The case of a 43-year-old man with a right skull base hypoglossal neurilemmoma excised via the extended posterolateral approach is presented. There is only one previous case of hypoglossal neurilemmoma in the literature. The surgical technique described is a new approach to the posterior skull base involving a suboccipital craniectomy, mastoidectomy and the removal of the lateral process of the atlas. It provides an inferior approach to the jugular foramen and hypoglossal canal that allows the lower cranial nerves to be identified as they exit from their skull base foramina. In the discussion …

OBJECTIVE: Computer-assisted navigation (CAN) is increasingly used in ENT surgery, notably in sinus and otoneurosurgical procedures. This study assessed the accuracy of a CAN system in temporal bone surgery using bone-anchored registration, without preoperative fiducial screws or facial CT imaging, focusing solely on the temporal bone.
MATERIALS AND METHODS: Ten cadaveric temporal bones (5 left, 5 right) underwent high-resolution CT imaging. Dissections (mastoidectomy with facial recess approach) were conducted using the Collin Navigation Solutions system. Surface registration employed the it…

The aim is to analyze Literature concerning mastoid obliteration in adults with either autologous or heterologous grafts in the last 10 years. Data Source: Databases such as NIH PubMed, Bookshelf, NLM Catalog, Cochrane Library, and Embase were consulted. Thirty-seven studies were selected (22 concerning autologous materials, 15 about heterologous ones). Only studies with more than 12 months of follow-up were considered. A statistical analysis with random-effects models was performed to allow the true effect sizes to differ from study to study. The present literature review and meta-analysis do

Klippel-Feil syndrome (KFS) is a rare congenital anomaly that is characterized by short neck, low posterior hairline, and restricted neck movements, all of which lead to the possibility of a difficult airway. These patients often have associated multiple systemic anomalies, which add to the challenges in anesthetic management. We describe the anesthetic management of 3 patients with KFS: 2 who underwent modified radical mastoidectomy and 1 patient who underwent vaginoplasty at our institute. Careful planning and preparedness for difficult airway and thorough knowledge of associated comorbiditi…

We report an extremely rare case of hearing aid silicone impression material as a foreign body in the middle ear. Symptoms of the patient were otorrhea and vertigo after taking of a mold impression on his only hearing ear, and the symptoms mimicked chronic otitis media. A temporal bone CT scan revealed foreign body material in the middle ear and Eustachian tube. An intact canal wall mastoidectomy with a facial recess approach and type IV tympanoplasty was performed to remove the silicone impression material. In addition to the case report, we review the literature regarding impression material…

Aspergillus mastoiditis and skull-base osteomyelitis are extremely rare, even in immunocompromised patients. We report a case of an 81-year-old immunocompetent man, who underwent a mastoidectomy because of unexplained, progressive otalgia in spite of a noninflamed and air-containing middle-ear space. Histopathology yielded Aspergillus fumigatus. When confronted with otitis with an unexpected clinical course a high index of suspicion is required to facilitate early diagnosis and appropriate therapy of a potential lethal Aspergillus infection, even in immunocompetent patients. This seems to be m…

The management of a mastoid subperiosteal abscess has traditionally required mastoidectomy. With the improvement of antibiotic therapy current literature supports the treatment of uncomplicated acute mastoiditis with myringotomy and intravenous antibiotics. Treatment of a mastoid subperiosteal abscess with tympanostomy tube insertion, intravenous antibiotics, and postauricular incision and drainage of the abscess avoids the morbidity and potential complications of mastoid surgery in young children. Three patients diagnosed with a mastoid subperiosteal abscess were managed in this way. The outc…

OBJECTIVE: To determine whether the subperiosteal abscess (SPA) volume in acute coalescent mastoiditis (ACM) as measured in imaging studies, should influence the decision-making process between performing mastoidectomy versus needle aspiration or incision and drainage (I&D) as the initial treatment of SPA.
SETTING: Single tertiary referral center.
PATIENTS AND METHODS: The records of all pediatric patients admitted with ACM between 1/2012 and 12/2023 were retrospectively reviewed. Baseline abscess volumes were measured on dedicated segmentation software for analyzing imaging findings. SPA vo…

Facial nerve schwannomas are rare tumors that pose diagnostic and surgical challenges. We report a 17-year-old female with progressive right-sided facial paresis initially misdiagnosed as Bell's palsy. MRI revealed a contrast-enhancing lesion of the facial nerve. She underwent a two-stage surgery: tumor resection via mastoidectomy and hearing rehabilitation, followed by facial nerve reconstruction using masseteric-to-facial nerve transfer and cross-face sural grafting. At nine months postoperatively, facial function improved from House-Brackmann grade V to III, and hearing was preserved. Early…

A nasopharyngeal mass with cranial neuropathies usually indicates an advanced neoplastic process. We present a case of nasopharyngeal carcinoma diagnosed and treated 38 years ago. A nasopharyngeal mass recurred with cranial neuropathies and concurrent otologic infection in June, 1990. Repeated nasopharyngeal biopsies were negative with respect to tumors. Suspecting direct extension of the otologic infection, modified radical mastoidectomy was performed. The nasopharyngeal mass resolved after the surgical intervention. Spread of otologic infection from the temporal bone to the nasopharynx may b…
