Deployment of the 1st Area Medical Laboratory in a Split-Based Configuration During the Largest Ebola Outbreak in History.

BACKGROUND: The U.S. Army 1st Area Medical Laboratory (1st AML) is currently the only deployable medical CBRNE (Chemical, Biological, Radiological, Nuclear, and Explosives) laboratory in the Army's Forces Command. In support of the United States Agency for International Development Ebola response, the U.S. military initiated Operation United Assistance (OUA), and deployed approximately 2,500 service members to support the Government of Liberia's Ebola control efforts. Due to its unique molecular diagnostic and expeditionary capabilities, the 1st AML was ordered to deploy in October of 2014 in

Upper Limb Disability in Men with Combat-Related Upper Limb Injury, 8 Years Post-Injury: The ADVANCE Cohort Study.

INTRODUCTION: Upper limb (UL) injuries place a considerable economic and readiness burden on the military. Long-term outcomes of UL combat-related injuries have not been examined to the same extent as those of lower limb injuries. Specifically, a better understanding of long-term outcomes following non-amputation UL injuries is essential given the crucial role of the ULs in daily activities and quality of life. This study will compare UL disability in (1) participants with UL combat injury, non-UL combat injury, and no combat injury, and (2) the presence/absence of UL injury types. This study

Out of sight out of mind: an examination of mental health problems in UK military reservists and veterans.

BACKGROUND: Reservists often have different experiences to regular military personnel which may impact their mental health. AIMS: To investigate the incidence of mental health problems in both active and veteran reservists and determine how this compares to regular service personnel and ex-regular veterans. METHOD: Five studies which included reservist and/or veteran participants, a validated assessment of mental health problems, and provided primary data were included in the synthesis. Common mental health disorders, post-traumatic stress disorder and alcohol use disorder were examined. RE

Energy Drink Use in U.S. Service Members After Deployment: Associations With Mental Health Problems, Aggression, and Fatigue.

INTRODUCTION: Energy drink use has become widespread, particularly by service members, but its association with mental health problems and other behavioral and health problems such as aggression and fatigue is unclear. The present study examines the association between energy drink use and mental health problems, aggressive behaviors, and fatigue in a military population. MATERIALS AND METHODS: At 7 months following a combat deployment, 627 male infantry soldiers were surveyed. Prevalence rates were examined for the frequency (defined as the number of energy drinks consumed per day) and volum

Exercise capacity following SARS-CoV-2 infection is related to changes in cardiovascular and lung function in military personnel.

BACKGROUND: Since the COVID-19 pandemic, post-COVID syndrome (persistent symptoms/complications lasting >12 weeks) continues to pose medical and economic challenges. In military personnel, where optimal fitness is crucial, prolonged limitations affecting their ability to perform duties has occupational and psychological implications, impacting deployability and retention. Research investigating post-COVID syndrome exercise capacity and cardiopulmonary effects in military personnel is limited. METHODS: UK military personnel were recruited from the Defence Medical Services COVID-19 Recovery Ser

Conversion between 2 military combat-related injury coding systems.

Deployable medical systems(DEPMEDS) patient conditions (PCs) and Military Combat Injury Scale (MCIS) are 2 important military medical coding systems. However, both of them have defects when applied in military medical planning. Although each PC code contains information about treatment, intensity of care, treatment time, length of stay, and probability of disposition that is relevant to simulation, its description is too comprehensive and ambiguous to code historical military medical records. Therefore, conversion between PC and other medical coding systems applied in standard medical data is

Skill sets required for the management of military head, face and neck trauma: a multidisciplinary consensus statement.

INTRODUCTION: The evolution of medical practice is resulting in increasing subspecialisation, with head, face and neck (HFN) trauma in a civilian environment usually managed by a combination of surgical specialties working as a team. However, the full combination of HFN specialties commonly available in the NHS may not be available in future UK military-led operations, necessitating the identification of a group of skill sets that could be delivered by one or more deployed surgeons. METHOD: A systematic review was undertaken to identify those surgical procedures performed to treat acute milit

Gulf War veterans and hemifacial microsomia.

BACKGROUND: Concerns have been raised that more infants with Goldenhar syndrome were born to U.S. Gulf War veterans than expected. Goldenhar syndrome is considered a variant of the malformation hemifacial microsomia (HFM). We used data collected from a case-control study of HFM to estimate risk in relation to parental military service and, in particular, Gulf War service. METHODS: Cases with HFM who were three years old or younger were identified at craniofacial clinics in 24 U.S. cities and matched to controls by age and pediatrician. The mothers of 232 cases and 832 controls were interviewe

Association of Combat Experiences With Suicide Attempts Among Active-Duty US Service Members.

IMPORTANCE: There is uncertainty about the role that military deployment experiences play in suicide-related outcomes. Most previous research has defined combat experiences broadly, and a limited number of cross-sectional studies have examined the association between specific combat exposure (eg, killing) and suicide-related outcomes. OBJECTIVE: To prospectively examine combat exposures associated with suicide attempts among active-duty US service members while accounting for demographic, military-specific, and mental health factors. DESIGN, SETTING, AND PARTICIPANTS: This cohort study analy…

Acute Respiratory Distress Syndrome: An Unexpected Outcome of Suspected Viral Gastroenteritis.

Acute respiratory distress syndrome (ARDS) is a life-threatening manifestation of diffuse inflammation damaging the lung pleura. Risk factors for development are numerous with most cases arising in those already hospitalized for critical illness. We describe a unique case of a healthy 20-year-old female developing myocarditis and severe ARDS while hospitalized for septic shock after initially presenting with gastroenteritis from a suspected Coxsackie B infection in the setting of an overseas military deployment. After two transfers via land and air, she reached a facility that delivered defini…

Brucella melitensis infection following military duty in Iraq.

Brucellosis is a common zoonotic disease worldwide; however, few cases are reported in the US. Brucella melitensis infections are primarily acquired via consumption of high-risk foods or travel to endemic areas. We describe a case of B. melitensis infection in a Tennessee soldier following deployment in Iraq. Initial symptoms included knee and back pain. Culture of an aspirate of the left sacroiliac joint yielded B. melitensis. Genetic analysis indicated that this isolate came from the Middle East. Investigation of laboratory workers identified risky exposures and positive serology prompting p

Deployment stressors, gender, and mental health outcomes among Gulf War I veterans.

Findings indicate that war-zone exposure has negative implications for the postdeployment adjustment of veterans; however, most studies have relied on limited conceptualizations of war-zone exposure and focused on male samples. In this study, an array of deployment stressors that were content valid for both female and male Gulf War I military personnel was examined to elucidate gender differences in war-zone exposure and identify gender-based differential associations between stressors and mental health outcomes. While women and men were exposed to both mission-related and interpersonal stress

The Relationship Between Blast-related Hearing Threshold Shift and Insomnia in U.S. Military Personnel.

INTRODUCTION: Hearing loss and insomnia emerged as preeminent sources of morbidity among military service members and veterans who served in the recent Iraq and Afghanistan conflicts. Significant threshold shift (STS), an early indicator of hearing loss, has not been studied in relation to insomnia. This study's objective was to examine the co-occurrence of STS and insomnia among U.S. military personnel with blast-related injury. MATERIALS AND METHODS: A total of 652 service members who were blast-injured during military operations in Iraq or Afghanistan between 2004 and 2012 were identified

Effects of a modular telehealth intervention on comorbid conditions in service members with mild traumatic brain injury.

OBJECTIVE: Many active duty service members with mild traumatic brain injury (mTBI) report comorbidities such as depression, anxiety, PTSD, insomnia, and pain. We analyzed data from a prior randomized controlled trial (RCT) to examine the effects of evidence-based treatment modules, delivered by telephone, on the number and symptom burden of five common comorbidities. SETTING AND PARTICIPANTS: 356 service members from two military medical centers who had sustained deployment-related mTBI in the preceding 2 years. DESIGN: Secondary analysis of RCT comparing 6 months of telephone-delivered pro

Utilization of 19F MRI for Identification of Iraq-Afghanistan War Lung Injury.

INTRODUCTION: There is mounting evidence of respiratory problems related to military service in the Middle East in the past two decades due to environmental exposures during deployment (eg, sand storms and burn pits). This pilot study tests the hypothesis that regional lung function in subjects with prior deployment in Iraq and/or Afghanistan with suspected War Lung Injury (WLI) would be worse than subjects with normal lung function. MATERIALS AND METHODS: Five subjects meeting the inclusion and exclusion criteria were recruited for this pilot study. All subjects underwent spirometry, high-re

Acute neurocognitive deficits in active duty service members following subconcussive blast exposure.

Military service members are frequently subjected to subconcussive blast events during training and deployment. Emerging evidence suggests blast exposures of these magnitudes may have long-term consequences for dimensions of cognitive function. Less is known about cognitive sequelae acutely following deployment-related subconcussive blast events. The current study addressed this knowledge gap by assessing the extent to which subconcussive blast exposure affected performance on the Automated Neuropsychological Assessment Metrics 4 TBI-MIL (ANAM). Baseline-referenced and normative comparisons of

The Impact of Infidelity on Combat-Exposed Service Members.

This study examined relationships between combat-exposed Operation Enduring Freedom/Operation Iraqi Freedom veterans' experiences related to infidelity during deployment (i.e., indicating that a partner was unfaithful or reporting concern about potential infidelity) and postdeployment mental health, as well as the role of subsequent stress exposure and social support in these associations. The sample consisted of 571 individuals (338 men). There were 128 participants (22.2%) who indicated that their partners were unfaithful during their most recent deployment. Of the remaining 443 participants
