When to suspect and how properly early detect and treat patients with endemic mycoses.

Endemic mycoses are caused by dimorphic fungi and eventually molds, as the case of implantation mycoses. In general, these diseases are acquired through trauma or inhalation of fungal elements in the environment, and less frequently by zoonotic acquisition or transmitted during organ transplantation. The target population for endemic mycoses is usually represented by normal hosts with low-income and intensive outdoor activities. Awareness of these diseases remains limited, even in regions with high prevalence, resulting in delayed diagnosis, and affecting the quality of life and outcomes of pa…

Is Pulmonary Mycoses Shadowed by Tuberculosis? Mandate to Hit the Bull's Eye-An Indian Perspective.

Tuberculosis (TB) poses serious challenges to healthcare personnel as it continues to be highly prevalent in a developing country like India. Several prominent strategies have been adopted to control this contagious infection, but the incidence rate remains high. Many studies have linked fungal infections to TB, apparently elevating concerns in the TB elimination program. Hence, it is essential to understand the mechanisms underlying TB co-infection and pulmonary mycoses to combat the problems caused by these diseases successfully. In addition, differential diagnosis of TB and fungal infection…

[Prophylactic and empirical treatment of mycoses in neutropenic fever. Report and comments on meta-analysis].

Patients treated for cancer with chemotherapy and other cytoreductive therapy often develop serious bacterial, viral, and fungal infections due to B- and T-cell depletion, neutropenia and decreased barrier function of mucosal membranes. In patients with neutropenic fever not responding to broad spectrum antibiotic therapy there is a high risk of fungal infection. In a Cochrane Library meta-analysis of the effect of prophylactic and empirical antifungal treatment, amphotericin B was found to be the only drug affecting total mortality. Fluconazole, ketoconazole and itraconazole affected risk of

Epidemiology of visceral mycoses: analysis of data in annual of the pathological autopsy cases in Japan.

The data on visceral mycoses that had been reported in the Annual of the Pathological Autopsy Cases in Japan from 1969 to 1994 by the Japanese Society of Pathology were analyzed epidemiologically. The frequency of visceral mycoses among the annual total number of pathological autopsy cases increased noticeably from 1.60% in 1969 to a peak of 4.66% in 1990. Among them, the incidences of candidiasis and aspergillosis increased the most. After 1990, however, the frequency of visceral mycoses decreased gradually. Until 1989, the predominant causative agent was Candida, followed in order by Aspergi…

[Prevention of opportunistic infections in HIV seropositive patients. Prevention of viral infections and mycoses].

CYTOMEGALOVIRUS INFECTION: Primary prophylaxis is logical in high-risk subjects (CD4 < 50) who are asymptomatic; per os ganciclovir is under evaluation. Systematic secondary prophylaxis relies on intravenous ganciclovir or foscarnet. HERPES SIMPLEX AND ZOSTER: Secondary prevention with acyclovir is not recommended due to the risk of resistance. Herpes zoster retinitis is an absolute indication for continuous oral treatment with acyclovir. CRYPTOCOCCUS: Oral fluconazol is required for secondary prophylaxis. CANDIDIASIS: Due to the risk of resistant strains, neither primary nor secondary prophyl

[Clinical manifestations of various mycoses of the upper respiratory and digestive tracts in African pathology].

Mycotic infections are prevalent in tropical Africa, and are sometimes observed North and South of this zone, but they are usually localized to the lower limbs (mycetoma), without manifestations in the ORL sphere. However, various ubiquitous superficial mycoses are also encountered in Africa, particularly Aspergilloses and Candidoses. Their clinical expression (apart from several cases of pseudo-tumoral sinusal aspergillosis...) differs only slightly from that observed and treated everywhere, and they are not discussed. Also excluded are several cases of North-American Blastomycosis reported N…

Time to re-evaluate combination antifungal therapy for invasive aspergillosis and other invasive mycoses.

Mortality from invasive aspergillosis remains unacceptably high, and the currently recommended treatment of triazoles is further challenged by rising resistance to these first-line agents. A previous randomised controlled trial of combination therapy with voriconazole plus anidulafungin versus voriconazole alone showed that the combination was safe and associated with a clinically significant reduction in all-cause mortality that, however, failed narrowly to meet conventional levels of significance. In this Personal View, we argue against a binary, negative interpretation of these results, and

Clinical Utility and Diagnostic Performance of (1→3)-β-D-Glucan in Invasive Mycoses.

BACKGROUND: The BDG assay is a non-culture-based biomarker widely used in the diagnosis of invasive fungal infections, particularly among immunocompromised patients. Conventional diagnostic methods, including culture and histopathology, are limited by low sensitivity and delayed turnaround times, prompting increased reliance on adjunctive serologic markers. METHODS: BDG, a conserved component of the fungal cell wall, is detectable in serum across multiple clinically relevant fungi, including Candida spp., Aspergillus spp., and Pneumocystis jirovecii, but is absent or minimally expressed in or

Systemic Mycoses by Novel Onygenalean Fungal Pathogens Emergomyces spp and Blastomyces percursus in Rwanda.

We report 2 cases of infection by fungi unprecedented in Rwanda. One patient with emergomycosis presented with disseminated disease and prominent cutaneous involvement and one patient with African blastomycosis had cutaneous and osseous disease. These cases illustrate the clinicopathologic and molecular traits of novel dimorphic onygenalean species in Rwanda.

Adjunctive methylene blue antimicrobial photodynamic therapy for mucocutaneous lesions of mycoses: three case reports.

Dermatophytosis, paracoccidioidomycosis and sporotrichosis are mycoses caused by different fungal species with significant prevalence in Brazil and other countries. In some situations, they affect quality of life, especially in the most vulnerable populations. Antifungal drug therapy is the conventional treatment for these diseases, although some difficulties may occur. Adjunctive use of antimicrobial photodynamic therapy (aPDT) may reduce these challenges. Three patients were treated with aPDT and conventional antifungals. In all cases, the patients did not report pain, discomfort or side eff…

[Rapid identification and susceptibility to killer toxins of yeasts isolated from non-systemic mycoses].

Rapid identification and susceptibility to killer toxins of yeasts isolated from non-systemic mycoses. The use of quick and reliable yeast identification methods, as well as the development of new antifungal agents with more specific targets, will enable a more efficient treatment of mycoses. In the present work, a total of 53 clinical isolates obtained from non-systemic infections in Neuquén Hospitals and an ophthalmologic clinic in Buenos Aires during 2005, were identified by means of a rapid molecular method (ITS1-5.8S ADNr-ITS2 PCR-RFLP). Additionally, the killer susceptibility of the isol…

Deep mycoses and palatal perforation with granulomatous pansinusitis in acquired immunodeficiency syndrome: case reports.

Palatal perforation with granulomatous pansinusitis caused by deep mycotic infection is a rare complication of acquired immunodeficiency syndrome. Two cases of palatal ulceration with osseous destruction, one caused by Histoplasma capsulatum the other by Cryptococcus neoformans, are discussed. Treatment with a palatal obturator restored function and speech in both patients during the remainder of their lives.
