Daughter diverticula arising from the body of Meckel's diverticulum are rare. We detected such an occurrence in a 35-year-old man at appendectomy. These are probably primary and not pulsion or traction diverticula, as they do not have a muscle wall.

Meckel's diverticulum (MD) is the most common congenital anomaly of the small intestine, occurring in up to 4 percent of the population. The majority of MD cases are discovered incidentally; however, they can occasionally cause serious bleeding or obstructive or inflammatory complications. We reviewed the charts of 58 patients with MD from 1984 to 1994 collecting data on age, sex, presentation, therapy, pathology, and surgical complications to try to identify factors suggestive of the need for surgical therapy and the associated morbidity and mortality of resection. The authors present a case of a Meckel's diverticulum identified during a routine prenatal ultrasound examination as a complex mass, superior and posterior to the urinary bladder, that prompted meticulous postnatal evaluation. A 4.0-cm Meckel's diverticulum containing heterotopic gastric mucosa was excised before complications could arise. Meckel's diverticulum should be included in the differential of a complex intraabdominal mass of the fetus/neonate.

A rare case of bleeding Meckel's diverticulum in an adult, in which Tc-99m pertechnetate scan, mesenteric angiography, and barium examination failed to establish the diagnosis, is presented. The diagnosis was made by laparoscopy, and a laparoscopically assisted resection was carried out with a smooth postoperative course, early discharge from the hospital, and rapid return to work.

Although complications of Meckel's diverticula are relatively common, there has not been a reported case, to our knowledge, of foreign bodies impacted within a Meckel's diverticulum. We report herein a case of a 2-year-old child who had two pennies trapped in a Meckel's diverticulum. This case demonstrates that foreign objects in the right lower quadrant that do not progress may be impacted in a Meckel's diverticulum.

Complications related to Meckel's diverticulum are not unusual. However, involvement of the urinary tract is extremely rare. To our knowledge this is the first reported case of ureteral obstruction due to perforation of Meckel's diverticulum.

The initial technetium-99m pertechnetate abdominal scintigraphy revealed equivocal or normal results. However, a second scintigraphy without pentagastrin demonstrated a focal area of persistently increasing radioactivity in the right lower quadrant of the abdomen. Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract. We reviewed our experience with 31 cases of Meckel's diverticulum in children from 1966 to 1987. Twenty-four patients presented with clinical manifestations: ten from bleeding, ten from obstruction, and four from diverticulitis. Seven Meckel's were incidental findings at laparotomy. Fifty percent of the patients with bleeding had abdominal pain and 100% had ectopic gastric mucosa in the Meckel's.
