Values of radiological examinations for the diagnosis and prognosis of invasive bronchial-pulmonary aspergillosis in critically ill patients with chronic obstructive pulmonary diseases.

INTRODUCTION: The important role of radiological examinations of invasive pulmonary aspergillosis (IPA) in patients with neutropenia has been well studied; however, little is known about IPA in critically ill chronic obstructive pulmonary disease (COPD) patients. OBJECTIVES: To evaluate the value of radiological examinations in the diagnosis and prognosis of invasive bronchial-pulmonary aspergillosis (IBPA) in critically ill COPD patients. METHODS: We included 61 critically ill COPD patients in the intensive care unit (ICU) in a retrospective, single-center cohort study. RESULTS: All of the

Pneumocystis pneumonia causing cavitating lung nodules in an immunocompetent individual.

Pneumocystis jirovecii pneumonia (PCP) is a potential life-threatening pulmonary infection which commonly manifests in immunosuppressed patients especially with HIV, with underlying malignancies, severe malnutrition as well as those on immunosuppressive treatments. There have been case reports of symptomatic PCP in individuals with a normally functioning immune system with typical clinical features and radiologic findings of bilateral and diffuse interstitial opacities. However, PCP in immunocompetent individuals presenting with lung nodules had been rarely reported. We report a 53-year-old im

Malignant mesothelioma masquerading as a multinodular bronchioloalveolar cell adenocarcinoma with widespread pulmonary nodules.

A 72-year-old man had a unilateral pleural effusion and multiple bilateral pulmonary nodules. Thoracoscopic biopsy revealed multiple discrete nodules in the pleura and lung. The latter consisted of tall columnar malignant cells arranged on alveolar surfaces in a lepidic growth pattern. Mucin filled the alveolar lumina, both in the nodules and surrounding lung. It stained with Alcian blue but not with periodic acid Schiff, suggesting that it was a glycosaminoglycan, which was confirmed as hyaluronic acid by complete digestion with hyaluronidase. Tumor cells were calretinin, Wilms tumor-1, and h

Cutaneous myxosarcoma with pulmonary metastases in a dog.

The pathological findings in a 13-year-old dog with cutaneous myxosarcoma with pulmonary metastases are described. Grossly, there was a large subcutaneous mass in the right scapular region, a smaller nodule in the caudal abdominal region and a fibrotic mass at a fracture site in the right hindlimb. Radiographic examination revealed several pulmonary nodules. Microscopical evaluation revealed a myxosarcoma characterized by the proliferation of spindle to stellate cells with multiple prominent nucleoli and vascular invasion. The neoplastic cells were haphazardly arranged in a mucopolysaccharide

The undiagnosed potential clinically significant incidental findings of neck CTA: A large retrospective single-center study.

To assess the prevalence and missed reporting rate of potential clinically-significant incidental findings (IFs) in the neck CTA scans. All consecutive patients undergoing neck CTA imaging, from January 1, 2017 to December 31, 2018, were retrospectively evaluated by a radiologist for the presence of incidental findings in the upper chest, lower head and neck regions. These incidental findings were subsequently classified into 3 categories in terms of clinical significance: Type I, highly significant, Type II, moderately significant; and Type III, mildly or not significant. Type I and Type II IF

Posterior mediastinal nodule diagnosed as a tuberculous granuloma infiltrating into the aorta.

Although tuberculous infection rarely results in aortic aneurysm formation or rupture, its precursor lesion has never been identified in cases with tuberculosis. We incidentally encountered a case of a posterior mediastinal nodule with concomitant chest computed tomography (CT) findings of multiple pulmonary micronodular shadows. Since an enlargement of the mediastinal nodule was retrospectively apparent, we considered the lesion as malignant. Enhanced CT showed luminal irregularity in the descending aorta, located adjacent to the nodule, in addition to the disappearance of the fat plane betwe

Associations of derived inflammatory, lipid, and anthropometric indices with CT-detected pulmonary nodules: a hospital-based cross-sectional study with explainable machine learning.

BACKGROUND: Pulmonary nodules are frequently detected during routine health-examination chest computed tomography (CT). From a systems-endocrinology perspective, routinely available indices integrating adiposity distribution, lipid metabolism, immune-cell balance, and low-grade inflammation may characterize cross-organ metabolic-inflammatory phenotypes, but their associations with CT-detected pulmonary nodules remain uncertain. METHODS: This hospital-based cross-sectional study included 1,072 adults undergoing routine health examinations at the Second Affiliated Hospital of Shandong First Med

Pulmonary nodules near fissures in hepatocellular carcinoma: assessing their clinical significance.

OBJECTIVE: To evaluate whether pulmonary nodules adjacent to fissures, particularly perifissural nodules detected on chest computed tomography in patients with hepatocellular carcinoma, can be reliably classified as benign based on their radiological characteristics. METHODS: This retrospective cohort study included patients diagnosed with hepatocellular carcinoma (LI-RADS 5) between 2018 and 2022 at two reference hospitals. The patients underwent at least two follow-up chest computed tomography scans, with a minimum interval of 1 year, to identify at least one pulmonary nodule adjacent to th

The psychological disorder and personality traits of individuals with pulmonary nodules.

INTRODUCTION: With the widespread use of Low-dose computed tomography (LDCT) in the chest, more and more people will be detected with pulmonary nodules. The presence of uncertainty following the detection of these nodules can impose significant psychological distress. This study aimed to investigate personality traits, psychological distress, and their impact on pulmonary nodule patients in China. METHODS: We conducted a cross-sectional survey of adults with pulmonary nodules accidently discovered by LDCT in the chest from the respiratory outpatient department. RESULTS: A total of 224 patien

Differentiating Pulmonary Nodule Malignancy Using Exhaled Volatile Organic Compounds: A Prospective Observational Study.

BACKGROUND: Advances in imaging technology have enhanced the detection of pulmonary nodules. However, determining malignancy often requires invasive procedures or repeated radiation exposure, underscoring the need for safer, noninvasive diagnostic alternatives. Analyzing exhaled volatile organic compounds (VOCs) shows promise, yet its effectiveness in assessing the malignancy of pulmonary nodules remains underexplored. METHODS: Employing a prospective study design from June 2023 to January 2024 at the Affiliated Hospital of Yangzhou University, we assessed the malignancy of pulmonary nodules

A comparison of an integrated and image-only deep learning model for predicting the disappearance of indeterminate pulmonary nodules.

BACKGROUND: Indeterminate pulmonary nodules (IPNs) require follow-up CT to assess potential growth; however, benign nodules may disappear. Accurately predicting whether IPNs will resolve is a challenge for radiologists. Therefore, we aim to utilize deep-learning (DL) methods to predict the disappearance of IPNs. MATERIAL AND METHODS: This retrospective study utilized data from the Dutch-Belgian Randomized Lung Cancer Screening Trial (NELSON) and Imaging in Lifelines (ImaLife) cohort. Participants underwent follow-up CT to determine the evolution of baseline IPNs. The NELSON data was used for

Trans-scapular approach coil localization for scapular-blocked pulmonary nodules: a retrospective study.

BACKGROUND: Preoperative computed tomography (CT)-guided coil localization (CL) is commonly used to facilitate video-assisted thoracoscopic surgery (VATS)-guided diagnostic wedge resection (WR) of pulmonary nodules (PNs). When a scapular-blocked PN (SBPN) is localized, the trans-scapular CL (TSCL) is commonly performed. In this study, we investigated the safety, feasibility, and clinical efficacy of preoperative CT-guided TSCL for SBPNs. MATERIALS AND METHODS: From January 2014 to September 2020, a total of 152 patients with PNs underwent CT-guided CL prior to VATS-guided WR. Of these patient

Cannon Ball Diagnosis and Management in a Flight Attendant Candidate.

BACKGROUND: ''Cannon ball'' opacities on chest X-rays are a common manifestation of hematogenous dissemination of a malignant tumor in the lungs. They indicate an advanced stage of disease with a very grim prognosis in terms of cure and survival. In aerospace medicine, this aspect means the patient is unfit for flight duties. Nonmetastatic etiologies of pulmonary nodules are rare. We report a case of cannon ball opacities discovered fortuitously during an admission visit. CASE REPORT: A 23-yr-old flight attendant candidate came to the Aeromedical Expertise Center of the Military Hospital in R

A benign metastasizing leiomyoma involving a nodule in the pulmonary artery: case and literature review.

Benign metastasizing leiomyoma (BML) is a rare disease defined as a primary benign uterine tumor with "metastatic" lesions preferentially occurring in the lung, pelvis and lymph nodes. There are few reports about local recurrence after initial surgery. We report a case of a BML with local recurrence and metastasis growing into the wall of the left pulmonary artery, diagnosed 11 years after initial hysterectomy. A 55-year-old woman complaining of abdominal discomfort, heaviness and asthenia was admitted to our hospital for investigation of a voluminous uterine mass with high vascularization and
