Rectal versus intramuscular morphine-scopolamine as premedication in children.

Intramuscular morphine-scopolamine for premedication was compared with a hydrogel of the same drugs for rectal administration in 205 healthy children scheduled for minor ENT surgery. The intramuscular dose was 0.15 +/- 0.006 mg x kg-1 compared to 0.25 +/- 0.015 mg x kg-1 rectally. Reaction at administration and anaesthetic induction, incidence of intraoperative airway difficulties, Spo2, ECG changes, postoperative pain and incidence of nausea were recorded. The administration for the rectal hydrogel group worked better and resulted in less postoperative nausea and slightly more postoperative p…

Morphine-induced deficits in sleep patterns: attenuation by glucose.

Morphine effects on many neural and behavioral measures, including tests of learning and memory, are attenuated by increased circulating glucose levels. Using systemic injections, we investigated the ability of glucose to attenuate sleep deficits induced by morphine administration in the rat. Morphine at 1 mg/kg produced a moderate decrease in slow wave sleep which was prevented by concomitant administration of 100 mg/kg of glucose. A higher dose of morphine (10 mg/kg) severely delayed the onset of both slow wave sleep and REM sleep. These delays were attenuated by concurrent administration of…

Morphine-isoflurane interaction in dogs, swine and rhesus monkeys.

In monkeys, dogs and swine (six each) we tested the reduction of the isoflurane MAC (minimal alveolar concentration) produced by 2 mg.kg-1 morphine intravenously (i.v.) and the concurrent effect on PCO2 with spontaneous ventilation. MAC fell to a minimum of 55% of control at 53 min in monkeys, 50% at 38 min in dogs and 13% at 33 min in swine. PaCO2 rose at constant MAC with morphine to 55-60 mmHg, but did not fall over the next several hours despite the decline of plasma morphine concentration, and the resulting needed rise in isoflurane concentration to keep the anaesthesia depth at 1 MAC. Af…

Pentobarbital administered intracerebroventricularly antagonizes morphine-induced antinociception in mice.

The purposes were to show that pentobarbital administered intracerebroventricularly (i.c.v.) antagonized the antinociceptive action of morphine given intrathecally in the tail-flick test in mice, determine whether this antagonistic action involved spinal release of dynorphin A (Dyn A) and demonstrate whether the mechanism involved gamma-aminobutyric acid (GABA) receptors. Previously, it was shown that midazolam given i.c.v. antagonizes morphine-induced antinociception by release of Dyn A in the spinal cord. This antagonism by midazolam is reversed by a variety of benzodiazepine/GABAA receptor …

Morphine-induced alterations in thymocyte subpopulations of B6C3F1 mice.

Morphine has been reported to possess immunosuppressive actions in both in vitro as well as in vivo assays of immune function. Our work in female B6C3F1 mice, surgically implanted with a 75-mg time release morphine pellet, has confirmed previous reports of a rapid loss in the cellularity of the spleen and thymus. To evaluate the effect of morphine on the subpopulations of cells in the thymus, two color fluorescence flow cytometry studies were performed. Fluorescently conjugated monoclonal antibodies specific for the murine cell surface CD4 and CD8 markers were used to identify the four major s…

Dose-response pharmacology of intrathecal morphine in human volunteers.

BACKGROUND: Intrathecal morphine sulfate (ITMS) administration was introduced into clinical practice in 1979. Inadequate information exists delineating ITMS respiratory effects in the dosage range most frequently employed today. This study evaluated 0.2, 0.4, and 0.6 mg ITMS in male volunteers. METHODS: Twenty healthy, young, adult male volunteers received 0.0, 0.2, 0.4, or 0.6 mg preservative-free ITMS in an isobaric solution administered at the L3-L4 interspace in a double-blind randomized fashion. Respiratory function was assessed by finger pulse oximetry (SpO2), respiratory rate, and arte…

Effect of morphine on mesangial immunoglobulin G aggregate kinetics.

Because mesangial expansion is considered a precursor of focal glomerulosclerosis, we studied whether morphine can cause mesangial expansion. We used radiolabeled human immunoglobulin G aggregates (125I-ahIgG) to study mesangial kinetics in control and experimental (morphine-treated) rats. Control and experimental rats were administered 125I-ahIgG by tail vein. Serum levels of 125I-ahIgG and uptake of 125I-ahIgG by liver, spleen, and mesangium were determined at 4, 8, 12, 24, and 36 h after 125I-ahIgG administration. Mesangial 125I-ahIgG levels were higher (P < 0.05) at 4 h and at later period…

5-HT3 antagonists reduce morphine self-administration in rats.

1. The effects of the 5-HT3 receptor antagonists, ondansetron and tropisetron, on morphine consumption were studied in naive and morphine-dependent rats. 2. The administration of ondansetron (1 microgram kg-1, i.p. twice daily) 7 days prior to, and during a 21-day period of, morphine availability (increasing concentration from 0.1 to 0.4 mg ml-1) in 5% sucrose solution reduced opiate intake from the 9th day of morphine treatment. 3. The administration of ondansetron (0.1 microgram kg-1, i.p. twice daily) or tropisetron (0.1 microgram kg-1, i.p. twice daily) on the 14th day of the 21-day period…

Inhibition of epidural morphine-induced pruritus by intravenous droperidol.

BACKGROUND AND OBJECTIVES: Our objective was to study in more detail our previous finding that intravenous droperidol inhibits pruritus induced by epidural morphine. METHODS: In a double blind study, 107 ASA I or II patients undergoing cesarean section received epidural anesthesia with 150 mg (30 ml) of 0.5% bupivacaine with 1:200,000 epinephrine and 2 mg of morphine hydrochloride in a single injection. They were randomly allocated to either receive (treated group, n = 54) or not receive (control group, n = 53) an intravenous injection of 2.5 mg of droperidol just after delivery. During the p…

Intra-articular morphine for pain relief after knee arthroscopy.

We performed a randomised double-blind controlled study in patients undergoing elective knee arthroscopy to assess the effect of intra-articular morphine on postoperative pain relief. Patients in the study group (n = 10) received 5 mg of morphine in a 25 ml dilution intra-articularly while those in the control group (n = 10) received 25 ml of saline. Postoperative pain was assessed at intervals by a visual analogue scale and the requirement for supplementary analgesia was recorded. Those in the study group had significantly lower pain scores and required less systemic analgesics than those in …

Morphine-augmented cholescintigraphy: its efficacy in detecting acute cholecystitis.

Cholescintigrams were performed in 158 patients suspected of having acute cholecystitis after administration of 185 Mbq (5 mCi) of 99mTc-mebrofenin or disofenin. Morphine sulfate, 0.04 mg/kg was given intravenously if there was nonvisualization of the gallbladder at 40-60 min provided that radiotracer was seen within the small bowel. Acute cholecystitis was deemed present if there was nonvisualization of the gallbladder 30 min post-morphine administration; no cystic duct obstruction was present if the gallbladder was demonstrated pre- or post-morphine administration. A final diagnosis was esti…

Relationship between plasma concentration of morphine and analgesic effectiveness.

Morphine is a key drug for cancer pain management. In this study, we analyzed plasma morphine concentration in cancer patients who received continuous morphine drip and/or oral administrations of morphine: (1) The plasma concentration of morphine varied widely in patients whose pain was satisfactorily controlled at a constant dosage of morphine. The absolute value of the plasma concentration of morphine necessary for effective pain control could not be estimated; (2) When MS Contin was given at the doses 20 mg, 30 mg, 40 mg, the plasma concentration of morphine increased with increasing dose. …

Morphine at gramme doses: kinetics, dynamics and clinical need.

The MS Contin tablet 200 mg (controlled release morphine sulphate) provides an equivalent rate and extent of absorption to two MS Contin tablets 100 mg. The characteristics of controlled release from this higher strength formulation are also consistent with those of the 100 mg tablet. The level of analgesia provided during the early post-dosing period, at the end of the dosing period and overall over the twelve hour interval were also equivalent between the preparations. The addition of the MS Contin tablet 200 mg to the existing range will facilitate oral dosing in patients requiring large do…

Morphine-6-glucuronide: effects on ventilation in normal volunteers.

The respiratory responses to intravenous morphine sulphate (0.12 mg/kg), morphine-6-glucuronide (M6G: 0.03 mg/kg) and placebo were assessed in 6 healthy volunteers, using a single blind randomised crossover design. Five of these subjects underwent an additional study of M6G at 0.06 mg/kg. Respiratory rate, minute volume and end-tidal CO2 were continuously measured using a low resistance non-rebreathing circuit, a mass spectrometer and a dry gas meter. The ventilatory responses to CO2 exposures (5.5% for 4 min) were assessed 40 and 20 min before, and 20, 40 and 80 min after drug administration.…

Morphine and ibuprofen compared using the cold pressor test.

The analgesic efficacy of single doses of oral morphine sulphate solution (10 mg) and ibuprofen 600 mg was compared in 12 volunteers using a double-blind, double-dummy, placebo-controlled design on the cold pressor experimental pain model. Measurement of pain intensity was made before medication and then at 30, 60, 90, 120 and 180 min; blood samples were taken at these times for measurement of morphine and glucuronide metabolites by radioimmunoassay. Sessions were at least 5 days apart. Correlations were sought between analgesic effect and plasma concentrations of either morphine or morphine-6…

Constant morphine infusion for severe sickle cell crisis pain.

The use of a constant infusion of intravenous morphine sulfate in a patient with severe sickle cell crisis is described. After several days of poor control with intramuscular and intravenous narcotic injections, adequate analgesia was obtained with the infusion of morphine within two hours of initiation of therapy. No adverse effects were noted. With the advantages provided by an intravenous narcotic infusion, this protocol should be considered as a suitable alternative to conventional methods for providing pain control in patients in sickle cell crisis.

Brain peptide reverses effect of morphine on human lymphocytes.

E-rosette formation by human lymphocytes incubated with sheep red blood cells (sRBC) is inhibited by morphine. We studied the ability of the opiate antagonists naloxone and Tyr-MIF-1 (Tyr-Pro-Leu-Gly-NH2) to block this action. Active E-rosette formation by lymphocytes incubated with morphine was reduced from the control of 35.7 +/- 1.7% to 23.7 +/- 1.5% (p less than 0.001). Similarly, total E-rosette formation was reduced by morphine from the control of 65.8 +/- 1.3% to 53.2 +/- 2.9% (p less than 0.001). These effects were blocked by co-incubation of the lymphocytes with either Tyr-MIF-1 or na…

Epidural morphine for postoperative pain relief in anorectal surgery.

Postoperative analgesia and side effects of a single dose of 20 ml of 2% epidural prilocaine followed immediately by either morphine (3 mg in 3 ml saline) or saline (3 ml) were studied in 79 patients scheduled for anorectal surgery. Pain relief was evaluated by a linear scale for 24 hr and by an assessment of the requirement for parenteral analgesics. Of the patients given epidural morphine, 25% did not require analgesics, whereas all patients given prilocaine alone asked for additional analgesia. Remaining patients given morphine were pain-free for a mean duration of 7.6 +/- 5.1 hr (SD). Only…

Spinal adenosine modulates descending antinociceptive pathways stimulated by morphine.

Adenosine-mediated analgesia and interactions between opioids and adenosine in the brain have been observed by several researchers. Our investigations were designed to examine opioid-adenosine interactions at spinal sites and possible modulation of opioid-stimulated descending antinociceptive pathways by adenosine in the spinal cord. Methylxanthines administered intrathecally (i.t.) were used as adenosine receptor antagonists to determine possible interactions between opioids and endogenous adenosine. Theophylline administered i.t. dose-dependently antagonized analgesia induced by morphine adm…
