The pleural empyema has unusual origin at extralung causes. Then, is essential to identify the primary process in order to obtain the clinical setting control. Previously described, but even exceptional, this case report show an empyema in which it's origin was an asymtomatic pyonephrosis, caused because of a big kidney calculus and it's discovery was suspected by the bacillus results. The treatment must include appropriate antibiotic therapy and thoracic drainage, and also the drain of the purulent accumulations at the kidney and perikidney area.

Stone formation in the double J ureteral catheter is an uncommon complication that generally appears in patients that are predisposed to stone formation and persistent infection of the urinary tract. We report on a 29-year-old female patient in whom stone formation was observed in both ends of the catheter, less than 3 months after it had been placed. Urine culture was positive for P. mirabilis. An analysis of 98 clinical consultations for dysuria in an urban general practice revealed that only 60 percent of patients had proven urinary tract infections. Two percent had gonorrhoea and the rest had an urethral syndrome. Most of the urinary tract infections were caused by Escherichia coli and Streptococcus epidermidis. CASE: A 22-month-old previously healthy woman developed chronic tibial osteomyelitis. Debridement uncovered a 10-cm wound with necrotic tibia, and cultures grew Proteus mirabilis. Intravenous antibiotics transitioned to 6 weeks of oral therapy. Fibular tibialization was performed with K-wire fixation, followed by 2 weeks of postoperative antibiotics. Six-week radiographs showed early callus formation, and K-wires were removed. At the 1-year follow-up, she was fully healed with full knee and ankle range of motion and no pain.
