Bisphosphonate-induced osteonecrosis of the jaws, bone markers, and a hypothesized candidate gene.

PURPOSE: To determine whether any abnormality in serum bone markers is related to bisphosphonate-induced osteonecrosis of the jaw. MATERIALS AND METHODS: We obtained serum bone markers and other relevant endocrine assays on 7 patients with osteonecrosis of the jaws (ONJ). The assays were C-telopeptide, N-telopeptide, bone specific alkaline phosphatase, osteocalcin, intact parathyroid hormone, T3, T4, TSH, and vitamin D 25 hydroxy. Diagnostic criteria for ONJ were those formulated by the American Association of Oral and Maxillofacial Surgeons. RESULTS: Five of our patients were women. Two had

Clinical features, pathophysiological mechanisms, and multidisciplinary management strategies for rhinitis-induced adenoid facies in children and adolescents: a review.

Chronic rhinitis and its associated persistent nasal obstruction and mouth breathing are core factors leading to the development of characteristic "rhinitis face" or "adenoid facies" in children and adolescents. This review elucidates the diverse clinical manifestations of "rhinitis face," including: persistent open-mouth posture; abnormal patterns of facial skeletal growth, such as midface hypoplasia and increased lower anterior facial height resulting in "long face syndrome"; alterations in jaw morphology and position, including maxillary constriction, high-arched palate, and mandibular retr

Does Social Media Affect a Patient's Decision to Undergo Orthognathic Surgery?

Orthognathic surgery, also known as corrective jaw surgery, is a procedure that corrects abnormalities of the jaw and face. It is used to treat malocclusions, where the teeth and jaws are misaligned. This surgery can improve the function and appearance of the jaw and face, leading to improved mastication, speech, and quality of life for the patients. To assess if social media had any effect on the patients' decision to undergo orthognathic surgery, a self-administered online questionnaire was distributed to patients who had undergone orthognathic surgery at the Oral and Maxillofacial departmen

Common tongue conditions in primary care.

Although easily examined, abnormalities of the tongue can present a diagnostic and therapeutic dilemma for physicians. Recognition and diagnosis require a thorough history, including onset and duration, antecedent symptoms, and tobacco and alcohol use. Examination of tongue morphology and a careful assessment for lymphadenopathy are also important. Geographic tongue, fissured tongue, and hairy tongue are the most common tongue problems and do not require treatment. Median rhomboid glossitis is usually associated with a candidal infection and responds to topical antifungals. Atrophic glossitis

First branchial cleft anomalies have relevance in otology and more.

INTRODUCTION: First branchial cleft anomalies account for less than 8% of all branchial abnormalities. Their rarity and diverse presentations have frequently led to misdiagnosis and inappropriate treatment. In a trend towards specialisation/subspecialisation, first branchial cleft duplication anomalies, with their varied clinical manifestations, may possibly present to an Otology, Head and Neck Surgery, Paediatric Otolaryngology, Maxillofacial or even a General Paediatric and General Surgery practice. There is a need to highlight the clinical features which can aid in accurate diagnosis. CLIN

Evaluation of the Therapeutic Effect of Bi-Maxillary Osteotomy Using the Stress Distribution on the Temporomandibular Joint When Doing Anterior Teeth Occlusion.

The purpose of this study was to investigate how sagittal split ramus osteotomy (SSRO) and Le Fort 1 osteotomy affected the stress distribution of the temporomandibular joint (TMJ) during an anterior teeth bite using the three-dimensional (3D) finite element (FE) method. Fourteen orthognathic surgery patients were examined with mandibular prognathism, facial asymmetry, and mandibular retraction. They underwent Le Fort 1 osteotomy in conjunction with SSRO. In addition, ten asymptomatic subjects were recruited as the control group. The 3D models of the mandible, disc, and maxilla were reconstruc

NIR-II-Responsive Hydrogel with Integrated All-in-One Nanotherapeutics for Immunomodulation and Regeneration of Infected Tissues or Bone.

Bacterial infection is now a significant obstacle to the effective repair and regeneration of both soft tissues and bone in clinical settings. Managing infected sites is challenging due to multiple antimicrobial resistance, abnormal inflammatory responses, and an excessive inflammatory microenvironment. To meet the complex healing needs following soft tissue or bone, an immunomodulatory thermosensitive hydrogel was designed and synthesized. The composite hydrogel (CQDs@UPDA@gel) combines carbon quantum dots (CQDs) as the second near-infrared window (NIR-II) photothermal agents (PTAs) and ultra

The chick embryo as a model for the effects of prenatal exposure to alcohol on craniofacial development.

Prenatal exposure to ethanol results in fetal alcohol spectrum disorder (FASD), a syndrome characterised by a broad range of clinical manifestations including craniofacial dysmorphologies and neurological defects. The characterisation of the mechanisms by which ethanol exerts its teratogenic effects is difficult due to the pleiotropic nature of its actions. Different experimental model systems have been employed to investigate the aetiology of FASD. Here, I will review studies using these different model organisms that have helped to elucidate how ethanol causes the craniofacial abnormalities

Oblique facial clefts in Johanson-Blizzard syndrome.

Johanson-Blizzard syndrome (JBS) is considered as an infrequent, but clinically easily recognizable autosomal recessive entity by the pathognomonic combination of congenital exocrine pancreatic insufficiency and hypoplastic alae nasi, in addition to other distinctive findings such as scalp defects, hypothyroidism, and rectourogenital malformations. There are few reports of patients with JBS in association with facial clefting, referring all to types 2 to 6 of Tessier's classification that can be characterized properly as oblique facial clefts (OFCs). We describe the clinical aspects in four pa

Primary Assessment of the Patient With Orbital Fractures Should Include Pupillary Response and Visual Acuity Changes to Detect Occult Major Ocular Injuries.

PURPOSE: Sight-threatening injuries associated with orbital fractures are of major concern to maxillofacial surgeons whom are often the first asked to assess these patients. Eliciting signs and symptoms that are predictive of these injuries would allow expedited ophthalmic consultation and appropriate management. We hypothesized that abnormal pupillary response is predictive of major ocular injuries. PATIENTS AND METHODS: A retrospective cohort study of patients with facial fractures was instituted with review of all associated ophthalmic injuries. The primary predictor variables were the pre

Anteriorly Based Galeo-Pericranial Frontalis Flap: A Novel Application in Secondary Rhinoplasty With Atrophic Skin.

BACKGROUND: Nasomaxillary dysplasia (Binder syndrome) and cleft are congenital malformations of the midface. Ideally, the nasal deformity is treated from childhood. This approach leads to the need for several surgeries and revisions until full growth is reached. As a result of multiples procedures, the skin over the nasal dorsum sometimes becomes thin, atrophic and could be strongly attached to underlying grafts. For this setting of patients when another rhinoplasty is required, it becomes a technical challenge. The authors describe their experience in complex secondary rhinoplasty performed i

The role of sensory organs and the forebrain for the development of the craniofacial shape as revealed by Foxg1-cre-mediated microRNA loss.

Cranial development is critically influenced by the relative growth of distinct elements. Previous studies have shown that the transcription factor Foxg1 is essential the for development of the telencephalon, olfactory epithelium, parts of the eye and the ear. Here we investigate the effects of a Foxg1-cre-mediated conditional deletion of Dicer1 and microRNA (miRNA) depletion on mouse embryos. We report the rapid and complete loss of the telencephalon and cerebellum as well as the severe reduction in the ears and loss of the anterior half of the eyes. These losses result in unexpectedly limite

Rigid External Distraction with Intranasal Bone-borne Traction Hooks for Midfacial Hypoplasia.

Rigid external distraction is currently performed to correct cases of severe maxillary hypoplasia. As an improvement of this technique, we propose the use of an intranasal bone-borne traction hook. This study is a retrospective chart review of the intranasal bone-borne traction hooks used in the treatment of severe maxillary hypoplasia. There were 110 patients treated with the hooks from 2005 to 2017. The maximum traction force was 7.75 kg, and there were few complications encountered during distraction. There were 76 patients who had the hooks removed under local anaesthesia. A cephalometric

Cone beam computed tomography--current understanding and evidence for its orthodontic applications?

Cone beam computed tomography (CBCT) is a fast developing technology which provides relatively low-dose, high spatial resolution imaging of the craniofacial complex in three dimensions (3D). During the past decade, the number of CBCT-related publications in the literature has increased significantly, but the crucial question is whether this technology leads to improved outcomes. In this paper, the literature is reviewed and the main applications of CBCT in orthodontics are highlighted, as well as the current evidence and understanding in each area. It is clear that at the present time, despite

Hard tissue structural changes in TMJ morphology prior to orthodontic therapy: a complex motion tomographic study.

OBJECTIVE: Temporomandibular joint (TMJ) disorders have been investigated for years due to their probable association with the neuromasticatory apparatus and the myoskeletal framework of the head and neck. In this study, we explored hard tissue structural changes within the TMJ (condyle, glenoid fossa, articular eminence, and joint space) using complex motion tomography. A correlation between these morphologic changes and clinical symptoms of disc-related TMJ disorders was attempted. METHOD AND MATERIALS: Submentovertex and complex motion tomograms were taken on 114 TMJs from 57 patients who
