Changes in mineralised tissue at the site of origin of condylar fracture are present before athletic training in Thoroughbred horses.

AIM: To show that changes are present at the site of origin of metacarpal condylar fracture in young Thoroughbred horses before they enter race training. METHOD: Bone slices, 2 mm thick, in three mediolateral planes through the centre of rotation of the metacarpo-phalangeal joint (MCPJ) of both distal third metacarpal bones (Mc3) of 12 Thoroughbred horses aged 17 months, were imaged using point-projection digital X-ray imaging (muXR). RESULTS: In some horses, linear or ovoid radiolucency was found in articular calcified cartilage (ACC) and subchondral bone of the palmaro-distal aspect of the

Correction of thumb deformities after burn: versatility of first dorsal metacarpal artery flap.

In the management of a debilitated burned hand due to contractures, thumb reconstruction constitutes the most crucial part for a beneficial functional outcome. Among the limited local flap alternatives for the thumb, the first dorsal metacarpal artery flap, harvested from the dorsal aspect of the index finger can provide elastic, durable and sensate coverage for soft tissue defects after contracture release. In a 3-year period, neurovascular island first dorsal metacarpal artery flap was used in 14 patients suffering thumb deformities. The time elapsed after the underlying injury until reconst

A molecular and clinical study of Larsen syndrome caused by mutations in FLNB.

BACKGROUND: Larsen syndrome is an autosomal dominant osteochondrodysplasia characterised by large-joint dislocations and craniofacial anomalies. Recently, Larsen syndrome was shown to be caused by missense mutations or small inframe deletions in FLNB, encoding the cytoskeletal protein filamin B. To further delineate the molecular causes of Larsen syndrome, 20 probands with Larsen syndrome together with their affected relatives were evaluated for mutations in FLNB and their phenotypes studied. METHODS: Probands were screened for mutations in FLNB using a combination of denaturing high-performa

[Lateral sagittal infraclavicular block: a case report of three patients].

Magnetic resonance imaging (MRI) analysis performed on healthy volunteers assessed the effectiveness of brachial plexus block via the lateral sagittal infraclavicular approach to be easy-performed and relatively safe. In this case report, we presented three cases to whom we performed infraclavicular block via the lateral sagittal approach. One of our cases was a high risk patient with osteomyelitis of the elbow joint and the other was a patient who had required surgery for Dupuytren's contracture. The third one was a patient admitted to emergency room who had got open metacarps fractures. Late

Angel-shaped phalanges in brachydactyly C: a case report, and speculation on pathogenesis.

We describe a woman and her daughter affected by brachydactyly type C. The unusual feature in the child included the striking 'angel-shaped' appearance of the proximal phalanges of the index and middle fingers of one hand, whereas more typical triangular epiphyses with elongation of their radial side were present at the same location in the opposite hand. It is suggested that this peculiar phalangeal configuration occurs as a transitory event in early or mid childhood in phalanges that are marked by severe ossification delay, which is most prominent at the level of the primary ossification cen

Percutaneous treatment of carpal, metacarpal, and phalangeal injuries.

Percutaneous fixation of hand fractures is a common technique that takes advantage of the subcutaneous nature of hand bones, their small size, and their limited loading potential for stress placed on hardware. Percutaneous wire fixation supplements cast fixation when plaster cannot hold particular reductions, and allow surgical fixation with limited postoperative swelling. In the first part of the current study, the types of wires that are used for hand fixation, fluoroscopy, helpful instruments, and the basic techniques used for this type of surgery are discussed. In the second part of the st

[Are there guidelines for treatment of metacarpal fractures? Personal results and literature analysis of the last 12 years].

As the treatment of metacarpal fractures is today still a controversial subject, we conducted an analysis of the literature in order to present the different therapy guidelines, indications, and their results. The data from the follow-up of 1602 patients was taken from literature which dated from 1 January 1984 to 31 March 1996. A total of 522 patients who underwent surgery received K-wires, screws or external minifixateur. The conservative approaches ranged from immobilization to various methods of mobilization with different aids or without fixation of the fracture. The mobilization in a bra

Osteolipoma of the hand: a case report.

A 61-year-old, healthy woman presented with a six-month history of a mass on the radial volar aspect of her dominant right hand. The patient had a history of trauma to the hand six years previously. Radiographs showed a bony lesion in the index metacarpal shaft and the MRI showed a bony lesion and soft tissue mass suggestive of a parosteal lipoma. Surgical excision of the lesion revealed a lipoma overlying a bony exostosis or osteolipoma of the hand. Post-operatively, the patient's symptoms resolved and she had no recurrence of the tumor.

Intra-articular pressure, elastance and range of motion in healthy and injured racehorse metacarpophalangeal joints.

The objective of this study was to determine if intraarticular pressure, elastance of the soft tissue forming the dorsal pouch, and range of motion in flexion measurements are significantly different in Thoroughbred metacarpophalangeal joints with clinical evidence of idiopathic synovitis, primary synovitis, synovitis/capsulitis, or osteoarthritis relative to clinically normal joints. Forty-two metacarpophalangeal joints, in 25 active or retired Thoroughbred racehorses, were categorised by palpation and visual inspection of the palmar pouch into one of 4 increasing grades of distention. Intra-

Physeal form of the longbones of the foal.

The possible relationship between physeal diseases and physeal form prompted investigation of change in steepness of the physis in young foals. The distal and proximal aspects of the longbones were sawn sagittally in the right and frontally in the left bones. The slabs were washed to remove saw debris, arranged in order and inspected. The proximal physes had a flat or gently arched form, without obvious inclination. In the distal physes there were distinct inclinations. Inspection of an identical slab from the medial aspect of the distal radius of two series of foals of different breeds showed

Congenital fusion of the fourth and fifth metacarpals associated with primary gonadal failure.

Congenital fusion of the fourth and fifth metacarpals is described in a male infant and his maternal grandfather. Primary gonadal failure, which is present in the infant, has not been noted in previously, reported cases. The pedigree in this family is compatible with X-linked recessive or autosomal dominant inheritance with incomplete penetrance.

Combined fourth and fifth metacarpal fracture and fifth carpometacarpal joint dislocation.

Three cases of hamato-metacarpal fracture-dislocation with fracture of the shaft or base of the fourth metacarpal and dorsal dislocation of the fifth metacarpal are described. In one case this was associated with coronal fracture of the hamate. An oblique radiograph of the hand with the forearm pronated 15 degrees and 45 degrees provided a good view of the extent of the fourth and fifth carpometacarpal injury. Treatment with open reduction and internal fixation achieves good clinical results.

Radiographic geometric variation of equine long bones.

As more sophisticated research is performed to refine fracture fixation techniques for horses, it is important that normal values for the geometric properties of the bones of the appendicular skeleton be determined and that suitable controls be available. We evaluated the geometric properties of total bone width, cortical bone width, and medullary canal/trabecular bone width measured from 2 radiographic projections of equine long bones (humerus, radius, third metacarpal bone, femur, tibia, and third metatarsal bone) obtained from a general population of horses. Measurements were performed on s

[Stabilized arthroplasty of the 5th metacarpal bone. A therapeutic proposal for the treatment of old fractures-luxations of the 5th metacarpal bone].

The goals of the surgical treatment of fracture-dislocations of the fifth ray are length restoration of the fifth metacarpal, release of impingement and preservation of the carpo-metacarpal mobility. Our technic of "stabilised arthroplasty" satisfies theoretically to these three goals. It consists in resection-arthroplasty of the base of the fifth metacarpal associated with stabilisation by arthrodesis between fourth and fifth metacarpal.
