INTRODUCTION: This study aims to compare the efficiency and outcomes in adults with obesity and intellectual and developmental disabilities in rural areas.
METHODS: Utilizing the administrative database of the rural health system and Nevada Medicaid (January 1, 2022 and December 31, 2024), 257 adults with obesity and with or without intellectual and developmental disabilities were identified. A propensity score was applied to match demographics and comorbidity conditions between subjects with and without intellectual and developmental disabilities. Logistic regression analysis was used to ide…

Background: Fungal infections, particularly among immunocompromised individuals, present significant challenges due to rising incidence rates, treatment costs, and increasing resistance to antifungal agents. This study evaluates trends in antifungal use among Medicaid beneficiaries, focusing on prescribing patterns, costs, and pricing to optimize therapy. Methods: Using the national Medicaid outpatient pharmacy claims data collected by the US Center of Medicare and Medicaid Services, a retrospective drug utilization analysis was conducted for antifungal medications from 2009 to 2023. Antifunga…

BACKGROUND: Substantial cancer underdiagnosis, especially early-stage cancers, occurred during the COVID-19 pandemic in the United States. Medicaid expansion under the Affordable Care Act could facilitate access to timely detection of cancer during pandemic-related financial and employment instability. This study examines the association of Medicaid expansion and changes in cancer stage at diagnosis during the COVID-19 pandemic.
METHODS: We compared changes in proportions of early-stage (stage I/II) cancer diagnosis in Medicaid expansion states versus non-expansion states among 1 844 515 indi…

BACKGROUND: Created to protect patients from coercive sterilization, the federally mandated 30-day waiting period for patients with Medicaid desiring permanent contraception is a known barrier to permanent contraception fulfillment. Existing research does not explicitly explore how physicians interpret and operationalize the consent policy, how they counsel regarding the waiting period, or whether physicians believe the policy should be revised. The purpose of this paper is to better understand obstetrician-gynecologist (OB-GYN) feelings, thoughts, and counseling practices around the waiting p…

BACKGROUND: Opioids are commonly used to treat moderate-to-severe acute pain in the United States (US) but are associated with serious and costly outcomes including opioid use disorder (OUD). Suzetrigine is the first oral nonopioid treatment approved in the US in over 20 years for the treatment of moderate-to-severe acute pain in adults. The objective of this analysis was to estimate the budget impact of adding suzetrigine to a state Medicaid formulary.
METHODS: A budget impact model (BIM) compared scenarios before and after suzetrigine was available from the perspective of a hypothetical Med…

OBJECTIVE: To estimate the prevalence and patterns of drug overdoses in adolescents presenting to an inpatient or emergency department setting by substance and intent in a national sample of Medicaid enrollees.
METHOD: National Medicaid administrative claims data covering publicly insured adolescent (12-18 years old) enrollees from 2016 to 2020 were analyzed. Drug overdose was defined by an inpatient or emergency department poisoning code (ICD-10-CM T36-T50) for intentional, unintentional, or undetermined intent. Per calendar year, we estimated the prevalence of adolescents with a drug overdo…

BACKGROUND: Uterine cancer, for which diagnosis is based on evaluation of symptoms, most commonly postmenopausal bleeding, is one of the few cancers in the United States with rising incidence and mortality. Inability to access diagnostic services due to lack of insurance coverage may lead to delayed diagnosis and inferior outcomes.
OBJECTIVE: The aim of the study was to examine whether the onset of Medicare eligibility at age 65 was associated with a spike in the incidence of uterine cancer and whether this association was attenuated by Medicaid expansion through the Affordable Care Act.

BACKGROUND: Administrative claims provide rich data for HIV-related program implementation or research. However, identifying people with HIV in these data remains challenging. We examined the validity of competing case identification algorithms to identify adults with HIV in claims data.
METHODS: Claims with diagnosis, procedure, and prescription drug codes, as well as enrollment and demographic information, came from Virginia Medicaid, 2012-2023. Surveillance data came from the Virginia Department of Health's Care Markers database, 2012-2023. We created 12 case identification algorithms base…

Background: In 2014, the Affordable Care Act Medicaid Expansion (ME) increased Medicaid eligibility for adults with an income level up to 138% of the federal poverty level. In this study, we examined the impact of ME on mortality and amputation in patients with peripheral artery disease (PAD). Methods: The 100% MedPAR and Part-B Carrier files from 2011 to 2018 were queried to identify all fee-for-service Medicare beneficiaries with PAD using International Classification of Diseases codes. Our primary exposure was whether a state had adopted the ME on January 1, 2014. Our primary outcomes were …

BACKGROUND: Medicaid expansion through the Affordable Care Act (ACA) has been associated with greater access to and utilization of surgical services in underserved populations. However, its impact on use of hand surgical care is less understood. The purpose of this study was to evaluate the association between New York State adoption of the ACA and carpal tunnel release (CTR) procedural volume in Medicaid beneficiaries.
METHODS: The authors conducted a pooled cross-sectional analysis of patients who underwent CTR using the Healthcare Cost and Utilization Project New York State all-payer datab…

Perinatal mood and anxiety disorders (PMAD), a leading cause of perinatal morbidity and mortality, affect approximately one in seven births in the US. To understand whether extending pregnancy-related Medicaid eligibility from sixty days to twelve months may increase the use of mental health care among low-income postpartum people, we measured the effect of retaining Medicaid as a low-income adult on mental health treatment in the postpartum year, using a "fuzzy" regression discontinuity design and linked all-payer claims data, birth records, and income data from Colorado from the period 2014-…

Under 2020 Centers for Medicare and Medicaid Services (CMS) conditions of coverage, Organ Procurement Organizations (OPOs) will be decertified if their 95% upper confidence limit for donation or transplant rate falls below the previous year's median (tier 3) and must recompete if either is below the 75th percentile (tier 2). This study aimed to examine the associations of CMS metrics with OPO volume and evaluate an alternate observed-to-expected tiering system using simulation analysis and CMS's OPO public report. In 2021, CMS tier 3 and 2 OPOs had significantly larger volumes than tier 1 OPOs…

Despite a growing number of adults with intellectual and developmental disabilities (IDD) and documented risk for adverse outcomes as they age, little is known about the health and healthcare patterns of adults with different IDD throughout adulthood. This study uses Wisconsin Medicaid claims data to characterize health conditions among adults with IDD. Results indicate high prevalence of asthma, diabetes, heart disease, and hypertension. Heart disease rates were particularly high, having been observed among 39% of autistic adults, 64% of autistic adults with intellectual disability (ID), 67% …

Opioid overdose remains a problem in the United States despite pharmacotherapies, such as buprenorphine, in the treatment of opioid use disorder. This study characterized changes in buprenorphine use. Using the Drug Enforcement Administration's ARCOS, Medicaid, and Medicare claims databases, patterns in buprenorphine usage in the United States from 2018 to 2020 were analyzed by examining percentage changes in total grams distributed and changes in grams per 100 K people in year-to-year usage based on ZIP code and state levels. For ARCOS from 2018 to 2019 and 2019 to 2020, total buprenorphine d…

BACKGROUND: Medicaid, unlike any other insurance mechanism, imposes a consent requirement on female patients desiring sterilization that must be completed at least 30 days, but no more than 180 days, before sterilization. Desired sterilization cannot be completed in the Medicaid population without this consent. Large-scale national evidence is lacking on the effect of this requirement.
OBJECTIVE: This study aimed to explore the influence of insurance status on the achievement of postpartum sterilization after a self-reported unwanted birth in a nationally representative sample.

Oregon expanded Emergency Medicaid coverage to 60 days of postpartum care in 2018, facilitating ongoing care for conditions such as gestational diabetes. We linked Medicaid claims and birth certificates from 2010 to 2019 in Oregon and South Carolina, which did not expand postpartum care. We used a difference-in-difference design to measure the effects of postpartum care coverage among Emergency Medicaid recipients with gestational diabetes. Primary outcomes were receipt of recommended glucose tolerance testing and new diagnosis of Type 2 diabetes. Our sample included 2,270 live births among a …
