[Creating the conditions for skeletal traction in mandibular condyle fracture in children and adolescents].

In this article the possibility of using the Clark appliance (Twin-block) for creating conditions for skeletal traction in case of fraction of mandibular condyle in children and teenagers is presented. The proposed method of using the double-jaw orthodontic appliance Twin-block in case of mandibular fractures with condylar displacement promotes release of traumatized condyle and mandible replacement to correct position, promotes optimal conditions for myofunctional balance, fragment consolidation and remodeling of affected temporomandibular joint.

Accuracy and outcome of mandibular fracture reduction without and with an aid of a repositioning forceps.

PURPOSE: It is presumed that adequate reduction of a fracture of the mandible favors bone healing and diminishes the risk of complications. In this retrospective study, we compared the accuracy of fracture alignment and complication rate of mandibular fractures reduced without or with aid of a repositioning forceps. METHODS: Retrospective analysis of consecutive 252 patients with mandibular fractures treated between January 2010 and December 2016. Eligible for this study were patients with isolated mandibular fractures needing open reduction and internal fixation in whom pre- and postoperativ…

Resorbable Implants for Mandibular Fracture Fixation: A Systematic Review and Meta-Analysis.

UNLABELLED: Mandibular fractures in adults commonly require rigid fixation to ensure proper occlusion while minimizing infection risks. Numerous centers have assessed the efficacy of resorbable materials as a potential alternative to metallic plates. The purpose of the current systematic review and meta-analysis is to shed light on overall outcomes for resorbable implants and to compare these results to those for metallic counterparts. METHODS: A systematic review of clinical studies reporting outcomes for resorbable plates for mandible fractures was carried out. The reported outcomes were ha…

Outcomes of treatment for intracapsular fractures of the mandibular condyle: recommendation for a new classification.

We know of no universally accepted classification for intracapsular condylar fractures. We propose here a new classification based on the concept of a "disc-condyle" unit, and validate the classification based on outcomes of treatment. From 1 January 2010 - 31 December 2014, 55 patients with unilateral intracapsular condylar fractures were classified into three types: type A has no reduction in mandibular height or displacement of the disc (n=7); type B has displacement of the disc with no reduction in mandibular height (n=17); and type C has reduced mandibular height with or without displacem

Contemporary Management of Mandibular Fracture Nonunion-A Retrospective Review and Treatment Algorithm.

PURPOSE: Nonunion is an uncommon complication after mandibular fractures. The purpose of this investigation was to compare outcomes of patients with mandibular fracture nonunion who were treated with a 1- versus 2-stage approach and propose a pragmatic treatment algorithm for surgical management based on preoperative characteristics. MATERIALS AND METHODS: The authors conducted a retrospective study consisting of patients who presented to 2 level 1 trauma centers for the management of mandibular fracture nonunion over a 10-year period. The primary predictor variable was 1- versus 2-stage trea…

Deep head and neck infection causing pathological fracture of the mandibular condyle.

We report an unusual case of a 46-year-old male with a severe deep head and neck infection after extraction of two mandibular molars that led to a potentially life-threatening condition and caused pathological fracture of the mandibular condyle. This is the first published spontaneous pathological fracture of the mandibular condyle caused by an infection spread from a lower molar tooth. Based on CT scan we discuss the pathways of infection of odontogenic origin and the reflections of treatment. This case report illustrates an unusual presentation of a spontaneous pathological condylar fracture

Orthodontic treatment of a patient with severe crowding and unilateral fracture of the mandibular condyle.

A 15-year-old girl who had a unilateral condylar fracture with severe crowding in both arches was treated with 4 premolar extractions followed by orthodontic therapy with a temporary skeletal anchorage device in the maxillary arch. The total active treatment time was 21 months. Her occlusion was significantly improved by orthodontic treatment, and the range of condylar movement was also improved. Posttreatment records after 30 months showed excellent results with a good stable occlusion. The remodeling process of the condyle was confirmed with cone-beam computed tomography images.

One miniplate compared with two in the fixation of isolated fractures of the mandibular angle.

The purpose of this study was to compare one miniplate with two in the management of isolated fractures of the mandibular angle as regards wound healing, failure of hardware, scarring, weakness of the facial nerve, and overall morbidity, by making a systematic review with a meta-analysis. I made a comprehensive electronic search with no date or language restrictions in October 2014. The inclusion criteria were studies in humans, including randomised or quasirandomised controlled trials (RCT), controlled clinical trials (CCT), and retrospective studies that compared the morbidity after treatmen

Mistaken Endobronchial Placement of a Nasogastric Tube During Mandibular Fracture Surgery.

A 64-year-old male had an awake right nasal fiber-optic intubation with an endotracheal tube for open reduction and internal fixation of bilateral displaced mandibular fractures. After induction of anesthesia, an 18 Fr nasogastric tube (NGT) was inserted through the left nostril and was secured. The patient required high flow rates to deliver adequate tidal volumes with the ventilator. A chest x-ray done in the postanesthesia care unit revealed a malpositioned NGT in the left lower lobe bronchus, which was immediately removed. The patient was extubated on postoperative day 2. Various tradition…

Parotid fistula from transparotid approach for mandibular subcondylar fracture reduction.

Injury and/or damage to the parotid salivary glandular element during mandible condylar fracture approach is a rare event that leads to accumulation of saliva in the gland, leading to formation of a sialocele. Huge sialocele often seeks drain through the most dependent area through an extraoral wound. A case of parotid fistula (PF) caused by damage to glandular elements during a transparotid approach for a subcondylar fracture reduction is reported. The mechanism of damage and all possible sequels for the condition is being discussed. The method of surgical intervention for this condition is a

Effect of ageing on healing of bilateral mandibular condyle fractures in a rat model.

To examine the hypothesis that conservative treatment is applicable to younger patients with bilateral mandibular condylar fractures, we studied the effect of ageing on the healing of bilateral mandibular condylar fractures in a rat model. Male Sprague-Dawley rats aged 3, 6, and 36 weeks (n=25/cohort, total n=75) were divided into a fracture group (n=12) and a sham control group (n=12); one rat from each cohort was used as a normal unoperated control. Cell proliferation was evaluated using the bromodeoxyuridine (BrdU) labelling index (LI). Osteochondrogenesis was assessed by the expression of

A mandibular body fracture related to mouth-opening training in a dialysis patient.

A mandibular body fracture related to mouth-opening training in a dialysis patient is reported. A 61-year-old male patient had noticed pain in the right mandibular body and difficulty in mouth opening a week previously. The patient had been performing mouth-opening training for a couple of weeks. The right lower face was slightly swollen with tenderness at the right lower border of the mandible. Hypoesthesia of the right lower lip was also observed. A bone step was palpable on the alveolar ridge of the right mandible, but mobility was not marked. The mandible was atrophic in the body region wi

Unerupted lower third molars and their influence on fractures of the mandibular angle and condyle.

Our aim was to assess the influence of the presence and state of impaction of mandibular third molars on the incidence of fractures of the mandibular angle and condyle. We designed a retrospective study of patients who presented for the treatment of mandibular fractures from January 2006 to April 2011. The independent variables were the presence and degree of impaction of lower third molars, and the outcome variables were the incidence of fractures of the mandibular angle and condyle. The information was acquired from hospital records and panoramic radiographs. Personal data included age, sex,…
