WDR44 Loss-of-Function Promoter Deletion in a Male Newborn With a Ciliopathy Phenotype.

Gain-of-function variants in the WDR44 gene have recently been associated with an X-linked ciliopathy-related neurodevelopmental phenotype. Here, we report on a WDR44 loss-of-function (LOF) variant identified in the genome sequence from a male fetus enrolled in the Prenatal Genetic Diagnosis by Genomic Sequencing (PrenatalSEQ) multicenter study. The phenotype is consistent with the described X-linked ciliopathy that includes developmental delay, microcephaly, congenital heart defects, kidney abnormalities, cryptorchidism, musculoskeletal abnormalities, craniofacial dysmorphism, and effusions. …

MR imaging of soft-tissue masses of the foot.

As with other parts of the musculoskeletal system, the soft tissues of the foot can be affected by a wide variety of pathologic entities including trauma, congenital abnormalities, infections, and neoplastic disorders. While plain radiographs are usually the initial examination for evaluation of pathology, magnetic resonance imaging (MRI) is critical to evaluate for abnormalities within the ligaments, tendons, and other nonosseous structures within the foot. The constellation of clinical and MRI findings often allows a relatively specific diagnosis to be rendered. This article discusses both b

Neuropathic symptoms and musculoskeletal pain in carpal tunnel syndrome: prognostic and therapeutic implications.

BACKGROUND: Repetitive activities in the workplace can not only jeopardize the median nerve across the carpal tunnel but also the musculoskeletal structures, such as the ligaments, synovia, tendons and muscles producing pain, and local tenderness at the wrist, elbow, and shoulder. The occurrence of this latter condition can dominate the overall clinical picture and affect the outcome of treatments for this very common condition. METHODS: A clinical evaluation of 30 patients who previously underwent a successful and uncomplicated carpal tunnel release surgery was conducted. These patients were

Painful spondylolisthesis in a two-year-old child.

Spondylolisthesis is considered one of the common causes of back pain in children. There are far fewer cases reported in children under the age of 4 years. However, all patients described were diagnosed by radiographic examination in a workup for other congenital or musculoskeletal abnormalities. This report describes a 2-year-old girl who presented with low back pain and was subsequently diagnosed with spondylolisthesis.

Spectrum of pediatric developmental and genetic renal lesions and associated congenital malformations--an autopsy study from north India.

Pediatric developmental and genetic renal lesions are a known cause of mortality in the perinatal/neonatal period. These lesions are associated with a wide range of extrarenal congenital malformations that influence the outcome of the patients. In this autopsy study, we have analyzed the spectrum of pediatric developmental and genetic renal lesions and their associated congenital malformations. A total of 4,099 autopsies (20 weeks of gestation to 1 year of life) were reviewed, of which 158 cases (3.85%) of pediatric developmental (143 cases) and genetic renal lesions (15 cases) were found. Assessment of the relationship between knee ultrasound and clinical symptoms in patients with thyroid dysfunction.

OBJECTIVE: To determine whether musculoskeletal ultrasound (MSUS) abnormalities were associated with thyroid dysfunction. METHODS: This study included 109 patients with thyroid diseases, including thyroid dysfunction or autoimmune thyroid disease. Patients were categorized as euthyroid, hypothyroid, or hyperthyroid based on their recent thyroid function tests. To evaluate MSUS, an experienced rheumatologist examined the presence of synovial fluid, synovial hypertrophy, and grade of inflammation in both gray-scale and power Doppler scans of the knee joint. Associations between MSUS abnormaliti

Severe musculoskeletal symptoms during continuous infusion of bumetanide.

BACKGROUND: The continuous infusion of the loop diuretic, bumetanide, has been shown to be effective in the treatment of severe refractory edema. Musculoskeletal symptoms have been reported with bumetanide following both oral and intermittent i.v. bolus therapy. It has been suggested that these adverse effects do not occur with continuous infusion. We describe, however, a series of patients with heart failure who developed severe disabling musculoskeletal symptoms during the continuous infusion of humetanide. OBJECTIVE: To evaluate the characteristics of bumetanide-induced musculoskeletal sym

Metabolic bone disease (anticonvulsant osteomalacia) and renal tubular acidosis in tuberous sclerosis.

The characteristics triad of tuberous sclerosis-adenoma sebaceum, mental deficiency and epilepsy-associated with distal-type renal tubular acidosis was combined with anticonvulsant osteomalacia in a 41-year-old woman. In addition to the specific bone lesions of tuberous sclerosis, the bone disease was caused by an adverse effect of a drug and possibly also by the renal disorder leading to significant musculoskeletal disability. In response to calcium carbonicum and 1-25-dihydroxyvitamin D therapy the musculoskeletal disability healed and the abnormal biochemical markers of anticonvulsant osteo

Clinical examination of musculoskeletal groin pain: a simple protocol for hernia surgeons (Groin-MAP).

PURPOSE: Patients with groin pain are often referred to hernia surgeons because of imaging-detected inguinal abnormalities, even when no hernia is palpable. Many of these patients have musculoskeletal pathology that would not benefit from surgical intervention. This project aimed to develop a rapid, evidence-informed musculoskeletal screening protocol for use as an adjunct to routine hernia assessment. METHODS: The protocol was developed in four phases: (1) Structured PubMed search for clinical examination tests relevant to each of the entities defined by the 2016 Doha agreement classificatio

Soft tissue pathology: regional pain syndromes, nerves and ligaments.

Musculoskeletal ultrasonography (MSUS) is a useful imaging technique in the diagnosis of various soft tissue pathologies. High-frequency linear array transducers provide excellent resolution of soft tissue pathology. Pathological changes in subcutaneous tissue, including soft tissue tumours, abscesses, tenosynovitis, ligamentous and tendinous abnormalities, and peripheral nerve lesions, including carpal tunnel syndrome, can be identified. This review addresses the role of US in diagnosing regional pain syndrome, ligament lesions and nerve lesions.

MUCOLIPIDOSIS II INFANTS PRESENTING WITH SKELETAL DEFORMITIES MIMICKING RICKETS AND A NEW MUTATION IN GNPTAB GENE.

Mucolipidosis II or I-cell disease is a rare lysosomal enzyme hydrolase trafficking due to deficient activity of the multimeric enzyme UDP-Nacetylglucosamine-l-phosphotransferase. It is a severe inborn error of lysosomal storage that causes progressive multisystem deterioration and death within the first year of life. The diagnosis of ML II is often difficult in an infant due to clinical variety, phenotypic overlap and the enzyme analysis required. Mucolipidosis II and rickets may have similar physical, biochemical and radiographic findings in newborns. The diagnosis of Mucolipidosis II is oft

Isolated ectrodactyly in a newborn with Down syndrome.

Down syndrome (DS), trisomy 21, is the most common numerical chromosome abnormality among live born infants. Dysmorphic features, congenital malformations, cognitive impairment are major features. Musculoskeletal anomalies are associated with abnormal collagen function. Ectrodactyly is characterized by a deep median cleft of the hand and/or foot. Failure of apical ectodermal ridge while developing limbs leads to ectrodactyly. Here we report a patient diagnosed with DS and ectrodactyly which were split between the third and fourth fingers and there were 4 fingers on both hands. There was no add

Development of an evidence-based clinical guideline for age-appropriate screening, prevention, and management of bone abnormalities in children post-hematopoietic stem cell transplant.

Musculoskeletal complaints, pain, and weakness are common among children post-hematopoietic stem cell transplant (HSCT). Bone abnormalities include decreased bone mineral density and avascular necrosis, both affecting ambulation and quality of life for survivors. Several risk factors for both disorders among adults have been described along with suggested interventions. However, similar recommendations for screening and management of bone abnormalities among children and adolescents post-HSCT are not clearly defined. Orthoplastic Approach for Surgical Treatment of Diabetic Foot Ulcers.

Diabetic foot ulcers remain a challenge to practitioners and costly to health care systems. They range from simple to complex and many treatment options are available. By following standard wound care protocols to ensure adequate blood supply, infection control, debridement, wound management, and offloading many diabetic foot ulcers heal in a timely fashion. For those that do not, an underlying foot and ankle musculoskeletal abnormality must be suspected and treated concomitantly with soft tissue treatment. This article presents a systematic orthoplastics approach for surgical treatment of dia
