disorder and controls.

Accelerated leukocyte telomere shortening has been previously associated to self-perceived stress and psychiatric disorders, including schizophrenia and mood disorders. We set out to investigate whether telomere length is affected in patients with anxiety disorders in which stress is a known risk factor. We also studied the effects of childhood and recent psychological distress on telomere length. We utilized samples from the nationally representative population-based Health 2000 Survey that was carried out between 2000-2001 in Finland to assess major public health problems and their determina

Intersecting losses: working with the inevitable vicissitudes in therapist and client lives.

Therapists working with clients with severe trauma histories over long periods of time may find themselves managing traumatic events in their own lives-illness, divorce, death-at the same time as they are assisting their clients to live productively with their painful pasts. While a few accounts exist describing how therapists deal with single overwhelming events, very little has been written about how therapists manage ongoing or prolonged episodes of severe stressors with a busy clinical practice of clients with severe trauma histories. Yet, we know that support for therapists is crucial to

Reactive and proactive aggression in childhood and adolescence: precursors, outcomes, processes, experiences, and measurement.

This paper reviews and critiques the growing literature on the distinction between reactive and proactive aggression in children and adolescents. Empirical findings suggest that the subtypes of aggression are (a) preceded by different familial precursors, (b) associated with different behavioral outcomes, (c) driven by different social-cognitive and emotional processes, and (d) related to different social experiences. Because measurement difficulties have been a prominent concern in the study of reactive and proactive aggression, a discussion of various assessment approaches is included. Sugge

Affective instability in borderline personality disorder: experience sampling findings.

Affective instability, defined as repeated, rapid, and abrupt shifts in mood, is considered the core pathology in borderline personality disorder. The temporal pattern of affective instability can be best captured with the experience sampling method-longitudinal assessment of people's affective states as they occur in real time and in their natural environment. A review of the experience sampling studies published to date for borderline personality disorder suggests the following mood variability pattern: intense negative mood, more frequent and abrupt mood changes than healthy controls and pa

Consequences of family disruption on children's educational outcomes in Norway.

Using high-quality data from Norwegian population registers, we examine the relationship between family disruption and children's educational outcomes. We distinguish between disruptions caused by parental divorce and paternal death and, using a simultaneous equation model, pay particular attention to selection bias in the effect of divorce. We also allow for the possibility that disruption may have different effects at different stages of a child's educational career. Our results suggest that selection on time-invariant maternal characteristics is important and works to overstate the effects

Ruminative self-focus, negative life events, and negative affect.

Ruminative thinking is believed to exacerbate the psychological distress that follows stressful life events. An experience-sampling study was conducted in which participants recorded negative life events, ruminative self-focus, and negative affect eight times daily over one week. Occasions when participants reported a negative event were marked by higher levels of negative affect. Additionally, negative events were prospectively associated with higher levels of negative affect at the next sampling occasion, and this relationship was partially mediated by momentary ruminative self-focus. Depres

Life events as predictors of mania and depression in bipolar I disorder.

To date, few prospective studies of life events and bipolar disorder are available, and even fewer have separately examined the role of life events in depression and mania. The goal of this study was to prospectively examine the role of negative and goal-attainment life events as predictors of the course of bipolar disorder. One hundred twenty-five individuals with bipolar I disorder were interviewed monthly for an average of 27 months. Negative and goal-attainment life events were assessed with the Life Events and Difficulties Schedule. Changes in symptoms were evaluated using the Modified Ha

Role of stressful life events in induction or exacerbation of psoriasis and chronic urticaria.

BACKGROUND: The possibility of a causal influence of emotional stress, especially of stressful life events, on the course of various skin diseases has long been postulated. Previous reports addressing its influence on skin psoriasis and chronic urticaria have been mainly anecdotal. OBJECTIVE: The aim of this study was to evaluate the stressful events of life within 1 year preceding onset or exacerbation of skin disease in patients of psoriasis vulgaris and chronic urticaria. METHOD: Fifty consecutive clinically diagnosed psoriasis patients and 50 consecutive clinically diagnosed chronic urti

Mexican American women aging with childhood-onset paralytic polio.

In this study the life histories of 11 Latinas of Mexican American descent aging with permanent impairment related to childhood-onset paralytic polio were explored. These women, age 45 to 62 years, were interviewed 3 times each. Field notes, audiotaped interviews, life course charts, and demographic data were used to collect data chronicling childhood to present day. In the results we present a thematic representation of the societal and cultural influences on the life course trajectories of these women.

Figuration of the real as an intersubjective process.

In this paper, I make use of the term figuration, which relates to the process of giving shape to unconscious contents in the form of meaningful presentations. My interest is in how traumatic experience succeeds or fails to become psychically figured and thereby susceptible to elaboration and absorption through intersubjective process. I argue that the process of figuration always occurs in an actual or implied intersubjective context and involves, thereby, a central feature of an exchange between subject and Other. These concepts are discussed and applied to a case report of an analytic psych

"Disturbing phenomenology" in the pain and engagement narratives of Cambodian American survivors of the Killing Fields.

In the clinical literature on trauma, the atrocity survivor's attempt to engage others around the experience of chronic, intractable pain is often viewed as an instance of "help-seeking," logotherapeutic "coherence-making," or-more darkly-"patient malingering." In this article, I challenge the utility of these rubrics through a close examination of the pain and engagement narratives of two survivors of the Cambodian Killing Fields. I demonstrate that survivor narratives can obtain a strategic multivocality, oscillating between phenomenological account and political critique, between clinical d
