Psychiatric Education for Pediatric Emergency Medicine Fellows: Should We be Doing More?

UNLABELLED: The number of psychiatric encounters is steadily increasing across all pediatric emergency departments. Based on this growing national crisis, we hypothesized there is a lack of robust education in behavioral/mental health emergencies during pediatric emergency medicine (PEM) fellowship. METHODS: This was a cross-sectional survey of PEM fellowship directors. The survey was submitted to the Pediatric Emergency Medicine Program Director Survey Committee via REDCap to all 83 fellowship directors. RESULTS: Fifty-five (66%) PEM fellowship directors completed the survey from September

Definitions of sleeplessness in children with attention-deficit hyperactivity disorder (ADHD): implications for mothers' mental state, daytime sleepiness and sleep-related cognitions.

BACKGROUND: Sleep disturbances are common in children with attention-deficit hyperactivity disorder (ADHD). Sleeplessness is frequently reported although results are inconsistent perhaps because different definitions for it are applied. This study looked at maternal functioning and child objective sleep patterns in relation to different definitions of sleeplessness in children with ADHD. METHODS: The study included 45 children (aged 3-14 years) with ADHD and their mothers. Sleeplessness was defined according to: (i) yes/no report of whether mothers thought their children had a problem with sl

Improving the oral function of patients with upper and lower lip tensor fascia lata slings for patients with flaccid facial nerve palsy.

BACKGROUND: Facial nerve paralysis (FNP) often causes decreased quality of life and may lead to significant facial dysfunction. Oral competence is frequently raised as a concern by patients as it impacts nutrition, hydration, social participation, and mental health. This can result in social isolation and reduced capacity to return to vocational roles. Despite its prevalence, it is incompletely understood and rarely described. This study prospectively evaluated the impact that facial nerve static and dynamic reanimation has on oral competence, with a specific focus on speech intelligibility an

A study protocol for the randomized controlled trial SAFIR FAMILY TALK: a selective primary preventive intervention vs. service as usual for children of parents with mental illness.

BACKGROUND: Children of parents with mental illness have an increased risk of developing mental illness themselves throughout their lifespan. This is due to genetic factors but also environmental disadvantages during childhood associated with parental mental illness. Selective primary preventive interventions for the children are recommended to mitigate risk factors and strengthen protective factors, but large-scale, longitudinal studies are needed. This study aims to investigate the effect of the Family Talk Preventive Intervention in a cohort of children and their parents with mental illness

Impaired Decision-Making Capacity in a Patient With Oral Squamous Cell Carcinoma: Weighing the Benefits and Harms of Treatment Against Objection.

Impairment of medical decision-making capacity is common in acutely ill patients especially those with solid malignancies. Many of these patients lack advance directives and healthcare proxies leaving clinicians with unclear guidance on subsequent treatment decisions and few existing mechanisms to address this issue. We present a case of a 55-year-old man with recurrent oral squamous cell carcinoma who lacked decision-making capacity due to cognitive impairment. He did not have an advance directive or documented healthcare proxy. He declined curative-intent cancer-directed therapy. The dilemma

Financial Decision-Making Capacity and Patient-Centered Discharge.

An ethically sound discharge from the hospital can be impeded by a number of factors, including a lack of payor for a patient's care, a lack of appropriate discharge options, and a lack of authority to sign a patient into a long-term facility. In some cases, the primary barrier involves the patient's lack of financial decision-making capacity. When a patient's income comes primarily from government assistance, financial decision making is connected to both the individual's well-being and to fair allocation of resources. Taking away another person's financial independence is a substantial intru

Involuntary Treatment of Patients With Life-Threatening Anorexia Nervosa.

Anorexia nervosa has the highest mortality rate of any psychiatric illness. Predictors of mortality include chronicity of the illness, critically low body weight, and binging and purging behavior. Delusional beliefs body image, coupled with impaired judgment and cognition caused by starvation, often result in these patients adamantly resisting efforts to treat them. Guardianship, although useful in assisting with medical treatment decisions for patients with anorexia nervosa who are critically medically ill, is usually an inadequate intervention with respect to psychiatric treatment for these

Maternal Diabetes and Cognitive Performance in the Offspring: A Systematic Review and Meta-Analysis.

OBJECTIVE: Diabetes during gestation is one of the most common pregnancy complications associated with adverse health effects for the mother and the child. Maternal diabetes has been proposed to negatively affect the cognitive abilities of the child, but experimental research assessing its impact is conflicting. The main aim of our study was to compare the cognitive function in children of diabetic and healthy pregnant women. METHODS: A systematic review and meta-analysis was conducted through a literature search using different electronic databases from the index date to January 31, 2015. We

Anxiety-promoting parenting behaviors: a comparison of anxious parents with and without social anxiety disorder.

While parenting behaviors among anxious parents have been implicated in the familial transmission of anxiety, little is known about whether these parenting behaviors are unique to specific parental anxiety disorders. The current study examined differences in the use of five specific parenting behaviors (i.e., warmth/positive affect, criticism, doubts of child competency, over-control, and granting of autonomy) in anxious parents with (n = 21) and without (n = 45) social anxiety disorder (SAD) during a 5-minute task with their non-anxious child (aged 7-12 years, M = 9.14). Competency to consent to treatment.

Competency to consent to treatment is an especially critical determination to make in the field of psychiatry. Psychiatric patients are often capable, despite their illness, of self-advocacy. Careful assessments are required to differentiate competent patients from incompetent patients. Moreover, the character of their illness, from psychosis to organic brain disease, has been found to correlate with a lack of competency. The presence of auditory hallucinations or delusions, however, are not pathognomonic of incompetency. Currently, there exists no standardized method to establish competency,

Exploring the relation between masculinity and mental illness stigma using the stereotype content model and BIAS map.

The current research explored the association of masculinity and stigma toward mental illness using theoretical predictions stemming from the stereotype content model and BIAS map. Two correlational studies (Ns = 245, 163) measured stereotypes, emotions, and behavioral intentions in relation to masculine, feminine, and gender-neutral disorders. Participants perceived masculine disorders as lacking personal warmth and competence. Masculine disorders also elicited more negative emotions and behavioral intentions. Two experimental studies (Ns = 161, 431) manipulated personal warmth, sex, and type

Early environment, emotions, responses to stress, and health.

A harsh early family environment is related to mental and physical health in adulthood. An important question is why family environment in childhood is associated with these outcomes so long after its initial occurrence. We describe a program of research that evaluates a model linking these variables to each other. Specifically, we hypothesize that low social competence and negative emotional states may mediate relations between a harsh early family environment and physiological/neuroendocrine responses to stress, as well as long-term health outcomes. We report evidence that the model characte

Caregiving and developmental factors differentiating young at-risk urban children showing resilient versus stress-affected outcomes: a replication and extension.

This study tested hypotheses from an organizational-developmental model for childhood resilience. In this model resilience reflects a child's mastery of age-salient objectives, in the face of substantial adversity, by drawing on internal and external resources that enhance processes of adaptation specific to each developmental stage. Interviews were conducted with parents of 122 7- to 9-year-old urban children exposed to multiple risk factors, 69 classified as resilient and 53 as maladjusted. Consistent with predictions generated by the model: (1) characteristics of a child's caregiving system

Similar eye movements in aphantasia and visualizers during mental map exploration.

During visual mental imagery, our eyes tend to reproduce patterns of movement similar to those observed in visual perception, even in the absence of external stimuli. However, imagery vividness varies along a spectrum, with congenital aphantasia at the lower extreme, where individuals report a reduced or absent ability to visualize voluntarily. Nevertheless, they can typically state from memory that Bordeaux is to the left of Paris on a map of France while denying having conjured any mental image. What cognitive mechanisms underlie this dissociation? Some theories suggest aphantasia stems from

Impact of Dementia, Depression, and Other Mental Disorders on Reoperation and Mortality Among Hip Fracture Patients: A Nationwide Danish Cohort Study.

BACKGROUND: Despite the rising global burden of mental disorders, their impact on complication risk following hip fracture surgery remains unclear. We examined reoperation and mortality risks after hip fracture surgery, investigating patients with and without moderate to severe mental disorders. METHODS: Using a nationwide cohort design, we identified patients undergoing hip fracture surgery from the Danish Multidisciplinary Hip Fracture Register. Mental disorders (including organic disorders (dementia), substance use, schizophrenia, mood disorders, and neurotic disorders) and reoperations we

Competencies required when police patrol officers encounter people in suicidal crisis: a Delphi study.

INTRODUCTION: Suicide is a leading cause of death globally and represents a common, resource-intensive duty for police patrol officers (PPOs). Responding to suicidal crises is often traumatic, and PPOs rarely receive comprehensive training, leaving them inadequately prepared. Police authorities frequently assume operational responsibility in such situations, encompassing emotional, legal, tactical and safety considerations. AIM: This study aimed to explore and describe the competencies required of PPOs when encountering individuals in suicidal crisis. METHOD: An exploratory, descriptive desi

Experiences of Disabled Women With Perinatal Care Providers: A Qualitative Study.

BACKGROUND: Despite growing attention to patient-provider relationships in the general population, there is a notable gap in the literature regarding the quality of these relationships in the context of maternal disability. OBJECTIVE: This qualitative study explored the experiences of pregnant and recently pregnant women with disabilities in their interactions with health care providers during the perinatal period. METHODS: As part of a larger study examining the effects of psychosocial stress on maternal and neonatal outcomes among women with disabilities, we conducted semi-structured inter
