European Paediatric Surgeons' Association Consensus Statement on the Management of Meconium Ileus.

INTRODUCTION: The management of meconium ileus (MI) has yet to reach a unified consensus among pediatric surgeons, particularly regarding diagnostics and variants, conservative treatment strategies, surgical approaches, and clinical outcomes. This topic was therefore selected for the 2024 Consensus Session of the Annual Congress European Paediatric Surgeons' Association (EUPSA). METHODS: A structured literature review was conducted across Cochrane, Medline, and EMBASE databases up to April 2024. Nine pediatric surgeons appointed by the European Paediatric Surgeons' Association (EUPSA) Consens…

Exosomes in meconium mediate horizontal transmission and immune evasion of reticuloendotheliosis virus.

Reticuloendotheliosis virus (REV) is an immunosuppressive virus that can be vertically transmitted through chicken embryos. Infected chicks continuously shed the virus in their meconium and feces, facilitating horizontal transmission. Even in flocks with positive maternal antibodies, horizontal transmission can still occur. It is hypothesized that REV-associated exosomes present in the meconium of chicks mediate this horizontal transmission and immune evasion. In this study, exosomes were extracted from the meconium of chicks that tested positive for REV after the virus was experimentally intr…

Early presentation of Hirschsprung's disease with initial meconium passage: A diagnostic challenge.

Hirschsprung's disease is a congenital condition characterized by an absence of enteric ganglionic cells. In this paper, we report a case of an 8-day-old male infant presenting with a history of constipation and progressive abdominal distension. Meconium had been passed in the newborn nursery. No stools were passed after 4 days of age, and the last one had required rectal stimulation. We emphasize the diagnostic challenges associated with Hirschsprung's disease in neonates who initially pass meconium. Timely recognition and surgical intervention achieved a favorable outcome.

Sweating It Out: When Meconium Ileus Is Not Cystic Fibrosis-A Case Report.

Meconium ileus (MI) is the result of the accumulation of thick, dry, inspissated meconium that creates a bowel blockage, most commonly in the terminal ileum. These pockets of meconium prevent passage of stool beyond the point of obstruction, which leads to distention of the proximal bowel, bowel wall thickening, and distal microcolon. Occurring most commonly (90%) in conjunction with cystic fibrosis (CF), the occurrence of MI without CF is rare. The literature describes the incidence of MI associated with CF occurring in as many as 24.9% of those who have a pair (homozygous) of the most common…

Meconium obstruction of prematurity in tiny babies - towards developing an international consensus.

Intestinal perforation occurring in extremely low gestational age neonates is a devastating complication, associated with high mortality and morbidity. Multiple phenotypes of bowel perforation in premature infants have been described, with the most common being spontaneous, or isolated, intestinal perforation and perforated necrotizing enterocolitis. The purpose of this article is to summarize literature describing "meconium obstruction of prematurity", increasingly recognized as a distinct clinical phenotype in the smallest and most immature neonates. The goal of this review is to improve int…

Complicated meconium ileus in a male neonate with cystic fibrosis: Case report.

In about 10%-15% of instances, meconium ileus (MI) is the first sign of cystic fibrosis (CF). If a newborn exhibits signs of intestinal obstruction and does not pass meconium within a short period of time after birth, MI is suspected. The cystic fibrosis transmembrane conductance regulator gene (CFTR), which is found on chromosome 7q31, is mutated in CF patients. A premature baby, 5 days old, with clinical signs that were typical of MI. These attempts failed in spite of conservative therapies, such as rectal injection of gastrografin. An ileostomy was made and meconium was manually evacuated a…

Lipid enemas for meconium evacuation in preterm infants - a retrospective cohort study.

BACKGROUND: Enemas are used in preterm infants to promote meconium evacuation, but frequent high-volume enemas might contribute to focal intestinal perforation (FIP). To replace a regime consisting of frequent enemas of varying volume and composition, we implemented a once-daily, low-volume lipid enema (LE) regimen. We investigated its impact on meconium evacuation, enteral nutrition, and gastrointestinal complications in preterm infants. METHODS: We performed a single-center retrospective study comparing cohorts of preterm infants < 28 weeks gestation or < 32 weeks, but with birth weight < 1…

Fetal magnetic resonance imaging contributes to the diagnosis and treatment of meconium peritonitis.

BACKGROUND: Meconium peritonitis (MP) is a rare fetal disease that needs to be urgently identified for surgical intervention. We report a series of 35 patients diagnosed prenatally with MP by magnetic resonance imaging (MRI), illustrate the imaging findings and investigate the predictive value of these findings for postpartum management. METHOD: A consecutive cohort of patients diagnosed with MP who were born at our institution from 2013 to 2018 was enrolled retrospectively. The prenatal ultrasound and MRI findings were analyzed. Fisher's exact probability test was used to evaluate the predic…

Factors Leading To Meconium Aspiration Syndrome in Term- and Post-term Neonates.

Background Meconium aspiration syndrome (MAS) is considered a major cause of respiratory morbidity. It is a common issue encountered in the delivery room and newborn nursery. There is a need to identify the factors that lead to MAS to develop strategies to screen such patients at an early stage to decrease the mortality and morbidity. The objective of this study was to determine the factors leading to MAS in neonates delivered at ≥37 weeks of gestational age. Methods A cross-sectional study was conducted through non-probability consecutive sampling technique at Liaquat University Hospital, Hyd…

Determination of antibiotic concentration in meconium and its association with fetal growth and development.

BACKGROUND: The association between antibiotic use during pregnancy and neonatal birth outcomes has received considerable attention. Most of the previous assessment of antibiotic exposure during pregnancy relied on questionnaires and clinical prescriptions, and very few studies examined pregnancy exposure to antibiotics using human biomonitoring data. OBJECTIVE: To explore the association between the cumulative exposure of antibiotics during the whole pregnancy and neonatal birth measurements using biomonitoring data of antibiotics in meconium. METHODS: Three hundred and sixty nine pregnant …

Patterns of Drugs and Drug Metabolites Observed in Meconium: What Do They Mean?

BACKGROUND: Meconium drug testing is performed to detect potentially harmful drug exposures in a newborn. Interpretation of meconium drug testing results can be complicated based on the patterns and proportional concentrations of the drug(s) and/or drug metabolite(s) detected. METHODS: The objective of this study was to analyze meconium drug testing patterns in a de-identified dataset from a national reference laboratory (n = 76,631) and in a subset of the data, wherein specimens originated at a single academic medical center for which detailed chart review was possible (n = 3635). Meconium t…

A case of meconium aspiration syndrome in a bottlenose dolphin (Tursiops truncatus) calf.

Stillbirth and neonatal mortality are significant problems in captive breeding of dolphins, however, the causes of these problems are not fully understood. Here, we report a case of meconium aspiration syndrome (MAS) in a male neonate of bottlenose dolphin (Tursiops truncates) who died immediately after birth. At necropsy, a true knot was found in the umbilical cord. The lungs showed diffuse intraalveolar edema, hyperemic congestion and atelectasis due to meconium aspiration with mild inflammatory cell infiltration. Although the exact cause of MAS in this case was unknown, fetal hypoxia due po…
