CO2 and O2 concentrations in integral motorcycle helmets.

Inhaling air which contains excess CO2 and/or is oxygen-deficient is known to present health risks and to diminish human cognitive abilities. The average CO2 concentrations relevant to a motorcyclist wearing an integral helmet were measured 20 years ago and found to be alarmingly large. The purpose of the present study was to examine gas concentrations typically inhaled by a motorcyclist. Average concentrations of CO2 near the upper lip for persons (n = 4) wearing integral motorcycle helmets were measured in the laboratory and the field to facilitate comparison to previous work, and similarly

Variability in vehicle and dummy responses in rear-end collisions.

OBJECTIVE: The objective of this study was to quantify the occupant response variability due to differences in vehicle and seat design in low-speed rear-end collisions. METHODS: Occupant response variability was quantified using a BioRID dummy exposed to rear-end collisions in 20 different vehicles. Vehicles were rolled rearward into a rigid barrier at 8 km/h and the dynamic responses of the vehicle and dummy were measured with the head restraint adjusted to the up most position. In vehicles not damaged by this collision, additional tests were conducted with the head restraint down and at dif

Concussion in professional football: location and direction of helmet impacts-Part 2.

OBJECTIVE: National Football League game video was analyzed for the typical locations of severe helmet impacts in professional football. By use of selected cases that were reconstructed in laboratory tests and reported previously, the magnitude and direction of force causing concussion was determined for these locations. METHODS: Multiple video views were obtained for 182 severe helmet impacts that occurred between 1996 and 2001. From a top view, the helmet was divided into 45-degree quadrants with 0 degrees eyes forward. From a side view, it was divided into seven equal levels, four (+Q1 to

Comparison of Miller and Airtraq laryngoscopes for orotracheal intubation by physicians wearing CBRN protective equipment during infant resuscitation: a randomized crossover simulation study.

BACKGROUND: The purpose of this study was to evaluate the performance of orotracheal intubation with the Miller laryngoscope compared with the Airtraq laryngoscope by emergency and pediatric physicians wearing CBRN-PPE type III on infant manikins with conventional airway. We hypothesized that in this situation, the orotracheal intubation with the Airtraq laryngoscope would be faster and more effective than with the Miller laryngoscope. METHODS: This was a prospective, randomized, crossover, single-center study who recruited emergency department physicians on a voluntary basis. Each physician …

Insomnia is a risk factor for spreading of chronic pain: A Swedish longitudinal population study (SwePain).

BACKGROUND: Recent evidence suggests that insomnia negatively influences the occurrence of generalized pain. This study examined whether insomnia is a risk factor for the transition from local pain (LP) to generalized pain (i.e. spreading of pain). METHODS: This longitudinal study, with a follow-up of 24 months, included 959 participants (mean age: 55.8 years; SD: 13.9) with local or regional pain at baseline. Participants were grouped by insomnia symptoms as measured by the Insomnia Severity Index. Spreading of pain was measured by body manikins based on the spatial distribution of pain on t…

A strategy to optimise the performance of the mouth-to-bag resuscitator using small tidal volumes: effects on lung and gastric ventilation in a bench model of an unprotected airway.

When ventilating an unintubated patient with a standard adult self-inflating bag, high peak inspiratory flow rates may result in high peak airway pressures with subsequent stomach inflation. In a previous study we have tested a newly developed mouth-to-bag-resuscitator (max. volume, 1500 ml) that limits peak inspiratory flow, but the possible advantages were masked by excessive tidal volumes. The mouth-to-bag-resuscitator requires blowing up a balloon inside the self-inflating bag that subsequently displaces air, which then flows into the patient's airway. Due to this mechanism, gas flow and p

Comparison of Adult Female and Male PMHS Pelvis and Lumbar Response to Underbody Blast.

The goal of this study was to gather and compare kinematic response and injury data on both female and male whole-body Post-mortem Human Surrogates (PMHS) responses to Underbody Blast (UBB) loading. Midsized males (50th percentile, MM) have historically been most used in biomechanical testing and were the focus of the Warrior Injury Assessment Manikin (WIAMan) program, thus this population subgroup was selected to be the baseline for female comparison. Both small female (5th percentile, SF) and large female (75th percentile, LF) PMHS were included in the test series to attempt to discern wheth

A Novel 3-Dimensional Printing Fabrication Approach for the Production of Pediatric Airway Models.

BACKGROUND: Pediatric airway models currently available for use in education or simulation do not replicate anatomy or tissue responses to procedures. Emphasis on mass production with sturdy but homogeneous materials and low-fidelity casting techniques diminishes these models' abilities to realistically represent the unique characteristics of the pediatric airway, particularly in the infant and younger age ranges. Newer fabrication technologies, including 3-dimensional (3D) printing and castable tissue-like silicones, open new approaches to the simulation of pediatric airways with greater anat

Gelatin Facial Skin Simulator for Cutaneous Reconstruction.

Reconstruction of facial defects can be an intimidating endeavor, especially to resident physicians. When local flap reconstruction is preferred, design of the optimal flap can be a difficult choice. Poor selection can lead to unsightly scarring as well as increased morbidity. A low-cost, easy-to-fabricate gelatin prosthetic facial skin simulator is presented to offer training experience in wound closure, local tissue rearrangement, and facial defect reconstruction for resident instruction. In conjunction with a didactic lecture, 10 participants were asked to perform a Z-plasty, bilobed, rhomb

[Comparison of GlideScope® Cobalt and McGrath® Series 5 video laryngoscopes with direct laryngoscopy in a simulated regurgitation/aspiration scenario].

BACKGROUND: Video laryngoscopy has become increasingly important in airway management in the pre- and intrahospital settings. However, using video laryngoscopes in the presence of body fluids can make visualization of the airway difficult. A simulated regurgitation model was used to compare two video laryngoscopes (VL) with direct laryngoscopy. MATERIALS AND METHODS: A total of 72 physicians participated in this randomized trial. The hypopharynx from an Airway Management Trainer was filled with artificially warmed turbid liquid. In addition, the cervical spine of the manikin was immobilized.

Computational fluid dynamics investigation of human aspiration in low-velocity air: orientation effects on mouth-breathing simulations.

Computational fluid dynamics was used to investigate particle aspiration efficiency in low-moving air typical of occupational settings (0.1-0.4 m s(-1)). Fluid flow surrounding an inhaling humanoid form and particle trajectories traveling into the mouth were simulated for seven discrete orientations relative to the oncoming wind (0°, 15°, 30°, 60°, 90°, 135° and 180°). Three continuous inhalation velocities (1.81, 4.33, and 12.11 m s(-1)), representing the mean inhalation velocity associated with sinusoidal at-rest, moderate, and heavy breathing (7.5, 20.8, and 50.3 l min(-1), respectively) we

Hybrid simulation combining a high fidelity scenario with a pelvic ultrasound task trainer enhances the training and evaluation of endovaginal ultrasound skills.

OBJECTIVES: In this study, an endovaginal ultrasound (US) task trainer was combined with a high-fidelity US mannequin to create a hybrid simulation model. In a scenario depicting a patient with ectopic pregnancy and hemorrhagic shock, this model was compared with a standard high-fidelity simulation during training sessions with emergency medicine (EM) residents. The authors hypothesized that use of the hybrid model would increase both the residents' self-reported educational experience and the faculty's self-reported ability to evaluate the residents' skills. METHODS: A total of 45 EM residen

A new cardiology patient simulator.

We report a new cardiology patient simulator developed through the application of new digital computer technology, capable of playing back selected physical findings (jugular-venous waves, arterial pulses of carotid, brachial, radial and femoral arteries, and cardiac impulses), as well as cardiac sounds, murmurs and respiratory sounds that have been prerecorded from actual patients. The examining physician uses an ordinary type of stethoscope. Four built-in speakers are located in the human-sized mannequin at the classic auscultatory sites (aortic, pulmonic, tricuspid and mitral). This compact

Coupling of a finite element human head model with a lumped parameter Hybrid III dummy model: preliminary results.

A skull-brain finite element model of the human head has been coupled with a multilink rigid body model of the Hybrid III dummy. The experimental coupled model is intended to represent anatomically a 50th percentile human to the extent the dummy and the skull-brain model represent a human. It has been verified by simulating several human cadaver head impact tests as well as dummy head 'impacts" during barrier crashes in an automotive environment. Skull-isostress and brain-isostrain response curves were established based on model calibration of experimental human cadaver tolerance data. The sku
