Recognizing Odontogenic Maxillary Sinusitis in the ENT Clinic: Four CT Findings That Should Prompt Dental Evaluation.

Objective: This retrospective cohort study evaluated the proportion of maxillary sinuses with radiographic findings consistent with an odontogenic contribution among patients with imaging-confirmed acute or chronic maxillary sinusitis presenting to a single academic ENT clinic over one year. Sixty-five CT scans (84 sinuses) underwent multidisciplinary review for prespecified radiographic features. By highlighting imaging findings indicative of dental origin this study aims to improve recognition of possible odontogenic disease and appropriate multidisciplinary evaluation. Methods: Sixty-five p…

Detection of maxillary sinusitis of endodontic origin in cone-beam CT images using deep learning algorithms.

Cone-Beam Computed Tomography (CBCT) scans were retrospectively collected and examined to acquire a balanced dataset representative of Normal Maxillary Sinus (NMS), Maxillary Sinusitis of Endodontic origin (MSEO), and Maxillary Sinusitis of Non-Endodontic origin (MS-NEO) according to established criteria. Data were manually labeled, pre-processed and split into training, validation, and testing groups. A custom model workflow started by anatomic classification of input images as being coronal, sagittal, or axial, followed by segmentation of roots, lining, and sinus regions, followed by extract…

Nasal Septal Deviation and Its Association With Maxillary Sinus Volume and Sinusitis: A Computed Tomography Study.

PURPOSE: The objective of this study was to assess the potential impact of nasal septal deviation (NSD) on the volume of the maxillary sinuses and its association with the development of maxillary sinusitis. METHODS: Patients who presented with sinonasal symptoms at our tertiary care center and underwent paranasal sinus CT scans were retrospectively evaluated. The study included individuals over the age of 18 who had no history of maxillofacial surgery or trauma and no facial masses. When evaluating the nasal septum, the convex portion of the septum was considered the direction of deviation. …

[Maxillary and sphenoid sinusitis complicated by acute inflammatory optic neuritis in a 12-year-old patient].

Acute inflammatory optic neuritis (retrobulbar optic neuritis) is an uncommon but described complication of acute sinusitis. It is due to a close anatomic relationship between the sinus and the optic nerve. Inflammatory mechanism is dominant with a possible bilateral neuritis, especially among children. In the reported case, as usual in childhood, visual acuity loss has been particularly severe, and recovery slow (it takes usually 6 weeks) but almost complete. Primary treatment is medical, with antibiotics and corticosteroids that must be prescribed for a prolonged time.

Microbiology of chronic maxillary sinusitis: comparison between specimens obtained by sinus endoscopy and by surgical drainage.

The aerobic and anaerobic microbiology of sinus aspirates obtained during surgery was compared with culture of samples obtained by endoscopy. Six patients with chronic maxillary sinusitis were evaluated. Polymicrobial flora was found in all specimens (two-to-five isolates/sample). A total of 24 isolates (18 anaerobic, five aerobic and one micro-aerophilic) was obtained from sinus aspirates, and 25 isolates (16 anaerobic and nine aerobic) were found in endoscopic specimens. The predominant organisms were Prevotella spp., Fusobacterium nucleatum, Peptostreptococcus spp. and Staphylococcus spp. C…

Quantitative PCR-Based Analysis of Bacterial Profiles in Periapical Lesions and Maxillary Sinus in Odontogenic Sinusitis.

Odontogenic sinusitis (ODS) is a common cause of unilateral maxillary sinusitis arising from periapical lesions (PALs) or other dental sources. The infection is typically polymicrobial and dominated by anaerobic bacteria, which are often under detected by routine culture. Molecular approaches such as quantitative polymerase chain reaction (QPCR) and next-generation sequencing (NGS) may provide improved characterization of the microbial burden and community structure. This study aimed to compare culture-based methods, targeted quantitative PCR, and 16S rRNA sequencing in paired samples to chara…

Microbiological Comparison of Maxillary Sinus Rinses in Non-Odontogenic and Odontogenic Sinusitis of Primarily Endodontic Origin.

Objectives: Odontogenic sinusitis (ODS) is common but frequently overlooked condition that differs from rhinogenic sinusitis (CRS) and should be suspected in each case of unilateral sinusitis. Clinical symptoms such as foul smell, congestion, rhinorrhea, and unilateral maxillary sinus opacification with overt dental pathology on radiological scans are more suggestive of ODS than CRS, but the distinctive microbiological flora are another clinical factor in diagnosis. The aim of this study was to compare the microbiological load of ODS and CRS and their clinical presentation for better disease r

Immunoglobulin- and complement-coated bacteria in effusions obtained during the early course of acute maxillary sinusitis.

Fifty samples of maxillary sinus effusion from 40 patients (median age 29 years; 26 females and 14 males) with acute maxillary sinusitis (AMS) were subjected to quantitative and qualitative bacteriological analysis. Furthermore, bacteria coated with immunoglobulin (IgG, secretory IgA, IgM) or complement (C3b) were evaluated using an immunofluorescence assay. 72% of the samples harboured detectable bacteria, the most common pathogens being Streptococcus pneumoniae and non-typable Haemophilus influenzae. Of the bacteria-positive samples, 28% harboured immunoglobulin-coated bacteria. During the f…
