The effects of cycling shoe stiffness on forefoot pressure.

Plantar pressure data were recorded in two different shoe types to determine the effect of cycling shoe stiffness on peak plantar forefoot p … plastic design (121 kPa vs. 103 kPa, p = .005). Competitive or professional cyclists suffering from metatarsalgia or ischemia should be especially careful when using carbon fiber cycling shoes because the shoes increase peak plantar pressure, which may aggravate these foot conditions.

Optimizing the correlation between first metatarsal head position and osteotomy plane angle in modified scarf osteotomy: a three-dimensional digital analysis.

BACKGROUND: A modified Scarf osteotomy can not only correct hallux valgus deformity but also relieve lateral metatarsalgia by lowering the head of the first metatarsal. This effect is achieved by inclining the longitudinal osteotomy plane from dorsomedial to plantar-lateral. However, with the increase in the inclined angle, the correction ability for the intermetatarsal angle will weaken, which is critical for hallux valgus correction. The balance between intermetatarsal angle correction(IMAC) and the lowering of the first metatarsal head is crucial for good clinical benefits, and the inclined…

Crescentic Osteotomy for Revision Hallux Interphalangeal Joint Arthrodesis.

UNLABELLED: Treatment for rigid mallet toe deformity of the hallux has commonly consisted of arthrodesis of this interphalangeal joint. However, such procedure is not without complications resulting in operative revision. We report on a case of considerable deformity of the distal phalanx of the hallux following hallux interphalangeal joint arthrodesis that underwent revision using a unique crescentic osteotomy technique. LEVELS OF EVIDENCE: Level V: Case report/Technique.

Second and Third Metatarsophalangeal Plantar Plate Tears: Diagnostic Performance of Direct and Indirect MRI Features Using Surgical Findings as the Reference Standard.

OBJECTIVE: The objective of our study was to assess the diagnostic performance and associations of the direct and indirect MRI features of the metatarsophalangeal (MTP) joint that are thought to be related to tears of the plantar plate (PP) using surgical findings as the reference standard. MATERIALS AND METHODS: We retrospectively included 23 patients with symptomatic instability of lesser MTP joints who had undergone preoperative 1.5-T MRI and surgical assessment. The MRI examinations were independently assessed by two musculoskeletal radiologists. Using the surgical data as the reference s

Extracorporeal Shockwave Therapy in Patients with Morton's Neuroma A Randomized, Placebo-Controlled Trial.

BACKGROUND: The aim of this study was to evaluate the efficacy of extracorporeal shockwave therapy (ESWT) for the treatment of Morton's neuroma by measuring changes in patient pain, function, and neuroma size. METHODS: Patients with Morton's neuroma were randomly assigned to either the ESWT group or the sham stimulation group. Outcome measures, including visual analog scale (VAS) and American Orthopaedic Foot and Ankle Society lesser toes (AOFAS) scores, were assessed at baseline and 1 and 4 weeks after treatment. The Johnson satisfaction test was also performed 1 and 4 weeks after treatment.

Metatarsal pain and plantar hyperkeratosis in the forefeet of female professional flamenco dancers.

OBJECTIVES: The objectives of this study were to determine the frequency of metatarsal pain and of hyperkeratosis on the plantar forefoot in female professional flamenco dancers, and to determine whether there is a relationship between the two disorders. METHOD: Forty-four female professional flamenco dancers, with a minimum activity of 25 hrs/wk, participated in this cross-sectional study. The presence or absence of metatarsal pain while dancing was recorded, and plantar pressures were measured on a pressure platform, both barefoot and shod with the usual dance shoe. The heel height of the d

Problems, obstacles, and complications of metatarsal lengthening for the treatment of brachymetatarsia.

Metatarsal lengthening by distraction osteogenesis is a challenging procedure that is associated with various adverse results. This article presents and classifies adverse results arising from metatarsal lengthening. Our premise of classification is that not all adverse results that occur secondary to distraction osteogenesis of the metatarsal are true complications that affect the final outcome, but rather are problems and obstacles that present hurdles to completion of treatment. Our classification differentiates among problems, obstacles, and complications during metatarsal lengthening with

Flexor tendon sheath as a source of pain in lesser metatarsal overload.

BACKGROUND: Lesser metatarsal overload may lead to pathologic conditions ranging from plantar metatarsophalangeal joint pain to dislocation. The flexor tendon sheath lies directly plantar to the joint. The increased pressure associated with lesser metatarsal overload may lead to synovitis and pain of the flexor tendon sheath. METHODS: Fifteen consecutive patients with lesser metatarsal overload had visual analog scale scores determined at three metatarsophalangeal joint areas to determine the source of pain. The patients underwent seven maneuvers to determine the pain scale score: single-leg

A prospective investigation of the BioPro hemi-arthroplasty for the first metatarsophalangeal joint.

First metatarsophalangeal joint hemi-implant arthroplasty is commonly used for 1-sided degenerative joint disease. This study examined outcomes in patients that underwent hemi-arthroplasty using the BioPro hemi-implant. Twenty-three of 30 patients completed the 12-month follow-up between October 2001 and June 2003. Repeated measures multivariate analysis of covariance was used to consider the influence of the implant on a number of dependent variables, while controlling for the amount of first metatarsal head cartilage degeneration. The average amount of first metatarsal head cartilage degener

The use of 3D-Printed cutting guide improves the accuracy of distal chevron osteotomy for hallux valgus surgery.

BackgroundDistal chevron osteotomy is a common procedure for hallux valgus. Alteration of the osteotomy axis and angle can result in excessive change in length of the first metatarsal and hence the increased risk of transfer metatarsalgia. However, many of the Chevron osteotomies were performed freehand without objective guidance.MethodTwo surgeons with different experience levels (Resident trainee, consultant) using two techniques (freehand or with cutting guide) to perform 15 distal chevron osteotomies on 3D-printed identical first metatarsal sawbone models. The medial cutting angle and late…

Biomechanical model of minimally invasive hallux valgus surgery.

Hallux valgus is a common foot deformity characterized by outward tilting and twisting of the big toe, often accompanied by a medial prominence at the base. Minimally invasive surgical techniques are widely utilized for treating metatarsus adductus due to their advantages of smaller incisions, faster recovery, and early weight-bearing. However, due to individual variations and limited sample size, the biomechanical effects of different Kirschner wire fixation methods and the underlying mechanisms of postoperative metatarsalgia remain unclear. In this study, a finite element method was employed…

Renunciations in forefoot surgery. Critical analysis of surgical waiting lists.

INTRODUCTION: The high prevalence of forefoot pathology generates long surgical waiting lists (SWL). We have detected a considerable number … in SWL, with 84.83% of women. The pathologies with the highest rate of resignation have been Morton metatarsalgia (24%) and hallux rigidus (20.16%). The most frequent pathology, hallux valgus, records 15.96% of resignations that occur mostly between 6 and 9 months. In the CTS and IM, the resignation rate has been 17.42 and 8.92%, respectively, with higher resig …

Anatomic plantar plate repair using the Weil metatarsal osteotomy approach.

The purpose of this study was to analyze the early results of a new technique for anatomic plantar plate repair and advancement using a Weil … reduction in pain rating (P < .0001, t = 8.9, df = 27). Complications included painful hardware and metatarsalgia. There were no cases of dehiscence, malunion, nonunion, or recurrent metatarsophalangeal joint subluxation. This unique surgical technique addresses metatarsal overload and the instability of the second metatarsophalangeal joint while allowing for a …

Outcomes after the Stainsby procedure in the lesser toes: an alternative procedure for the correction of rigid claw toe deformity.

Clawing of the digits is a deformity seen both in patients with and without rheumatoid arthritis, resulting in pain and deformity in the forefoot. After failure of conservative treatment, the Stainsby procedure is one surgical option for severe clawing and metatarsalgia in both rheumatoid and nonrheumatoid feet. Results from the originating authors (G.D. Stainsby and P.J. Briggs) are consistent and reliable; however, there is little material outside of the originating center. This article reviews our experience in the Western Sussex Hospitals NHS Trust. Sixteen consecutive patients who underwe…

Results of distal metatarsal osteotomy using absorbable pin fixation.

BACKGROUND: Metatarsalgia with plantar hyperkeratosis can be a difficult problem to treat. Many different techniques have been reported with the Weil osteotomy very popular currently. Despite good results complications include metatarsophalangeal stiffness, hyperextension of the toe, prehensile deficit and a reduction in strength. We evaluated our results with a distal diaphyseal osteotomy. MATERIALS AND METHODS: From 1999 to 2005, we performed 66 distal midshaft subtraction osteotomies in 62 patients suffering from metatarsal overload without metatarsophalangeal subluxation.
