Evaluation of the completeness of a medical record dataset linked to administrative claims through tokenization in patients with Hemophilia B.

Real-world evidence (RWE) is increasingly used to inform healthcare delivery and expedite access to new therapies, but its value depends on the quality and completeness of information in real-world data (RWD) sources. Tokenization technology enables the integration of patient data from multiple sources, facilitating the development of a comprehensive overview of the patient journey. This study evaluated the accuracy of linked electronic health record (EHR) and insurance claims data for 93 patients with hemophilia B (HB). Tokenization enables secure linkage of patient data from multiple sources

Screening for Depression, Anxiety, and Suicidality: Insights From Electronic Health Record Data in an Academic Medical Center.

INTRODUCTION: In 2023, 14.6 million adult Americans had serious mental illness, including Major Depressive Disorder (MDD) and anxiety disorders. Further, 12.3 million adult Americans had serious suicidal thoughts and behaviors. This report examined mental health screening in non-psychiatry clinics to identify gaps in screening practices using electronic health record (EHR) data. METHODS: De-identified data were accessed from the Epic EHR from 2016 to 2022. All participants with a recorded encounter with a mental health screening within 5 years of the first encounter date were included. The sc

Implementation of Electronic Medical Record Interventions to Maximize Opioid Disposal after Surgery in Routine Clinical Practice.

BACKGROUND: Despite guideline-directed opioid prescribing after operation, many patients retain excess opioids. Leftover pills increase the risk for misuse, diversion, and dependency. It is unclear whether interventions applied in routine clinical practice can increase excess opioid disposal rate. STUDY DESIGN: All adult patients at our institution with postoperative opioid prescriptions were identified in general surgery, otolaryngology, plastic surgery, neurosurgery, urology, and transplant surgery. Disposal rates for 453 preintervention patients (operation between March 1, 2023, and June 3

A qualitative exploration of the early adoption of an electronic medical record system for type 1 diabetes management in Rwanda.

BACKGROUND: Delivering adequate quality care remains a challenge in many low-and middle-income countries (LMICs), especially for people living with type 1 diabetes (T1D), requiring a complex treatment regimen. Digital health solutions, including electronic medical record (EMR) systems, have shown potential to improve delivery and quality of care but still require a successful implementation. OBJECTIVE: To analyze the adoption of a newly implemented EMR system in Rwanda from the perspectives of individuals with T1D, health care providers, and experts. STUDY DESIGN: A qualitative study was con

Benchmarking transformer-based models for medical record de-identification in a single center multi-specialty evaluation.

Protecting patient confidentiality is central to enabling research using electronic health records. Automated text de-identification offers a scalable alternative to manual redaction. However, different approaches vary in accuracy and adaptability. We evaluated four transformer-based, task-specific models and five large language models on 3,650 clinical records spanning general and specialty datasets from a UK hospital group. Records were dual-annotated by clinicians, allowing precise comparison of performance. The Microsoft Azure de-identification service achieved the highest F1 score, approa

Reduced order computational fluid dynamic simulations in the thoracic aorta are associated with disease recorded in a medical biobank.

This study examines the association of aortic geometric traits with flow characteristics and disease outcomes in 3204 patients from the Penn Medicine Biobank (PMBB). Using an nnU-Net, the thoracic aorta was segmented from CT scans to measure traits such as diameter and length. A one-dimensional reduced-order Navier-Stokes model (ROM) simulated aortic pulse pressure under various physiological conditions. Phenome-wide association studies (PheWAS) were conducted to link aortic traits to diseases using electronic health records (EHR). Significant associations were identified between aortic pulse

Towards a prescribing monitoring system for medication safety evaluation within electronic health records: a scoping review.

BACKGROUND: Medical care can fail for various reasons: diseases can remain undetected and their severity misjudged, therapies can be incorrectly dosed or ineffective, and therapies can trigger new conditions or adverse drug reactions (ADR). To manage the complexity of changing patient circumstances, data-driven techniques play an increasingly important role in monitoring patient safety and treatment success. Therefore, clinical prediction models need to consider longitudinal factors ("Prescribing Monitoring") to ensure clinically meaningful results and avoid misclassification in the dynamic he

Ascertaining accurate exposure to aspirin and other antithrombotic medications using structured electronic health record data.

BACKGROUND: Ascertaining accurately the exposure to antithrombotic medications for both research and quality initiatives has been challenging due to a multitude of reasons: aspirin, the most commonly used antithrombotic, is available over the counter in the United States. Additionally, antithrombotic medications are frequently interrupted for bleeding and procedures. OBJECTIVES: We aimed to develop and validate an algorithm to capture accurately the longitudinal exposure to antithrombotic medications including aspirin using the electronic health record. METHODS: We used the Medical Inpatient

Accounting for informative observation process in transition models of binary longitudinal outcome: Application to medical record data.

When extracting medical record data to form a retrospective cohort, investigators typically focus on a pre-specified study window, and select subjects who had hospital visits during that study window. However, such data extraction may suffer from an informative observation process, since sicker patients may have hospital visits more frequently. For example, Consecutive Pregnancy Study is a retrospective cohort study of women with multiple pregnancies in 23 Utah hospitals from 2003 to 2010, where the interest is to understand the risk factors of recurrent pregnancy outcomes, such as preterm bir

Comparing self-report medication data from a longitudinal study on intellectual disability and national dispensing records.

BACKGROUND: Medication data are a valuable resource in epidemiological studies. As the most common data collection method of medication data is self-report, it is important to understand the accuracy of this in comparison with other methods such as dispensing records. The aim of this study was to compare the agreement between two different sources of medication data of older adults with intellectual disability (ID). METHODS: Self-report medication data were gathered from the Intellectual Disability Supplement to the Irish Longitudinal Study on Ageing and linked to national pharmacy dispensing

Cognitive impairment predicts medication discrepancies in Huntington's Disease: patient self-report compared to pharmacy records.

BACKGROUND: Proper medication reconciliation (= comparing the accuracy of patient-reported medication use with pharmacy records) could prevent potentially dangerous situations such as drug-drug interactions and hospitalization. This is particularly important when patients rely on multiple medications, such as in neurodegenerative disorders like Huntington's Disease (HD). Currently, it is unknown how often medication discrepancies occur in HD patients and which factors contribute to the discrepancies. OBJECTIVE: Identify prognostic factors of medication discrepancies in HD, using patient-repor

Validating pertussis data measures using electronic medical record data in Ontario, Canada 1986-2016.

BACKGROUND: Pertussis is a reportable disease in many countries, but ascertainment bias has limited data accuracy. This study aims to validate pertussis data measures using a reference standard that incorporates different suspected case severities, allowing for the impact of case severity on accuracy and detection to be explored. METHODS: We evaluated 25 pertussis detection algorithms in a primary care electronic medical record database between January 1, 1986 and December 30, 2016. We estimated sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV). We

Exertional Heat Stroke and Rhabdomyolysis: A Medical Record Review and Patient Perspective on Management and Long-Term Symptoms.

INTRODUCTION: Exertional heat stroke (EHS) is a medical emergency, occurring when the body generates more heat than it can dissipate, and frequently associated with exertional rhabdomyolysis (ERM). In the present study we aimed to (I) identify clinical features and risk factors, (II) describe current prehospital management, (III) investigate long-term outcomes including the impact on mental health, and review the guidance received during restarting activities. We hope that our approach will improve individual and organizational heat illness preparedness, and improve follow-up care. METHODS: W
