An Interpretable and Expandable Deep Learning Diagnostic System for Multiple Ocular Diseases: Qualitative Study.

BACKGROUND: Although artificial intelligence performs promisingly in medicine, few automatic disease diagnosis platforms can clearly explain why a specific medical decision is made. OBJECTIVE: We aimed to devise and develop an interpretable and expandable diagnosis framework for automatically diagnosing multiple ocular diseases and providing treatment recommendations for the particular illness of a specific patient. METHODS: As the diagnosis of ocular diseases highly depends on observing medical images, we chose ophthalmic images as research material. All medical images were labeled to 4 typ

Drug-induced Liver Injury: The Hepatic Pathologist's Approach.

The evaluation of liver biopsies in suspected drug-induced liver injury (DILI) can be complex. The biopsy may be approached systematically, by identification of histologic lesions and then identification of the overall pattern of injury. Potential DILI must be separated from concomitant non-DILI liver disease. The findings can be analyzed with respect to the various prescription and nonprescription medications and dietary supplements under suspicion to provide a complete interpretation of the findings. The pathologic findings, the histologic differential diagnosis, and expert interpretation ar

Continuous reference curves for hematology and biochemistry analytes in pregnancy - a prospective study.

OBJECTIVES: Pregnancy involves dynamic metabolic changes that cannot be accurately interpreted using population reference intervals (RI). The aim was to develop continuous reference curves for common hematology and biochemistry laboratory analytes in pregnancy and to assess their agreement with the RIs for non-pregnant women, which are routinely reported for pregnant women. METHODS: A prospective study was conducted at the Clinical Hospital Centre Rijeka, Croatia, from September 2022 to December 2025. Inclusion criteria were age ≥18 years, singleton pregnancy, normal ultrasound and prenatal s

The Simulated Cardiology Clinic: A Standardized Patient Exercise Supporting Medical Students' Biomedical Knowledge and Clinical Skills Integration.

INTRODUCTION: Development of cardiac disease-related diagnostic skills-including hypothesis-driven data gathering, heart sound interpretation, and ECG interpretation-is an important component of medical student training. Prior studies indicate trainees' performance of these skills is limited. Simulation provides students with opportunities to practice integrating their developing knowledge in a relevant clinical context. We developed a simulated clinic activity for second-year medical students consisting of standardized patient (SP) cases representing cardiovascular (CV) diseases. METHODS: St

Interrater reliability in interpretation of electrocorticographic seizure detections of the responsive neurostimulator.

OBJECTIVE: Electrocorticographic (ECoG) recordings from patients with medically intractable partial-onset seizures treated with a responsive neurostimulator system (the RNS System) that detects and stores physician-specified ECoG events provide a new data resource. Interpretation of these recordings has not yet been validated. The purpose was to evaluate the interrater interpretation of chronic ambulatory ECoG recordings obtained by the RNS System. METHODS: Five pairs of five experts independently classified 7,221 ECoG recordings obtained from 128 patients with medically intractable partial s

Tab-Cox: An Interpretable Deep Survival Analysis Model for Patients With Nasopharyngeal Carcinoma Based on TabNet.

The nutritional status of cancer patients is closely associated with the clinical progression of the disease. A survival analysis model combined with a neural network can predict future disease trends in patients, facilitating early prevention and assisting physicians in making diagnoses. However, the complexity of neural networks and their incompatibility with medical tabular data can reduce the interpretability of the model. To address this issue, thr paper propose a novel survival analysis model called Tab-Cox, which combines TabNet and Cox models. This model is specifically designed to pre

Association between psychoactive medications and delirium in hospitalized patients: a critical review.

Psychoactive medications are often reported as delirium risk factors in hospitalized patients, and delirium induced by medication is potentially avoidable. The authors critically reviewed the evidence for a role of medications in delirium etiology. Only a few positive associations were noted. Use of psychoactive medications, considered together as a single variable, and use of opioids increased the risk of delirium. Data were scarce and sometimes conflicting, and methodological limitations were often present. The suspected association between psychoactive drugs and delirium cannot be unambiguo

From "Customary Morality" to "Reflective Morality": a physician's personalized competence development facilitated by narrative medicine.

BACKGROUND: Contemporary medicine faces ethical complexity arising from biotechnological advances, the growth of patients' needs, and the occurrence of cross-cultural value conflicts. These challenges expose the limits of customary morality, an unreflective and habit-driven moral framework prevalent in traditional medical training. This situation demands a more dynamic, context-sensitive, and humanistic form of moral commitment from physicians, while also requiring the development of professional competence. Narrative medicine provides a potential solution to this problem, yet its theoretical

Reporting the CENP-F-like (AC-14) antinuclear antibody pattern in the internet age: a case report.

INTRODUCTION: Laboratory immunologists occasionally encounter rare immunofluorescence patterns, such as the centromere protein F (CENP-F)-like pattern, classified as AC-14 by the International Consensus on ANA Patterns (ICAP). The clinical and biological significance of anti-CENP-F antibodies remains unclear, but their presence appears to be related to events involving increased or abnormal cell proliferation, including malignancies. METHODS: This case report describes the diagnostic journey of a patient who presented with clinical manifestations of chronic spontaneous urticaria (CSU), the la

Multisite Validation of a Functional Assay to Adjudicate SCN5A Brugada Syndrome-Associated Variants.

BACKGROUND: Brugada syndrome is an inheritable arrhythmia condition that is associated with rare, loss-of-function variants in SCN5A. Interpreting the pathogenicity of SCN5A missense variants is challenging, and ≈79% of SCN5A missense variants in ClinVar are currently classified as variants of uncertain significance. Automated patch clamp technology enables high-throughput functional studies of ion channel variants and can provide evidence for variant reclassification. METHODS: An in vitro SCN5A-Brugada syndrome automated patch clamp assay was independently performed at Vanderbilt University

Detection of potential drug-drug interactions for risk of acute kidney injury: a population-based case-control study using interpretable machine-learning models.

Background: Acute kidney injury (AKI), with an increase in serum creatinine, is a common adverse drug event. Although various clinical studies have investigated whether a combination of two nephrotoxic drugs has an increased risk of AKI using traditional statistical models such as multivariable logistic regression (MLR), the evaluation metrics have not been evaluated despite the fact that traditional statistical models may over-fit the data. The aim of the present study was to detect drug-drug interactions with an increased risk of AKI by interpreting machine-learning models to avoid overfitti

Yaws recurrence in children at continued risk of infection.

BACKGROUND: In yaws-endemic areas, children with Treponema pallidum subsp. pertenue infection may suffer recurrent episodes due to either reinfection or relapse. However, the possibility of infection with other cutaneous ulcer causative agents and difficulties in interpreting standard laboratory results challenges the estimation of yaws recurrence rates. METHODS: We estimated the rates of yaws recurrences in the Lihir Island (Papua New Guinea) using two approaches: passive surveillance based on a retrospective screening of electronic medical records of cutaneous ulcers diagnosed using serolog

Examining the relationship between medical diagnoses and patterns of performance on the modified Dynamic Gait Index.

BACKGROUND: In the original and modified Dynamic Gait Index (mDGI), 8 tasks are used to measure mobility; however, disagreement exists regarding whether all tasks are necessary. The relationship between mDGI scores and Centers for Medicare & Medicaid Services (CMS) severity indicators in the mobility domain has not been explored. OBJECTIVE: The study objectives were to examine the relationship between medical diagnoses and mDGI scores, to determine whether administration of the mDGI can be shortened on the basis of expected diagnostic patterns of performance, and to create a model in which mD
