Background and Clinical Significance: Osteomas of the mastoid process are extremely rare tumors. In their development, they are usually asymptomatic, they can manifest with cosmetic deformity, pain, hearing loss, and weakness of the facial nerve. Case Presentation: We present a clinical case of a 13-year-old girl with complaints of swelling in the area behind the left pinna, which was painless but created a cosmetic defect and an unpleasant sensation. She reported no pain in the ear and no hearing loss. An otorhinolaryngological examination, an audiometry of the patient, and a computed tomogra…

OBJECTIVE: The role of antibiotics in ear surgery is still controversial. The aim of this study was to assess their need in cholesteatoma surgery when performing obliteration with S53P4 bioactive glass, a biocompatible material with antibacterial properties.
METHODS: This retrospective cohort study was conducted in a tertiary referral center between January 2017 and May 2019. Sixty-nine consecutive patients, who underwent surgery for cholesteatoma removal and/or rehabilitation of canal-wall-down mastoidectomy with mastoid and epitympanic obliteration using S53P4 granules were included. Before…

PURPOSE: After radical surgery for chronic cholesteatoma (CWD mastoidectomy), patients have the option to have the posterior wall of their external auditory canal surgically reconstructed with S53P4 bioactive glass. The procedure eliminates some of the restrictions related to having a postoperative cavity and extends the options for a hearing prosthesis. If classic reconstruction is not possible and a hearing aid is not used, we suggest use of a Bonebridge implant.
METHODS: This study describes, over 18 months of follow-up, 16 patients after a two-stage surgical procedure: obliteration of the…

OBJECTIVES: A broad mastoid extension limits cholesteatoma resection via a transmeatal approach including endoscopic ear surgery. Therefore, a preoperative diagnosis of mastoid extension is a the most critical factor to determine whether to perform mastoidectomy. The purpose of this study was to assess the efficacy of non-echoplanar diffusion-weighted imaging (non-EPI DWI) and T1-weighted imaging in the evaluation of mastoid extension in cholesteatomas of the middle ear.
METHODS: Patients who underwent magnetic resonance imaging (MRI) for pretreatment evaluation before primary surgery for par…

Canal wall down (CWD) mastoidectomy is a surgical procedure that is essential to eradicate chronic ear diseases but results into bone defects due to the resulting mastoid cavity. This study investigated the ability of human tonsil-derived mesenchymal stem cells (hTMSCs) combined with a hydroxyapatite (HAp)-chitosan patch to promote osteogenesis in the treatment of postoperative temporal bone defects using an animal model. At 12 weeks postsurgery, the obliteration ratio and bone formation in the tympanic bulla of rats that were grafted with hTMSCs and a hydroxyapatite (HAp)-chitosan patch or no…

BACKGROUND: A mastoid cavity resulting from a canal wall down mastoidectomy can result in major morbidity for patients due to chronic otorrhea and infection, difficulty with hearing aids and vertigo with temperature changes. Mastoid obliteration with reconstruction of the bony external ear canal recreates the normal anatomy to avoid such morbidity. Few have the studied the quality of life benefit that this procedure confers.
METHODS: This retrospective observational study was conducted to determine if mastoid obliteration with autologous cranial bone graft following mastoidectomy improves qua…

Physiologic and developmental role of mastoid pneumatisation in children with otitis media with effusion (OME) is still controversial. For measuring mastoid pneumatisation and examine developmental characteristics, we used children with orofacial malformation of high risk for long term negative pressure in the middle ear and are expected to have lower rate of size and growth of pneumatisation. Mastoid were measured on Schuller's mastoid X-ray pictures planimetrically in study group of 146 children with bilateral (BCLP), unilateral (UCLP) and isolated (ICP) cleft palate, and control group of no…

This study was designed to assess the anatomic characteristics and clinical versatility of superficial mastoid fascia as a soft-tissue donor for augmentation in Asian rhinoplasty. Dissections were performed on four fresh cadavers (eight ears) for histologic study. A 3×10 mm2-sized full layer of skin and underlying soft tissue was harvested from the postauricular area, transversely and longitudinally (cephalic and caudal directions), 5 mm apart from the midpoint of the auriculocephalic sulcus. The average fascial thickness and nerve distribution were assessed using digital microscopic images af…

Traumatic facial nerve neuroma is rare. There are only 10 reported cases in the literature, caused either by physical trauma or chronic inflammation. Traumatic facial neuromata differ from true facial nerve neoplasms in radiological, macroscopic and microscopic appearance, but clinical presentation is less reliable in differentiating the two. Management depends on the pre-operative grade of facial palsy, as this is a benign condition and surgical management carries a risk of further affecting facial nerve function. We present a further case of traumatic facial nerve neuroma following surgery f

OBJECTIVES: Despite the utility of intraoperative facial nerve monitoring in reducing the risk of iatrogenic facial nerve injury during neurotologic surgery, its routine use during primary or revision surgery remains controversial. One of the major barriers to its acceptance is cost. This study evaluates the cost-effectiveness of intraoperative facial nerve monitoring during middle ear or mastoid surgery.
RESEARCH DESIGN/METHODS: A simple decision analytic cost-effectiveness model and a societal approach were used to evaluate three cohorts of individuals who received (1) intraoperative facial…
